
OBJECTIVES:The purpose of this study was to compare one and four weeks of active mobilization during balneotherapy (B) with mobilization on land (L) in terms of the respective effects on static and dynamic balance in elderly subjects. METHODS:Twelve elderly volunteers (mean age: 65.6+/-6.3 years) were evaluated for pain (on a visual analogue scale, [VAS]), static balance (as measured by the centre of foot pressure, [COP]) and dynamic balance (the "Timed Up and Go" [TUG] test). Six measurement sessions were performed: three baseline sessions before exercise sessions (PostL, PreB1 and PreB4) and then after a land-based session L and after one and four weeks (B1 and B4) of balneotherapy at 34 degrees C (PostL, PostB1 and PostB4, respectively). RESULTS:A Friedman analysis of variance (ANOVA) revealed significant differences between the six conditions in terms of the TUG and VAS results (p<0.01 and p<0.05, respectively). The TUG time decreased after the balneotherapy session(s) (p<0.01, PreB1 versus PostB1 and PreB4 versus PostB4). After sessions B1 and B4, a decrease in the VAS score was noted (p<0.05, PreB1 versus PostB1 and PreB4 versus PostB4). In contrast, there were no significant postbalneotherapy changes in any of the postural parameters. CONCLUSION:Balneotherapy sessions appear to induce a decrease in pain and an increase in dynamic mobility. This might be explained by enhanced proprioceptive input during neuromuscular mobilization in water (i.e. with increased resistance but a lower load).
AIM:The aim of the present study was to analyse the effects of training performed on a rotating, motorised platform (the Huber/SpineForce device from LPG Systems, Valence, France) intended to improve, postural control and muscle function.SUBJECTS:Twelve healthy adults (divided into a sedentary group and an active group) took part in a two-month training programme (involving three sessions a week) on the SpineForce whole body rehabilitation device.METHOD:Instrumental assessment of postural control (on a Satel platform) and muscle function (on a Cybex Norm) was performed before and after training. Postural control in various conditions was measured using a position parameter (the mean anteroposterior position of the centre of foot pressure [CoP]) and two stability parameters (maximum CoP displacement and CoP sway area). Assessment of the muscle function was performed during knee and spine extension and featured maximum voluntary isometric contraction (MVIC), root mean square (RMS) and neuromuscular efficiency (MVIC/RMS) measurements.RESULTS:For static postural control, we observed a more forward CoP position in the maximum backward inclination condition (p<0.01) and a decrease in maximum CoP displacement in the "eyes closed on foam" and "maximum anterior inclination" conditions. In this latter condition, a lower CoP sway area was also noted (p<0.01). In terms of muscle function, a greater MVIC for knee extension was observed in the sedentary group only (p<0.05). These changes were not correlated with each another (p<0.05). However, the value of the pretraining maximum CoP displacement predicted its final value (p<0.05).CONCLUSION:Our results suggest that static postural control responds to training on a Huber((R))/SpineForce rehabilitation device. It seems probable that a population with a low initial level of physical activity would benefit most from training on this type of device. This training could notably be applied to elderly or disabled people and especially those with sensorimotor disabilities.
