The apron of the greater omentum of gastric cancer patients contains various lymphoid structures including lymph nodes

•The omental apron (at least of gastric cancer patients) can, in addition to the commonly known omental milky spots, contain other lymphoid structures as well.•These lymphoid structures represented lymph nodes and intermediate lymphoid structures, the latter being structures that did not did not meet the criteria to classify as lymph nodes or omental milky spots.•Both omental lymph nodes and intermediate lymphoid structures might represent activated omental milky spots that have gained lymph node like morphological characteristics.•Pathologists might wrongly identify these lymphoid structures as true lymph nodes and thereby mistake metastases in these structures as local spread instead of peritoneal spread.
