
Fall prevention for hospitalized patients is an important nursing quality indicator. Current studies do not describe characteristics of hospitalized patients with cancer who fall, although these patients have been noted to have higher fall and injury rates. This descriptive study represents an initial attempt to identify characteristics of patients hospitalized with cancer who fall compared with adult medical-surgical hospitalized patients who fall. We found that many characteristics of our sample were similar to those of other patients who had experienced a fall during their hospitalization.
The purpose of this study was to obtain a comprehensive understanding of how nurses respond to medication errors and identify strategies that nurses believe may improve reporting of these errors within hospitals. The findings suggest that nurses use a decision-making process when reporting medication errors. Medication error reporting may be improved by instituting a problem-solving approach to respectfully manage the event and providing nurses with clear guidelines, remedial education, and timely feedback.

Advances in prevention and treatment of HIV have led to an increased life expectancy for patients with HIV infection. Due to their increased life span, HIV+ patients are now presenting to hospitals with an increased number of diverse late-stage complications, such as cardiomyopathy and other cardiovascular conditions. These complications are as a direct or indirect result of HIV disease, HIV treatment modalities, comorbid conditions, dietary and lifestyle factors, and unknown etiologies. Cardiac complications, particularly HIV-related dilated cardiomyopathy, are potentially life-threatening diagnoses, with symptoms that may be minimized with appropriate cardiac-specific assessments and treatments, patient teaching, and collaboration among nurses caring for the HIV-positive client with cardiac disease.

The metabolic syndrome is a clinical condition that is a powerful predictor for cardiovascular morbidity and mortality. Hypertension, abdominal obesity, high blood glucose levels, and abnormal blood lipid levels characterize metabolic syndrome. Therapeutic treatment of the metabolic syndrome confers a significant risk reduction for both type 2 diabetes and premature cardiovascular events. In the hospital setting, the management of hyperglycemia, one of the clinical components of the metabolic syndrome, has been secondary in importance to the condition that prompted admission. Hyperglycemia in the hospitalized patient has been associated with increased lengths of stay, higher rates of hospital-acquired infections, and increased mortality. Early recognition and treatment of hyperglycemia and the associated metabolic components that comprise the metabolic syndrome may reduce morbidity and mortality in the hospital setting. More aggressive interventions will aid in reducing costs while simultaneously improving patient care and safety.

Although endocrine regulation of growth and sexual maturation is a significant issue in general pediatrics, disorders of energy production and utilization, fluid and electrolyte balance, and circulatory function are the endocrine causes of critical illness in children. Care of the child with critical endocrine disease requires an understanding of endocrine pathophysiology, keen history taking and assessment skills, and knowledge of the pharmacology of synthetic hormone treatment. Severe stress, associated with critical illness, activates the hypothalamic- pituitary-adrenal (HPA) axis and stimulates the release of cortisol from the adrenal cortex. Cortisol is essential for general adaptation to stress and plays a crucial role in cardiovascular, metabolic, and immunologic homeostasis. During critical illness, prolonged activation of the HPA axis can result in hypercortisolemia and hypocortisolemia; both can be detrimental to recovery from critical illness. Recognition of adrenal dysfunction in critically ill patients is difficult because a reliable history is not available and laboratory results are difficult to interpret. The review in this article will illustrate how adrenal dysfunction presents in critically ill patients and how appropriate diagnosis and management can be achieved in the critical care setting.
Hyperglycemia is common in critically ill humans. Recent clinical trials have shown a significant reduction in morbidity and mortality rates with the use of intensive insulin therapy to maintain strict normoglycemia in critically ill patients. Hyperglycemia is associated with many detrimental effects, including reduced immune function, increased inflammation and coagulation, and modulation of the endothelium. Most of the evidence regarding the adverse effects of hyperglycemia is derived from humans with diabetes, cardiac failure, or traumatic brain injury. In addition to its anabolic effects on metabolism, insulin has antiinflammatory properties. To define the potential risks and benefits of intensive insulin therapy in critically ill animals, prospective, randomized clinical trials are necessary.

Individuals with HIV are living longer and developing conditions usually associated with aging, as well as complications from pre-existing or subsequently acquired conditions. The brain depends on a continuous flow of blood to provide it with oxygen and glucose needed to maintain normal function and structural integrity, thus cerebral blood flow is normally tightly regulated. A decrease in cerebral blood flow to ischemic levels may be tolerated for only minutes to hours, depending on the severity of the ischemia. If cerebral blood flow ceases completely, brain cell death occurs within minutes. A variety of conditions are encountered clinically, such as stroke or traumatic brain injury, where an actual or potential alteration in cerebral blood flow puts the brain at risk for ischemia and infarction. In this article, the physiology of cerebral blood flow will be presented as a basis for understanding cerebral blood flow regulation and the rationale for clinical interventions to optimize cerebral blood flow. Techniques currently available to assess cerebral blood flow and clinical situations in which cerebral blood flow is measured will be discussed. Clinical interventions will be presented briefly.

The use of dopamine for the treatment of renal insufficiency has become a controversial issue. Dopamine exerts its effects on the kidneys through activity on the catecholamine receptors and by its diuretic and natriuretic properties. Utilization of renal dose dopamine to increase renal blood flow has been considered beneficial for preservation of renal function for over 30 years. The hypothesis proposed was that increasing urine volume must indicate improving renal function, particularly in oliguric patients. However, recent clinical trials in adult and pediatric patients have not only failed to demonstrate any benefit, but have also suggested that this therapy may actually have detrimental effects. This article reviews basic pharmacology and physiologic effects and the potential adverse effects of "renal dose dopamine." It also examines the results of clinical trials, in both pediatric and adult patients, that evaluated its usefulness for the treatment of renal insufficiency.
Peer review has multiple manifestations and purposes. Two stated purposes are the demonstration of professionalism and clinical competency. The American Nurses Association (ANA) defines nursing peer-review as a process for evaluating the care provided by an individual according to accepted standards. Further, the ANA proposes that nurses with similar rank and clinical expertise should conduct these evaluations. Some local jurisdictions may also mandate that advanced practice nurses (APNs) review one another's care. Therefore, APNs should become familiar with sources for evaluation criteria and tool formats for APN peer review. The advantages and limitations of the various formats and processes of peer review should also be considered.