Breaking hat sich mit seiner Mischung aus anspruchsvollen Tanzbewegungen und komplexen akrobatischen Bewegungen zu einer weltweit praktizierten Ausdrucksform entwickelt, welche nun in das Programm der Olympischen Spiele aufgenommen wurde. Eine damit einhergehende Professionalisierung und Steigerung von Trainingsumfängen und -intensitäten erhöht dabei auch die Ansprüche an das muskuloskelettale System der Sportler. Über Verletzungsmuster, Ausmaß, Ursache und Präventionsansätzen ist bisher noch wenig bekannt. Ziel dieses systematischen Reviews ist es, Verletzungsprofile im Breaking zu identifizieren und herauszufinden, welches Trainingsverhalten zu Verletzungen beiträgt, um infolgedessen effektive Präventionsmaßnahmen zu entwickeln.Es erfolgte eine systematische Literaturrecherche in verschiedenen Datenbanken (PubMed, web of science, SAGEPub, JStor und CINAHL). Von den initial 24 Treffern wurden nach Anwendung der Ein- und Ausschlusskriterien 9 Studien untersucht und analysiert.Es konnte ein detailliertes Verletzungsprofil mit Angaben zu Trainingsverhalten und verschiedenen Verletzungsparametern erstellt werden: 78,4% des Trainings findet ohne Anleitung statt. Die am häufigsten verletzten Körperregionen sind Hand (18,8%), Wirbelsäule (17,9%) und Knie (17,4%). Die häufigste Verletzungsursache ist eine insuffiziente Erwärmung (57,7%). Anhand dieser und weiterer Ergebnisse wurden erste Präventionsmaßnahmen abgeleitet.Neben der dem Breaking inhärenten intensiven körperlichen Belastung scheint die Art der Trainingsgestaltung für das hohe Verletzungsrisiko mitverantwortlich zu sein. Mit den Erkenntnissen kann das Verständnis bei medizinischem und trainingswissenschaftlichem Personal für diese Tanzform verbessert werden und das Training hinsichtlich der Verletzungsprävention optimiert werden. Die Maßnahmen umfassen Trainingsrhythmus und -dauer, Schutzbekleidung, Bodenbeschaffenheit, ergänzendes Fitnesstraining und das Erkennen von Frühwarnzeichen. Außerdem zeigte sich, dass es weiteren Forschungsbedarf gibt, um die Trainingsgestaltung weiter zu professionalisieren.Breaking, with its mixture of demanding dance moves and complex acrobatic movements, has become a form of dance expression practiced worldwide, which has now been included in the program of the Olympic Games. The accompanying professionalization and increase in training volumes and intensity also increases the demands on the musculoskeletal system of athletes. However, little is known about the extent, cause, injury patterns and prevention approaches. The aim of this systematic review is to identify injury profiles in breaking, highlight training behaviors that contribute to injuries and to propose effective prevention measures.A systematic literature search was conducted using various databases (PubMed, web of science, SAGEPub, JStor and CINAHL). The initial search produced twenty-four results. However, only nine met the inclusion criteria and where further analyzed.A detailed injury profile with information on training behavior and various injury parameters could be created: 78.4% of the training takes place without supervision. The most frequently injured body regions are: hand (18.8%), spine (17.9%) and knee (17.4%). The most common cause of injury was an insufficient warm-up (57.7%). Based on these and other results, initial prevention measures were derived.In addition to the intense physical stress inherent in breaking, the way training is designed seems to be partly responsible for the high risk of injury. The findings can be used to improve the understanding among medical and kinesiological staff for this dance form and to optimize Breaking training with regard to injury prevention. For injury prevention, we suggested dancers take into consideration: training rhythm and duration, protective gear, floor conditions, supplemental fitness training, and recognition of early warning signs. Lastly our findings suggest a need for further research in order to further professionalize the training design.
The European Community is based on a series of treaties and legal decisions, which result from preliminary documents prepared long before by different organizations and lobbies. The European union of medical specialists (Union européenne des médecins specialists [UEMS]) came into being in order to address the questions raised by European directives (e.g., free circulation of people and services, reciprocal recognition of diplomas, medical training, quality improvements). The specialty sections of the UEMS contribute actively to this work. The physical and rehabilitation medicine (PRM) section is composed of three committees: the PRM board is devoted to initial and continuing education and has published a harmonized teaching programme and organized a certification procedure, which can be considered as a European seal of quality; the Clinical Affairs Committee is concerned with the quality of PRM care, and it has set up a European accreditation system for PRM programs of care, which will help to describe PRM clinical activity more concretely; and the Professional Practice Committee works on the fields of competence in our specialty. This third committee has already published a White Book, and further documents are being prepared, based on both the International classification of functioning, disability and health (ICF) and reference texts developed by the French Federation of PRM.L’Europe communautaire repose sur une construction juridique, préparée longtemps en amont par des textes issus de différentes organisations et groupes de pression. L’Union européenne des médecins spécialistes s’est organisée pour répondre aux problématiques posées par les Directives européennes (libre circulation des personnes et des services, reconnaissance mutuelle des diplômes, formation et démarche qualité). Les sections spécialisées contribuent activement à ces travaux. La section de MPR s’est structurée en trois commissions : le board, dédié à la formation initiale et continue ; il a harmonisé les programmes et organisé une certification qu’il faut considérer comme un label européen de qualité. La Commission des affaires cliniques s’occupe de la qualité des soins en MPR ; elle a mis en place une accréditation européenne des programmes de soins, utile à la description concrète de l’activité MPR. La Commission des pratiques professionnelles travaille sur les domaines de compétence de notre spécialité. Elle a publié un Livre Blanc de la MPR et prépare un texte s’appuyant sur la Classification internationale du fonctionnement, du handicap et de la santé (CIF) et sur les référentiels élaborés par la Fédération française de médecine physique et de réadaptation.
