Bull’s eye pattern of aberrometry in anterior lenticonus

We report an 18-year-old male patient with a progressive diminution of vision in both eyes. On examination, he had bilateral anterior lenticonus and sensorineural hearing loss. Slit–lamp examination finding of the anterior lenticonus was confirmed by wave front aberrometry on i-Trace. A diagnosis of Alport syndrome was made. Normal corneal topography and elevation maps with high total eye aberrations pointed to internal optics as the source of aberrations with predominant negative spherical aberrations. Uneventful phacoemulsification with intraocular lens implantation was performed. A significant reduction in total aberrations was noticed, and the patient’s visual disturbances were alleviated.
