Outcomes of Living Donor Liver Transplantation Compared with Deceased Donor Liver Transplantation

Liver transplantation is life saving and serving the greatest number of candidates in a timely manner is important. LDLT is more technically challenging given the anatom-ical factors, leading to more biliary and vascular complications. However, the major-ity of internation and domestic data demonstrate that LDLT is non-inferior to DBD-DDLT and DCD-DDLT after 30 days of graft and patient survival. Although currently, many centers with less experience, favor LDLT in lower MELD patients, we must pro-vide the best care possible for all of our patients. Therefore, LDLT has been used in patients with MELD greater than 11 with careful consideration for donor and recipient factors to maximize their survival benefit. Over time, we hope that complex high -MELD patients without access to DDLT, will eventually be given quick access to LDLT when safe.