OBJECTIVE:The main objective is to determine the influence of treatment by Chêneau brace on the quality of life (QoL) of adolescents with idiopathic scoliosis.MATERIAL AND METHOD:We used the QoL scale of Climent et al. "the Quality of Life Profile for Spine Deformities" (QLPSD, in which a higher score means a worse QoL) and visual analogue scales (VAS) for pain and for QoL ranging from 0 to 100mm. The study includes 108 subjects divided in three groups, a control group of 32 patients without brace, a full-time treated group (wearing a Chêneau brace 23/24 hours) of 41 patients, and a part-time treated group (wearing the brace during the night only) of 35 patients.RESULTS:The QoL is significantly worse in the full-time treated group than in the group with night treatment and in the group without brace for the psychosocial functioning, the sleep disturbances, the back flexibility (P<0.001), the body image (P<0.01), as well as the overall score (P<0.001). On the other hand, there was no difference for the back pain. Among patients wearing the brace, the girls had an average QLPSD score higher than the boys for the psychosocial functioning, the body image, the overall score (P<0.05) and the back flexibility (P<0.01). Whereas there was no significant difference between the sexes for the sleep disturbances and the back pain. The QoL of patients who wear the brace was significantly correlated with degrees corrected wearing the brace and the patient's satisfaction on his or her health status. However, this correlation was very weak. On the other hand, there was no correlation between the quality of life and the age, the duration of brace treatment or the skeletal maturity (Risser sign). On VAS, the patients without brace have the highest score of QoL, followed the part-time treated patients while the full-time treated patients have the lowest score.CONCLUSION:The wearing of the Chêneau brace involves a significant reduction in the QoL whatever instruments of evaluations of QoL, QLPSD or VAS. The QoL of the full-time treated patients was the lowest followed part-time treated patients while the QoL of the patients without brace was the highest. However, the brace does not influence the back pain in idiopathic scoliosis at teenage.
Funded under the 7th Research Framework Programme of the European Commission, the goal of the Robo-Mate project is to develop an intelligent, easy-to-maneuver, and wearable body exoskeleton for manual handling work. Workers in the manufacturing industry are exposed to factors that increase their likelihood of developing Musculoskeletal Disorders (MSDs). The Robo-Mate industrial exoskeleton will be designed in accordance with best practice ergonomic principles to facilitate manual handling activities in multiple case study settings. An array of existing standards will be referenced in detail when designing the exoskeleton. As there is currently no standard that specifically targets the safety exoskeletons for industrial workers, it is intended to use Robo-Mate results to promote such development. To facilitate this process, a roadmap outlining tasks and responsibilities was created. It details who will oversee the process to promote and facilitate the further development of existing standards and ensure Robo-Mate results are targeted at suitable stakeholders. This will be achieved on three levels. In Level 1, the information will be directly communicated to standards developing bodies, specifically ISO, CEN and their members. In Level 2, assistance will be sought from organizations with interests in industrial robotics to add support to Robo-Mate when seeking to use the project's results to develop standards. Finally, the project details will be distributed to increase the awareness of the Robo-Mate project to the public, end-users, manufacturers, and distributers of industrial exoskeletons.
