
In comparative studies in 36 mongrel dogs it was tried to find out how far intraaortic balloon pumping (IABP) applied solely and in combination with glucose-insulin-potassium (GIK) might influence the degree of acute myocardial ischemia. The ischemia lasted 3 hours. After 60 min myocardial ischemia IABP, the isolated GIK infusion, or the combination of IABP plus GIK were used. The directed GIK application into the aortic root was carried out by electronic triggering during the diastolic augmentation via a precoronary catheter. In the ischemic and in the non-ischemic myocardium of the left ventricle, ATP, creatine phosphate, and lactate were determined. Electronmicroscopical studies were carried out qualitatively as well as quantitatively. The isolated use of IABP suggested no protection of the energy-rich phosphates. The most significant morphological changes were found when exclusively the coronary ligature was applied and when only IABP was used, respectively. The best protective effect was achieved by our technique of the triggered GIK infusion via the precoronary catheter in combination with IABP. The ATP value (n mol/mg w.w.) in the ischemic area was 3.06 and in the non-ischemic one 4.96 in comparison to 1.96 and 3.99, respectively, in the control group. The creatine phosphate (n mol/mg w.w.) was 4.18 in the ischemic and 9.38 in the non-ischemic area in contrast to 2.36 and 6.99, respectively, in the control group. The technique of the triggered drug supply in combination with IABP offers the following advantages: --additive summation of IABP and substrate supply directed to the myocardium --high drug concentration in the myocardium --smaller drug load of the whole organism --clinical use of instable substances.
55 of 113 receivers of cadaver donor kidney transplants could be control-examined and analysed postoperatively immunologically with the electrophoresis mobility test (EM test). A high coincidence appears in the group with clinical rejection symptoms and a rejections hint in the EM test (28 of 29) and also in the group with the rejection clinically not conceivable and with a negative test result (20 of 26). The EM test appears suitable for an additional support of the diagnosis "acute rejection". More cases should confirm this assertion.
An animal experimental study was carried out in order to compare the therapeutical value of two different surgical methods for excising the subcutaneous implanted Lewis lung carcinoma - tumor excision with scalpel or with carbon dioxide laser. The radicalism of operation methods, the survival time, and the tumor local recurrences were performed. The therapeutic effectivity of tumor excision with the carbon dioxide laser is more favourable than the scalpel method.
The function of the ileo-ureter and the operated kidney was investigated by means of the camera functional scintigraphy in 23 dogs operated under different techniques. This non-invasive examination was carried out with two different radio-remedies (131I-Hippuran and 99mTc-DTPA) by a gamma camera microprocessor. The direct single-layered ileo-ureteric anastomosis showed the best urine transportation. This method has a great practical importance in the opinions of the authors.
The biostimulatic effect of laser radiation was proved in animal investigations and also observed in 259 patients with disturbed wound healing. Laser rays can be applied without any risk in patients. The therapy was successful in over 90% of the patients. The duration of the therapy amounted to 10 to 12 weeks on an average.
The investigations were carried out in rats by means of the amino acid 15N-glycinee 6 to 12 weeks after the gastrectomy. 10 rats were operated by Graham and 9 by Longmire, Gütgemann. 12 rats as a control group were not operated. The animals got a single dose 15N-glycin with the food in the beginning of the experiment. The excretion of the isotope was measured every twelve hours for 5 days. All rats had a positive nitrogen balance. The rats operated by Graham excreted with the urine significantly less 15N than the rats of the other two groups. These differences were caused by different fast adsorption of the amino acids likely, at this the rats operated by Graham absorbed retardingly evident qualified by the absent duodenal passage. These disturbances of the amino acid absorption are not aggravating, because the nitrogen balance and the protein synthesis rate of the animals were normal.
The effects of ultrasonic saw cuts in the epiphyseal cartilage reach were investigated experimentally in dogs. Saw cuts across led to an impaired growth, whereas longitudinal cuts caused a stimulation of growth. The dimensions of the growth influence were, however, very different and not calculable. Further on, the temporal lapse of the bony repair was investigated radiologically comparatively after osteotomy by the ultrasonic saw or by the oscillated saw. The bony repair took place more slowly after ultrasonic osteotomy generally speaking. The indications and limits for the application of the ultrasonic bone saw were deduced from the investigation results by including the bibliography.
