Breast cancer during pregnancy-what are the special features of diagnostics and therapy. A challenge for all

Due to the fact that more and more women are having children after the age of 30, the incidence of breast cancer during pregnancy is increasing. The diagnosis during pregnancy can be made using sonography and mammography, but the staging procedure differs from the standard. Depending on the tumor biology and tumor stage, the therapy recommendation should be as close as possible to that for a non-pregnant patient. Both chemotherapy and surgery can be carried out in the usual way. Future family planning should be discussed with the patient and, if necessary, taken into account in the therapy planning. Radiation therapy is usually given postpartum. The following therapies are contraindicated both during pregnancy and while breastfeeding: endocrine, antibodies, cyclin-dependent kinase inhibitors (CDK4/6i), poly adenosine diphosphate-ribose polymerase inhibitors (PARPi), and bisphosphonates. Therefore, postpartum weaning must be performed. The patient should be cared for by an interdisciplinary team at a breast cancer center.