L’objectif principal est de déterminer l’influence du traitement par corset de Chêneau sur la qualité de vie (QDV) de l’adolescent porteur d’une scoliose idiopathique. Nous avons utilisé l’échelle de qualité de vie de Climent et al. the Quality of Life Profile for Spine Deformities (QLPSD, pour laquelle un score haut signifie une mauvaise QDV) et des échelles visuelles analogiques (EVA) de douleur et de QDV graduées de 0 à 100 mm. Cette étude comprend 108 sujets scoliotiques divisés en trois groupes, un groupe témoin de 32 sujets qui n’ont pas encore bénéficié d’un corset (sans corset), un groupe de 41 sujets appareillé à temps plein (23/24 heures) par corset de Chêneau et un groupe traité à temps partiel (porté la nuit uniquement) de 35 sujets. La QDV du groupe traité à temps plein est significativement moins bonne que celle du groupe avec traitement nocturne et du groupe ne portant pas le corset dans les domaines de l’état psychosocial, des troubles du sommeil, de la flexibilité dorsale (souplesse du tronc) (p < 0,001), de l’image corporelle (p < 0,01) ainsi que pour le score global (p < 0,001). En revanche, on ne trouve aucune différence entre les groupes pour l’existence de rachialgies. Chez les patients portant le corset, les filles ont un score au QLPSD plus grand que les garçons pour l’état psychosocial, l’image corporelle, le score global (p < 0,05) et la flexibilité dorsale (p < 0,01). Il n’y a pas de différence significative entre les sexes pour les troubles du sommeil et les douleurs de dos. La qualité de vie des patients qui portent le corset est significativement corrélée avec le degré de correction obtenue par le corset et la satisfaction du patient sur l’état de santé. Cependant, cette corrélation est très faible. En revanche, aucune corrélation entre la qualité de vie et l’âge, la durée du traitement ou l’indice de Risser n’a été observée. Sur l’EVA, les patients sans corset ont le score de QDV le plus grand, suivis des patients traités à temps partiel tandis que les patients traités à temps plein ont le score de QDV le plus bas. Le port du corset de Chêneau entraîne une diminution significative de la qualité de vie quel que soit les instruments d’évaluations, QLPSD ou EVA. La QDV des patients traités à temps plein est plus basse que celle des patients traités à temps partiels tandis que la QDV des patients sans corset est la meilleure. Par ailleurs, le corset de Chêneau n’influence pas les rachialgies dans la scoliose idiopathique de l’adolescent.
Le syndrome dysexécutif est une séquelle très fréquente du traumatisme crânien sévère. Il est très invalidant et incomplètement évalué par les tests neuropsychologiques classiques « papier–crayon ». Nous avons développé une évaluation « écologique », consistant en la réalisation d’une tâche de cuisine et nous avons décrit une classification des erreurs. Cette évaluation est très sensible à un syndrome dysexécutif, même discret.
Les décrets de 2008 réglementant l’activité en soins de suite et de réadaptation (SSR) ont pour objectifs de clarifier ce domaine de soins et de donner les mêmes règles aux secteurs publics et privés, notamment dans la perspective de l’application de la tarification à l’activité. Le champ du SSR sera ainsi composé d’un socle commun à toutes les structures et de secteurs d’activités spécialisées, pour lesquelles chaque structure qui le souhaite peut demander une autorisation d’activité sur la base de moyens humains et techniques minimum. La nécessaire coordination par un médecin de la médecine physique et de réadaptation (MPR) est prévue dans tous les secteurs de rééducation et de réadaptation, obligatoire notamment à la pratique d’une activité spécialisée en « affections de l’appareil locomoteur » ou en « affections de l’appareil locomoteur et du système nerveux ». L’admission sur la base d’un projet thérapeutique évalué est exigée et le rôle d’expertise de ces structures est reconnu par la possibilité d’admission directe à partir du domicile. Les textes ne détaillent pas les indications précises d’admission et ne fixent que les grandes lignes des moyens. Ils seront accompagnés d’une circulaire d’application qui guide les agences régionales d’hospitalisation pour la mise en place de cette réglementation et de fiches techniques qui décrivent le champ d’activité des domaines spécialisés. Les notions de niveau de proximité, de recours et de référence devront être apportées régionalement par les schéma régionaux d’organisation sanitaire (SROS).
