•Most people who inject drugs in the rural U.S. inject stimulants, alone or with opioids.•HCV antibody prevalence was highest among recent injection of opioids, alone or with stimulants.•HCV antibody prevalence was lowest among those who recently injected only stimulants.•Among a subsample with positive antibodies, 74% tested with current HCV infection.•Injection rates were highest for same-syringe injection of opioids and stimulants.
