•Current guidelines recommend minimally invasive (MIS) inguinal hernia repair for females to assess for femoral hernias•MIS approaches detected more femoral hernias compared to Lichtenstein approaches, but most were <1.5 cm•There was no difference in femoral hernias >3 cm identified during MIS or Lichtenstein repairs•Female patients experienced similar perioperative and recurrence outcomes with MIS and Lichtenstein repairs
