Background Awake fiberoptic intubation (AFOI) is essential for managing challenging airways but is frequently constrained by patient anxiety and discomfort. This study evaluates oral gabapentin and nebulized dexmedetomidine, two noninvasive agents, for anxiolysis during AFOI, with the objective of enhancing patient cooperation, procedural success, and hemodynamic stability. Methods This prospective, randomized, double-blind study involved patients undergoing AFOI due to anticipated difficult airways. Participants were randomly allocated to receive either oral gabapentin or nebulized dexmedetomidine 1 h prior to the procedure. Standard airway topicalization was executed. Anxiety levels, hemodynamic parameters, patient comfort, and intubation conditions were evaluated using validated scales at specified time intervals. The data were analyzed utilizing SPSS software. Continuous variables were presented as mean ± SD and analyzed using the Student's t-test. The chi-square test was employed to compare categorical variables. A p-value of less than 0.05 was deemed statistically significant. The sample size was determined to ensure sufficient statistical power. Results The demographics were analogous. Group G exhibited markedly reduced anxiety scores compared to Group D (p = 0.0001). Heart rate was more effectively regulated in Group D (p = 0.001), whereas changes in mean arterial pressure (MAP) were comparable (p = 0.53). Other secondary outcomes were satisfactory and comparable between the groups, exhibiting no significant differences. Conclusion Both oral gabapentin and nebulized dexmedetomidine demonstrated efficacy in providing anxiolysis during AFOI. Gabapentin demonstrated a superior anxiolytic effect, whereas dexmedetomidine more effectively regulated heart rate. Blood pressure, sedation, cough, airway patency, and comfort were equivalent. Both agents were secure and well accepted, endorsing their noninvasive application in challenging airway management.
Struma ovarii is an extremely rare type of malignant germ cell tumor and its incidence is 0.1% of all ovarian malignancies. Only 5 percent of carcinoid cancers originate in the ovary. The histopathology is a combination of carcinoid and thyroid tissue along with other components of mature teratoma. In this case, patient was presented with irregular menses only. Cryotherapy is being used widely as a treatment option for various skin conditions due to its cost-effectiveness, convenience, and lack of invasiveness. As with any other procedure, cryotherapy has its share of complications. Among these, hemorrhagic bullae are not an uncommon complication. Hereby, we are reporting two cases with hemorrhagic bullae following cryotherapy. The first case was an 18-year-old male with multiple viral warts bilaterally over the limbs. But the patient developed bullae over all the treated sites around 24 h after cryotherapy. Similarly, another patient, a 55-year-old male, who was treated for multiple keloids present over the chest, abdomen, and left shoulder, also developed bullae over all the treated sites within the same time frame. Based on the findings of our report, it is essential to resolve the technical issues related to the delivery system and temperature monitoring during treatment administration. Before beginning cryotherapy, it is important to take into account not only the total number of lesions but also the total area they cover.
Cerebral malaria is an important cause of mortality and neurodisability in endemic regions. We show magnetic resonance imaging (MRI) features suggestive of cytotoxic and vasogenic cerebral edema followed by microhemorrhages in 2 adult UK cases, comparing them with an Indian cohort. Long-term follow-up images correlate ongoing changes with residual functional impairment.
Sarcina ventriculi (SV), which is also called Clostridium ventriculi, is a rare, non-motile, aerobic, Gram-positive coccus that breaks down carbohydrates through fermentation. It thrives in acidic environments, making the human stomach a suitable habitat, particularly in conditions associated with gastric stasis. SV belongs to the Clostridiaceae family. Recent studies have implicated Sarcina ventriculi in severe gastrointestinal conditions, including emphysematous gastritis, pyloric stenosis, gastroparesis, gastric outlet obstruction, gastric perforation, and gastric adenocarcinoma. Increased awareness of this organism and further studies are essential to understanding its role in human disease.
