•Severe valve calcification increases risk of annular rupture during transcatheter aortic valve replacement.•A novel L6 lithotripsy balloon was used to modify valve calcium prior to transcatheter aortic valve replacement.•Lithotripsy may enhance safety and effectiveness of transcatheter aortic valve replacement with severe calcium.