The premature manifestation of the arteriosclerosis in the adductor canal makes this vascular region to a first-class site of predilection. These localised mechanical morphological factors were malingered by means of polyvinylchloride splinting in animal experiment. All degrees of severity of the arteriosclerotic vascular wall changes were shown histologically. The changes of the arteries were developed essentially slighter under medication of pyridinolcarbamat, however, they could not be prevented.
The thrombosis in dependence on the stream velocity in the electrically induced thrombosis was investigated by blood flow measurements in the femoral artery in 16 mongrel dogs. Thromboses were shown histologically in all 16 animals. 8 of them did not obturate. Obturating thromboses were produced with the application of anodic direct current of 1 mA during 60 minutes only with a blood stream velocity between 0.0465 and 0.0928 m/s. Not any obturating thrombosis was visible outside of this range.
Chemical and structural surface changes at enosseous bioglass-ceramic-implants (Ap 40) were investigated by means of electron microprobe after an implantation period of 4 to 52 weeks. An intimate bonding between regenerated bone and bioglass-ceramic surface was evident at points already after 4 weeks. Initial degradation processes at implant surface came to a standstill after 12th week. A calcium and phosphorus rich layer of great importance for implant stability and biocompatibility was formed immediately contacting the bone. This surface layer had a thickness of nearly 25 micrometers -- 30 micrometers at 12 weeks and remains unchanged up to 52 weeks. The registered calcium and phosphorus impulse rate (cps) is equal to that of neighbouring bone. Chemical similarity of implant surface makes bioglass-ceramic Ap 40 a suitable biomaterial for durable bone replacement.
A totally artificial heart--the Rostock totally artificial heart (Rostock TAH) was implanted in calves. We can give the following deductions after 27 examinations: -- The Rostock TAH was tested successfully in endurance test about 7 months; it show no sign of wear. -- The surgical technique of the implantation was standardised and a successful experimentation is possible. -- The longest survival time was 15 days. The hemodynamic early results were very good in 3 other animals. -- Modifications of the Rostock TAH were taken up relating to the dimensions of the artificial ventriculi and by mounting a screw cap for avoiding an air embolism. -- Adaption tests in the corpse show that the present form of the artificial heart is not ideal for a possible application in man. -- The problem is still existing in developing a suitable drive system locates intrathoracically. The present drive and control system on a pneumatic base is qualified only for the animal experiment but not for a possible application in man.
The influence of the function of the thrombocytes after electrical thrombosis induction by means of anodic direct current was investigated in 39 mongrel dogs. The platelet factor 3, the rotation thromboelastography, and the collagen induced thrombocytes aggregation in the optical test by Born was examined. The platelet factor 3 adhering on the thrombocytes decreased from 95% to 49% in the course of the thrombosis induction. Its activity was provable by means of the rotation thrombelastogram in the plasma. Besides of it the electrical field caused an activation of the thrombocytes which corresponds to a contact activation on foreign body surfaces. The collagen induced aggregation ability of the thrombocytes remains unaffected after the passage of the electrical field.
The authors carried out choledochal grafts in dogs with autologous parietal peritoneum and adjoining fascia of the musculus rectus abdominis. The patch was fixed with the tissue adhesive hystoacryl without suture. Additionally there was used an interior splinting with a polyethylene tube coated with paraffin. The inserted tube rubed off itself in case of a proper size. Adhesive must not get into the choledochus. 15 of the 20 experiments in dogs were successful.
The determination of the concentration in acetate puffered methanol solutions (formaldehyde solutions) with pH = 5.60 during 14 months disproved the opinion of the limited durability of these solutions. Neither a significant decrease of the methanal mass concentration nor a noticeable change of the pH value could be proved during the period of the examination under different influences of light and temperature. Methanal solutions can be applied farther in the practice of the preservation.
Absorption of ultrasound leads to reversible or irreversible changes in biological tissues in dependence on the sound intensity. The ultrasound causes under appointed conditions in skin, muscles, vessels, nerve tissue, and bone histopathological changes in a different scale which must be observed in the ultrasonic surgery. 200 tissue specimens from 40 guinea pigs were taken from defined localisations after an ample ultrasound exposition and were evaluated microscopically by hematoxylin-eosin staining technique. Skin and vessels showed very vehement reactions. Intact osseous tissue reacted with periosteal callus formation. Practical conclusions can be draws for the ultrasonic surgery by the discussion on histopathological changes.
