The majority of guidelines recommend RBUS after first febrile UTI despite the low yield for significant findings in most children that are screened. To this end, the authors main study goal was to apply a more selective use of RBUS after febrile UTI based on recent evidence that shows an association between the presence of non-E.coli UTI and likelihood for VUR (manuscript references 15,16, 17). The authors assessed the prevalence of undetected kidney abnormalities on RBUS after first febrile UTI with non-Ecoli pathogens or after recurrent UTI, irrespective of pathogen, and compared this approach with the standard evaluation recommended by the AAP guidelines that call for RBUS in all children after febrile UTI [1].