L'objectif de ce travail est d'explorer les mécanismes responsables de la faiblesse musculaire chez des sujets âgés atteints de gonarthrose unilatérale. Sept patients souffrant de gonarthrose unilatérale symptomatique ont participé à cette étude. L'estimation du sens de position articulaire et les tests de force maximale isométrique (MVC) du quadriceps sont réalisés dans un premier temps. L'activité électromyographique (EMG) du biceps femoris (BF), du rectus femoris (RF), du vastus lateralis (VL) et du vastus medialis (VM) est mesurée pour différents niveaux de contraction (25, 50, 75 et 100 % MVC). L'efficience neuromusculaire du quadriceps est également calculée (MVC/EMG). Des clichés échographiques sont réalisés au niveau médian du VL à différents niveaux de contraction (25, 50, 75 % MVC) pour l'analyse des paramètres architecturaux. Pour tous les tests effectués, les deux côtés sont comparés (sain versus arthrosique). La MVC des extenseurs du genou arthrosique est diminuée de 30 % par rapport au côté sain. L'épaisseur du VL est inférieure de 10 % du côté arthrosique. L'activité EMG du VL est similaire des deux côtés. Celle du RF, du VM et du BF est supérieure du côté sain par rapport au côté arthrosique. L'efficience neuromusculaire est supérieure de 26 % du côté sain par rapport au côté arthrosique. Enfin, le sens de position articulaire est altéré de 33 % du côté arthrosique. La diminution de la force quadricipitale associée à la gonarthrose s'explique par une diminution de l'efficience neuromusculaire et de la masse musculaire. Les faibles modifications de pennation et de longueur des fascicules musculaires ne semblent pas impliquées. The aim of this study was to determine the mechanisms involved in muscle weakness in elderly patients with unilateral knee osteoarthritis. We investigated 7 patients with unilateral knee osteoarthritis. We measured knee position sense and isometric maximal voluntary contraction (MVC) of the knee extensors. Electromyographic (EMG) measurement of biceps femoris (BF), rectus femoris (RF), vastus lateralis (VL) and vastus medialis (VM) involved different levels of contraction (25, 50, 75 and 100% MVC). Neuromuscular efficiency of quadriceps was also calculated (MVC/EMG). Ultrasonography was used to investigate the VL architectural parameters at the median part of the VL for different levels of contraction (25, 50, and 75% MVC). All tests were performed on the osteoarthritic and healthy knees. The quadriceps MVC of the affected knee was reduced by 30%. The VL thickness of the affected knee was 10% smaller than that of the unaffected knee. VL activity seen on EMG did not differ between knees, but RF, VM and BF activity was greater in the unaffected than affected knee. Neuromuscular efficiency was higher (26%) in the unaffected knee. Knee position sense was reduced by 33% in the affected knee. Quadriceps weakness associated with knee osteoarthritis seems to be related to changes in muscle rather than pennation angle and changes in fascicule length.
OBJECTIVES:To study the clinimetric properties of the Dijon Physical Activity Score (PAS) in patients with coronary artery disease (CAD).PATIENTS:Two populations of patients with CAD: one group of stabilized patients from the RICO county-wide monitoring program and one group in the initial phase of a cardiovascular rehabilitation program (CVR group).METHODS:The patients carried out a maximal effort test on a cycle ergometer, plus two walking tests (a six-minute walk test and a 200 m fast walk test). They completed the Dijon PAS questionnaire on two occasions at an interval of 10 days. The reproducibility of the score and the latter's correlations with physical parameters were analyzed.RESULTS:Sixty-seven subjects were included and 52 answered the questionnaire both times. The average time spent answering the questionnaire was 173+/-37 seconds and reproducibility was satisfactory in the RICO group only. In this group, there were significant correlations between total score and maximal power during the effort test (r=0.41; P<0.05) and between the "sports/leisure activities" sub-score and maximal power (r=0.57; P<0.01). No correlations were found in the CVR group.CONCLUSION:The Dijon PAS is a simple, generic, reproducible and reliable score for measuring physical activity in patients with stable coronary artery disease but, because of the conjunction of confounding factors, it is not suitable for subjects who experienced a recent acute cardiac event. It could thus be used in epidemiological studies to determine the impact of a sedentary lifestyle and the efficacy of methods intended to counter sedentariness and to help design personalized secondary prevention programs.
BACKGROUND:The use of a fatigue monitoring questionnaire is an increasingly widespread practice in sports medicine.OBJECTIVE:To evaluate variations in the QFES fatigue questionnaire score according to periods of rest, training or competition.METHODS:Sixty-six skiers (either alpine or cross-country skiers, aged between 11 and 14 years and training for between 12 and 18 hours per week) filled out a total of 400 fatigue questionnaires over a four-year follow-up period.RESULTS:Our analysis of the QFES fatigue scores showed that only a small proportion of the skiers (5.25%, on average) exceeded the fatigue alert threshold at some point during the longitudinal follow-up; periods of competition correspond to rising fatigue scores, in comparison with the beginning of season and girls always have higher scores than boys (25.32+/-0.90 versus 20.46+/-0.91, respectively; p<0.0002). In contrast, there was no correlation between fatigue levels and the type of skiing discipline practiced.CONCLUSION:Our main results show that the QFES fatigue questionnaire is a sensitive tool for tracking down particular periods of fatigue (competition, intense training). Even though the mechanism of fatigue is still not fully understood (highlighting the complexity of this syndrome), the systematic use of a fatigue questionnaire when monitoring children performing competitive sport and high volumes of training confirms the possible preventive role of this type of tool.
