    1STUDY OF THE FEATURES OF PHOSPHORUS-CALCIUM METABOLISM IN THE STATE OF DENTAL AND BONE TISSUE IN CHILDREN WITH OSTEOGENESIS IMPERFACTA

    Background. Osteogenesis imperfecta (OI) is a hereditary connective tissue disorder affecting skeletal and dental tissues. The aim: to assess the dental status and content of vitamin D, osteocalcin, osteoprotegerin, bone alkaline phosphatase, and parathyroid hormone in mixed saliva, and to study the relationship between clinical and laboratory parameters and bone tissue status in children with osteogenesis imperfecta. Materials and Methods: A study was conducted at the Department of Pediatric Dentistry "CS and Maxillofacial Surgery" of the A.I. Evdokimov Scientific Research Institute of Dentistry. The study involved 20 apparently healthy children (health groups 1 and 2) aged 8–17 years (average age 12 years) seeking dental care (comparison group) and 26 children of the same age with a genetically diagnosed condition, osteogenesis imperfecta (study group), referred from the GMSClinic in Moscow. The study was approved by the Ethics Committee of the Russian University of Medicine (Extract from Protocol No. 02-24 of the Interuniversity Ethics Committee dated February 15, 2024). The study consisted of clinical and biochemical phases. The dental status was assessed using standard dental indices. The content of mineralization markers in mixed saliva was determined using enzyme-linked immunosorbent assay (ELISA). Results: Compared with healthy children, children with osteogenesis imperfecta had higher dental status indicators associated with a deficiency of vitamin D and bone metabolism regulatory factors (ABP, OST, TFR), possibly indicating impaired mineralization of the skeleton, dental tissues, and periodontium in children with this pathology. From a clinical point of view, the established relationships between the studied clinical and laboratory parameters determine the need to classify children with imperfect osteogenesis as a high-risk group for dental problems.

    2Can Aquablation Treatment Help to Effectively Reduce the Long Waiting List of Patients with Surgical Indication for Benign Prostatic Obstruction in a Fully Public Healthcare System? Results of the First 100 Cases from a Single-Center Same-Operator Cohort

    Purpose To assess the efficacy and safety of Aquablation treatment in a single-center same-operator cohort and its potential role in reducing the long waiting list of patients with surgical indication for benign prostatic obstruction (BPO). Materials and methods Patients with surgical indication for BPO underwent Aquablation. All patients were treated by the same operator (D.P.) in a single center (Sant’Anna hospital, Como, Italy). Ablation was performed according to the standardized technique and coagulation through bipolar resectoscope was done in all cases. Perioperative parameters, functional outcomes and complications were assessed. Results 100 consecutive patients were included. Mean prostate size was 90.9 ml (min 40 ml, max 300 ml). Mean total operative time was 38.7 min, with a mean ablation time of 14.3 min and coagulation time of 21.8 min. No intraoperative complications were reported. In 2 cases a technical problem occurred and solved by replacing the handpiece. Postoperatively, 3 patients (3.0%) underwent urinary retention without haematuria, 2 patients (2.0%) underwent clots-induced retention, 1 patient (1.0%) developed an infection. No urethral strictures, nor bladder neck sclerosis were diagnosed during the follow-up, as well as no cases of stress urinary incontinence. Functional outcomes significantly improved compared to the preoperative evaluation. Conclusions Aquablation proved to be not only a fast treatment when performed by the same hands, but also a reliable option in terms of efficacy and safety, with good functional outcomes and very low complication rates. The shorter operative time provided by this technique may help to reduce the long waiting list of BPO patients with surgical indication.

