AbstractPurpose: Aromatase inhibitor (AI)–associated musculoskeletal symptoms (AIMSS) are common and frequently lead to AI discontinuation. SNPs in candidate genes have been associated with AIMSS and AI discontinuation. E1Z11 is a prospective cohort study designed to validate the association between 10 SNPs and AI discontinuation due to AIMSS. Patients and Methods: Postmenopausal women with stage I to III hormone receptor–positive breast cancer received anastrozole 1 mg daily and completed patient-reported outcome measures to assess AIMSS (Stanford Health Assessment Questionnaire) at baseline, 3, 6, 9, and 12 months. We estimated that 40% of participants would develop AIMSS and 25% would discontinue AI treatment within 12 months. Enrollment of 1,000 women with a fixed number per racial stratum provided 80% power to detect an effect size of 1.5 to 4. SNPs were found in ESR1 (rs2234693, rs2347868, and rs9340835), CYP19A1 (rs1062033 and rs4646), TCL1A (rs11849538, rs2369049, rs7158782, and rs7159713), and HTR2A (rs2296972). Results: Of the 970 evaluable women, 43% developed AIMSS and 12% discontinued AI therapy within 12 months. Although more Black and Asian women developed AIMSS than White women (49% vs. 39%, P = 0.017; 50% vs. 39%, P = 0.004, respectively), the AI discontinuation rates were similar across groups. None of the SNPs were significantly associated with AIMSS or AI discontinuation in the overall population or in distinct cohorts. The OR for rs2296972 (HTR2A) approached significance for developing AIMSS. Conclusions: We were unable to prospectively validate candidate SNPs previously associated with AI discontinuation due to AIMSS. Future analyses will explore additional genetic markers, patient-reported outcome predictors of AIMSS, and differences by race.
Several studies reported that patients with acute myeloid leukemia (AML) who remain in long-term remission after allogeneic or autologous transplant have a shorter life expectancy, compared to the general population. However, little is known about the life expectancy of adult long-term survivors of AML who were treated with chemotherapy alone without a transplant and there have been no comparisons with survival among the general population. The current study indicates that the life expectancy of AML patients who achieved and maintained CR for at least 3 years is shorter than expected for age in the US population. This was observed also in patients who did not undergo a transplant including those who have not relapsed during the entire long follow-up period. Thus, late relapse does not explain why patients without transplants have a shortened life expectancy. Taken together, these data strongly suggest that prior chemotherapy for the underlying AML is at least a major contributing factor for the known shortened life expectancy post-transplant.
Metaplastic breast cancer (MBC) is rare, has a worse prognosis than invasive ductal breast carcinoma, and most cases are triple-negative phenotype. Due to its low incidence, clinical studies of the disease are rare, and relatively poor outcomes are observed in clinical practice despite treatment. The present case report focuses on a patient with triple-negative MBC, clinical stage IIIB at diagnosis, and with local disease progression during standard neoadjuvant treatment. The patient was referred for surgical treatment but developed a distant recurrence of the disease shortly after. She was then treated with sacituzumab govitecan, an antibody-drug conjugate (ADC), and responded very well to treatment. Because this aggressive disease has a low response rate to chemotherapy, a better understanding of its molecular biology and therapeutic possibilities using new agents such as ADCs is crucial to advancing treatment.
Abstract An international Energy Company & independent engineering company have performed preliminary studies for an In-Line Robot (ILR) Project including: feasibility study, turbine design (with CFD calculations and flow assurance) and Energy Balance Assessments. This Robot will be a tetherless autonomous device capable of travelling with/against production flow to accomplish pigging and inspection missions inside pipelines with minimum production impacts. This is particularly adapted for single line long tie-backs, thanks to regenerative power management but the complexity of subsea architecture, flow conditions & fluids services raises some challenges. The ILR development is programmed over five phases (Feasibility study, Preliminary Systems design & Energy Balance Assessment, Flow Loop Bench Testing, Prototype Testing and Commercialisation). Phase 2 utilised Computational Fluid Dynamics (CFD) simulation models to assess power extraction levels from production flow across various scenarios whilst minimising pressure drop. The results obtained included the turbine CFD models that were coupled to power conversion and storage modules in order to ensure that system drive and power managementwere captured in a closed loop. An operational envelope was established considering the preliminary turbine design simulations as well as the associated energy balance. This paper will present the results to date along with the key design features of the ILR and how the data will be used to verify the operational envelope during the next phase, Flow Loop Bench Testing which is due to start in late 2019. This will provide data to configure and predict operational envelopes of the robot for different flow patterns and fluid types.