Dans le contexte des douleurs réfractaires aux traitements médicamenteux, les techniques de neuromodulation occupent une place importante notamment en cas de douleurs neuropathiques sous couvert d’une sélection précise des indications tenant compte des données cliniques, anatomiques et neurophysiologiques. Le choix est essentiellement fonction de la sévérité et de la topographie des douleurs. Il s’agit de techniques totalement conservatrices, adaptables et réversibles nécessitant cependant une surveillance clinique et technique régulière dans un contexte pluridisciplinaire.
Revue de la littérature concernant la capsulite rétractile de hanche à partir de trois cas cliniques. À partir de trois cas cliniques, décrire la démarche diagnostique et la prise en charge thérapeutique. La capsulite de hanche, pathologie rare mais probablement sous-estimée touche préférentiellement les femmes d’âge moyen. L’examen clinique objective une limitation douloureuse de la mobilité articulaire de la hanche. Le diagnostic est confirmé par la réalisation d’une arthrographie qui démontre une capacité articulaire inférieure à 12 ml. L’arthrose et le syndrome douloureux régional complexe de type 1 sont les deux principaux diagnostics différentiels. Le traitement médical ou chirurgical est proposé en fonction de l’étiologie. La rééducation est fondamentale pour limiter les déficiences et incapacités fonctionnelles résiduelles. Peu décrite dans la littérature, la capsulite rétractile de hanche n’est cependant pas une rareté clinique. Fréquemment idiopathique, elle peut aussi être associée à une pathologie articulaire. Le traitement de base consiste en l’association d’injection intra-articulaire de corticoïde dépôt et d’une prise en charge en rééducation. Arthroscopie et mobilisation sous anesthésie sont utiles pour les cas récalcitrants.
Décrire l’état de santé et la situation professionnelle d’une population de patients atteints de sclérose en plaques en âge de travailler et comparaison d’un groupe de sujets qui travaillent (groupe T1) à un groupe de sujets qui ne travaillent pas (groupe T2). C’est une étude descriptive. Des données démographiques, médicales et professionnelles ont été recueillies par questionnaires à l’issue d’une consultation spécialisée de neurologie. Une analyse statistique descriptive et comparative a été réalisée. Soixante-seize patients ont été inclus dans l’étude : 54 travaillent (groupe T1), 22 ne travaillent plus (groupe T2). Le taux d’emploi était de 71 %, avec une durée moyenne d’évolution de la maladie de neuf ans au moment de l’étude. Le groupe T2 avait un niveau scolaire plus faible (p = 0,02), comportait davantage de formes progressives (p = 0,0001), d’atteintes motrices (p = 0,01), cérébelleuses (p = 0,02) ou cognitives (p = 0,03) avec un retentissement fonctionnel (score EDSS) plus important (p = 0,0001). En considérant le dernier poste de travail occupé dans les deux groupes, les personnes du groupe T1 étaient employées plus souvent dans des grandes entreprises (p = 0,03), ou dans le secteur public (p = 0,003), la reconnaissance de la qualité de travailleur handicapé (ex. Commission technique d’orientation et de reclassement professionnel [Cotorep]) était moins fréquente (p = 0,03). C’est aussi dans ce groupe que le travail demandait le moins de force physique (p = 0,05), le moins de précision manuelle (p = 0,05), avec le moins de problème d’accès (p = 0,03). Cette étude montre les différences qui existent dans cette population en fonction de la situation professionnelle. Elle souligne l’importance des variables cliniques et démographiques comme déterminants des différences de statut professionnel. Elle montre sans surprise que c’est dans le groupe sans activité professionnelle que le handicap au travail est reconnu le plus important. Les facteurs liés aux contraintes induites par le travail ressortent peu car les items du questionnaire ne sont pas suffisamment adaptés. To describe the health and professional status of multiple sclerosis patients of working age and to compare a group of patients in work (group T1) with a group of unemployed patients (group T2). A case-controlled study was performed. In the course of a specific consultation with a neurologist, demographic, medical and professional data were gathering using a questionnaire. Descriptive and comparative statistical analyses were then performed. A total of 76 patients were included in the study: 54 were in work (group T1) and 22 were unemployed (group T2). Hence, the employment rate was 71%, with an average time since disease onset of nine years at the time of the study. Low educational level (p = 0.02), disease progression (p = 0.0001), the presence of motor symptoms (p = 0.01), cerebellar symptoms (p = 0.02) or cognitive symptoms (p = 0.03), a worse EDSS (p = 0.0001) and a job requiring force (p = 0.05) or manual dexterity (p = 0.05) were found to be negative factors. Employment in the public sector (p = 0.003) or large companies (p = 0.03) were found to be protective factors. Access to the workplace was better for currently employed patients (p = 0.03). This study shows that differences exist within the MS patient population according to the professional situation. It underlines the importance of clinical and demographics variables as determinants of differences in employment status. Not surprisingly, unemployed patients are more likely to have been classified as handicapped workers. Factors linked to work-induced constraints did not emerge from the survey because the questionnaire items were not appropriate for addressing this latter issue.
