
During the period 1957 to 1962, postpartum sterilization was performed on 5968 patients. Of these, 1055 patients (34%) were followed up in 1965. The patients were interviewed by a doctor, and a detailed history was taken followed by a general abdominal and pelvic examination, including a cervical smear for exfoliative cytology. The commonest indication for postpartum sterilization was grand multiparity (973 or 93.2% of the patients had 5 or more chldren), followed by obstetrical or medical indications (eg, toxemia of pregnancy; antepartum hemorrhage); ceasarian section, and socioeconomic reason. Average age for postpartum sterilization was 33 years; parity was 6.0, and number of living children at time of interview was 5.7. Tubal ligation by Pomeroy's technique using No. 2 plain catgut was done in 1049 cases. The findings show that postpartum tubal ligation does not significantly affect body weight nor the general state of health. No alteration in the menstrual pattern was observed among half of the patients. Minor menstrual disorders were observed in the other half. 4 cases had severe menorrhagia requiring hysterectomy. No gross pelvic pathology was seen. Psychological effects were also minimal and most patients had satisfactory marital relationship. 98.7% were happy about the operation. It was concluded that postpartum sterilization is a safe and reliable contraceptive method for women desiring permanent contraception.
Megestrol acetate alone (47 cases), combined (115 cases) with mestranol, or sequential with dienestrol diacetate, was prescribed for various gynecologic disorders or for studies of its progestational effects. Doses were usually 1 Planovin tablet (5 mg megestrol acetate and .1 mg mestranol) for 20 day cycles, or from 2.5-15 mg megestrol acetate with or without 2.5-5 mg dienestrol diacetate. Results of treatment were 4 of 10 permanent cures of secondary amenorrhea; 10 permanent cures of dysmenorrhea with temporary improvement in 6; 6 of 7 patients with endometriosis were relieved, although 2 had to maintain cyclic treatment after up to 9 months continuous ingestion; 20 of 21 cases of metrorrhagia were relieved; 6 of 7 women with menopausal symptoms were improved; 18 of 21 women used Planovin successfully for contraception. Studies with 21 women who took megestrol acetate before undergoing planned sterilization showed that neither 5 mg megestrol acetate or .1 mg mestranol alone inhibited ovulation in all women, as observed during laparotomy, but the combined tables was 100% effective.

IN recent years numerous papers have appeared on the use of vaginal cytology as a guide to the prognosis and treatment of habitual and threatened abortion (Hall, 1945 ; Gaudefroy, 1950; Randall et al., 1950; Pundel and van Meensel, 1951 ; Rodriguez Lima and Kamnitzer, 1952; Pierce and Cope, 1954; Pierce, 1957; Pundel, 1959; Hochstaedt, Lange and Spira, 1960; Nesbitt, Garcia and Rome, 1961). It has been shown that the likelihood of ultimate abortion in these cases can be assessed with a reasonable degree of accuracy by estimating the percentage of karyopyknotic cells in the vaginal smear. Early in the course of a cytologic study undertaken with the object of determining the value of the above-mentioned method in assessing the ultimate prognosis in cases of early threatened abortion, it was discovered that the vaginal smears of some of these patients contained multinucleated cytoplasmic masses which could be definitely designated as chorionic syncytial cells. These were identical in appearance with the chorionic cells previously observed by us in the immediate post-abortion and postpartum smears (Youssef and Fayad, 1963). The detection of chorionic cells in the smears of patients who clinically had only early threatened abortion seemed to be a finding of considerable interest which to our knowledge has not hitherto been described. The incidence of these cells in early threatened abortion smears has been investigated and the prognostic significance of their presence with regard to the outcome of pregnancy has been studied and compared with the already known prognostic criteria based on estimation of the karyopyknotic index of the U.A.R.
