Associations ofPrenatalPer- andPolyfluoroalkylSubstance(PFAS) Exposureswith Offspring AdiposityandBodyCompositionat16-20 YearsofAge: ProjectViva

BACKGROUND: Findings ontheassociationsbetweenprenatalPFASexposuresandoffspring adiposityareinconsistent. Whethersuchassociations may extendtoadolescenceisespeciallyunderstudied. OBJECTIVES: We investigatedassociationsofprenatalPFASexposureswithoffspring adiposityandbodycompositionat16-20 yearsofage. METHODS: We studied545mother-child pairsintheprospectiveprebirthcohortProjectViva(Boston, Massachusetts). WemeasuredsixPFAS (PFOA, PFOS, PFNA, PFHxS, EtFOSAA, andMeFOSAA) inmaternalearlypregnancy(medianage= 9:6 wk, range: 5.7-19.6 wk) plasmasamples. At thelateadolescencevisit(medianage= 17:4 y, range: 15.9-20.0 y), weobtainedanthropometricmeasuresandassessedbodycompositionusing bioelectrical impedanceanalysisanddual-energyX-rayabsorptiometry. WeexaminedassociationsofindividualPFASwithobesity[i.e., age- and sex-specific bodymassindex(BMI) = 95th percentile] andadiposityandbodycompositionusingmultivariablePoissonandlinearregressionmodels, respectively. WeassessedPFASmixtureeffects usingBayesiankernelmachineregression(BKMR) andquantileg-computation. Weusedfractionalpolynomial modelstoassessBMItrajectories(at3-20 yearsofage) byprenatalPFASlevels. RESULTS: Thirteen percent(n= 73) ofthechildrenhadobesityinlateadolescence. Aftermultivariableadjustment, higherprenatalPFASconcentrations wereassociatedwithhigherobesityrisk[e.g., 1.59(95% CI: 1.19,2.12), 1.24(95% CI: 0.98,1.57), and1.49(95% CI: 1.11,1.99) timestheobesity riskperdoublingofPFOS, PFOA, andPFNA, respectively]. BKMRshowedaninteractionbetweenPFOAandPFOS, wherethepositive association betweenPFOSandobesitywasstrongerwhenPFOAlevelswerelower. EachquartileincrementofthePFASmixturewasassociated with 1.52(95% CI: 1.03,2.25) timestheobesityriskand0.52(95% CI: -0:02, 1.06) kg/m(2) higher BMI. ChildrenwithhigherprenatalPFOS, EtFOSAA, andMeFOSAAconcentrationshadhigherratesofBMIincreasestartingfrom9-11 yearsofage.
