•Considerable variation in the management of phyllodes tumours and breast sarcoma.•A proportion of breast sarcomas are managed in trusts that do not hold sarcoma MDT.•Lack of consensus on breast surgical margins and axillary surgery indications.•Diverse practice with regards to adjuvant radiotherapy and chemotherapy indications.•Urgent need for consensus-based guidelines and centralisation of management.