L'objectif de notre étude était de déterminer l'effet du traitement conservateur (rééducation périnéale plus conseils d'hygiène de vie) dans la prise en charge d'un prolapsus urogénital en comparaison avec l'absence de traitement. Quarante-sept patientes d'âge moyen 53,42 ± 11,01 ans adressées à notre consultation pour prolapsus urogénital (cystocèle stade I ou II) ont été randomisées en deux groupes : un groupe de femmes ayant reçu un traitement conservateur (GTC) et un groupe sans traitement (GST) où les patientes n'ont bénéficié d'aucune mesure thérapeutique. Les résultats de la rééducation ont été appréciés par les éléments de l'examen cliniques, un bilan urodynamique et des échelles d'évaluation. Les résultats immédiatement après rééducation ont montré que la sensation de pesanteur pelvienne n'a persisté que chez cinq patientes (18,51 %) du GTC versus 14 patientes (70 %) du GST (p < 0,001). Une amélioration des scores de l'échelle MHU a été aussi constatée dans GTC en comparaison avec le GST avec une différence significative. Une amélioration du score de Ditrovie a été notée. La pression de clôture urétrale a passé de 54,4 ± 13,23 à 57,81 ± 12,8 cm H2O pour le GTC versus de 54,05 ± 12,18 à 52,95 ± 12,18 cm H2O dans le GST. L'enregistrement de la débitmétrie a montré une amélioration significative du débit urinaire maximal. Ces résultats ont été maintenus deux ans après l'arrêt de la rééducation chez 20 patientes du GTC. Le traitement conservateur peut être efficace chez la femme ayant un prolapsus urogénital de bas grade permettant une amélioration de la symptomatologie clinique et des paramètres urodynamiques. Ces résultats sont maintenus deux ans après l'arrêt de la rééducation. The objective of our study was to determine the respective effects of conservative treatment (pelvic muscle exercises plus advice on healthy living) and the absence of treatment in the management of urogenital prolapse. Forty-seven women (mean ± S.D. age: 53.42 ± 11.01 years) consulting for genital prolapse (grade I or II cystocele) were randomized into two groups: a conservative treatment group (CTG) and a nontreated group (NTG). The patients' outcomes were rated according to several parameters; a clinical examination, the "Measurement of Urinary Handicap" (MUH) scale, urodynamic tests, the Ditrovie quality of life scale and patient satisfaction on a visual analogue scale (VAS). The immediate post-treatment results showed that pelvic heaviness persisted in only five patients (18.51%) in the CTG compared with fourteen (70%) in the NTG (p < 0.001). Furthermore, a significant improvement in the MUH scale score was noted in the CTG compared with the NTG. The Ditrovie score also improved. The maximum urethral closure pressure (MUCP) ranged from 54.4 ± 13.23 to 57.81 ± 12.8 cm H2O in the CTG versus 54.05 ± 12.18 to 52.95 ± 12.18 cm H2O in the NTG. Uroflowmetry revealed a significant improvement in the maximum flow rate. These benefits were maintained two years after cessation of the conservative treatment in 20 of the CTG patients. Conservative treatment can be effective in the treatment of low-degree urogenital prolapse and enables the improvement of clinical symptoms and urodynamic parameters. The benefits were maintained two years after treatment cessation.
