•Among 1068 US hospitals in 2020, more than 85% could perform intracoronary imaging.•Despite this, the median use of intracoronary imaging for percutaneous coronary intervention was 6.3%.•There was substantial hospital variability in the use of intracoronary imaging for percutaneous coronary intervention.