    3New Import-Substituting Technology for Obtaining Cellular Plasma Therapy Drugs

    In recent decades, autologous cellular plasma therapy (ACPT) has become a promising method for treating musculoskeletal diseases due to the ability of activated blood cells to secrete growth factors, anti-inflammatory and analgesic cytokines. However, the high cost of foreign consumables and equipment limits the availability of therapy. Study of a new domestic technology for obtaining plasma preparations (L-PE) and its comparison with the foreign RegenLab (RL) technology in terms of production methods, activation methods and cellular composition of the resulting preparations. The study was conducted from March 2024 to May 2025 at the "Center for Traumatology and Orthopedics" of the N.A. Semashko Russian Railways - Medicine Clinical Hospital, which is the base for the Department of Traumatology, Orthopedics and Disaster Medicine of the Russian University of Medicine of the Ministry of Health of the Russian Federation, and at the Department of Pathological Physiology of the Russian University of Medicine of the Ministry of Health of the Russian Federation. The study involved 20 patients aged 25 to 58 years with diseases and injuries of the musculoskeletal system, from whom 10 ml of blood was taken from a vein. SPT preparations were prepared and studied using a new domestic technology - 12 patients, and using RL technology - 8 patients. L-PE contains platelets, leukocytes and erythrocytes, which expands the range of biologically active substances (BAS) compared to PRP, which includes only platelets. The study of the cellular composition showed that the domestic technology makes it possible to controllably obtain preparations with both high and low concentrations of formed elements. Activation with calcium chloride increases the release of growth factors, including hemolysis products and erythrocyte microvesicles, which enhances the regenerative potential. L-PE: cost-effective, easy to use, no mechanical damage to cells. However, further research is required to optimize clinical use.

    4A COMPLEX APPROACH TO THE DIAGNOSIS AND TREATMENT OF TMJ DYSFUNCTION: RATIONALE FOR THE INCLUSION POSTUROLOGICAL SCREENING

    Relevance. Modern dentistry is witnessing a steady increase in the number of patients with temporomandibular joint dysfunction (TMD), necessitating the development of a comprehensive approach that considers the stomatognathic system as part of the musculoskeletal system. Despite data on the relationship between occlusion and postural balance, standard diagnostic algorithms often lack objective methods to assess this relationship. Aim. Based on an analysis of current literature, to substantiate the necessity of including posturological and stabilometric screening in the diagnostic protocol for patients with TMD to improve the effectiveness and stability of rehabilitation. Materials and methods. An analytical review of sources selected from PubMed, Scopus, Web of Science, eLIBRARY, and Cyberleninka databases for the period 2015–2025 was conducted using keywords related to TMD, occlusion, posturology, and stabilometry. The final selection included 26 relevant publications. Results. The literature analysis confirmed the existence of a neurophysiological and biomechanical relationship between occlusion, TMJ status, and postural control. It was shown that various mandibular positions have a statistically significant effect on postural balance. Key mechanisms of this relationship were identified: disrupted proprioceptive afferent input from the trigeminal system and changes in muscle chain activity. Studies demonstrate that patients with TMD have significantly lower postural stability parameters, and the nature of their changes during teeth clenching depends on the type of disorder. Conclusion. The inclusion of stabilometry in the comprehensive diagnostic protocol for TMD provides an objective criterion for treatment effectiveness. Improvement in stabilometric parameters after using an occlusal stabilization appliance indicates normalization of TMJ function and improved overall postural balance.

    5Living-related Kidney Transplantation in a Patient with Bardet-Biedl Syndrome

    Background . Bardet–Biedl syndrome (BBS) is a hereditary diencephalic-retinal disorder characterized by a combination of clinical features, including obesity, mental retardation, retinal degeneration, brachydactyly, polydactyly, hypogenitalism, and renal dysfunction. Up to 25% of patients develop end-stage chronic renal failure requiring renal replacement therapy or kidney transplant (KT), often during childhood. Objective : to present the treatment outcomes of severe chronic renal failure in a 16-year-old female patient with the rare genetic condition BBS, who previously underwent KT from a related donor at the age of 6. Materials and methods . A retrospective analysis was conducted of the patient’s medical records, including the clinical course of the disease, laboratory and instrumental findings, and details of the treatment provided to the 16-year-old patient diagnosed with BBS. Results . On November 25, 2015, an AB0-compatible related (the patient’s father) KT was performed on the right side. Maintenance immunosuppressive therapy included tacrolimus, mycophenolic acid, methylprednisolone. Immediate graft function was observed after the surgery. At the follow-up consultation on November 13, 2024, the patient was receiving tacrolimus (Prograf) 1.5 mg twice daily, mycophenolic acid 180 mg twice daily, methylprednisolone 4 mg per day orally, as well as Cardiomagnyl and Omeprazole 20 mg orally. Laboratory findings showed a serum creatinine level of 65.9 μmol/L, with an estimated glomerular filtration rate calculated using the CKD-EPI equation of 119.72 mL/min/1.73 m 2 . Conclusion . Despite the presence of severe congenital pathologies associated with BBS, this clinical case presents a successful KT with minimal complications in both the early and long-term postoperative periods.
