
Acute mesenteric ischemia (AMI) is a rare surgical emergency as it involves perfusion and need frequent surgical interventions.It is a life threatening emergency with a poor prognosis.It is classified according to the etiological basis thromboembolism, non-obstructive and venous origin.AMI has various risk factors ranging from cardiac arrhythmias to the intraabdominal hypertension.Apart from the signs and symptoms, the multi-detector computer tomography remains a diagnostic tool with accuracy.In the management of AMI patients' initial resuscitation, hydration and analgesia play important role, however the operative /interventional management is either endovascular thrombectomy with or without stenting when there is no bowel involvement or peritonitis.The laparotomy with open vascular thrombectomy with vascular graft, it is indicated when bowel involvement and peritonitis patients.Often the whole bowel is gangrenous and no further treatment is needed because of certain mortality.The 2 nd step is to know the extent of bowel involvement meticulously and resection of bowel with necrosis or gangrene with anastomosis, further relook laparotomies and resection of the bowel may be need.Initially abdomen will be closed with Bagota bag or VAC (Vacuum Assisted Closed) dressing and final staged abdominal closer, once pathology resolves completely.As far as prevention of all types
Chronic kidney disease (CKD) has become a worldwide public health priority and is estimated to affect approximately 12% of the global population.CKD is associated with an increased cardiovascular morbidity, premature mortality, and a substantial economic burden.Increased generation of reactive oxygen species has been observed throughout CKD progression, suggesting that mitochondrial dysfunction may be important in the pathogenesis of kidney disease.The mitochondrial genome is a circular double stranded DNA molecule composed of 16,569 base pairs harbouring 37 genes, which encode 13 key proteins of the electron transport chain along with two rRNAs and 22 tRNAs.At least 2,309 nuclear genes are also necessary for efficient mitochondrial function.Mitochondrial dysfunction leads to a reduction in ATP production, cellular damage and loss of renal function.Damage or mutations in mtDNA will lead to defects in mitochondrial oxidative phosphorylation (OXPHOS), which can result in a range of clinical symptoms involving several different organs broadly termed as 'mitochondrial diseases'.Defects in nuclear encoded genes may also lead to OXPHOS defects, abnormal protein translation and loss of mtDNA copy number.This review provides an update on molecular features influencing mitochondrial homeostasis and function, highlighting how these are compromised during CKD.
Background: Few targeted therapies are currently available for triple-negative breast cancers (TNBC) due to their lack of estrogen receptor-α and progesterone receptor expression and HER-2 amplification. In the clinic, TNBC is associated with a range of adverse biologic features including a highly metastatic phenotype and early patient relapse, leading to poor clinical outcomes. Identifying effective treatment strategies to inhibit or prevent TNBC metastasis could help to improve patient treatment options and clinical outcomes.
Background:The adverse effects of breast cancer and treatments to confront it impact negatively on the sexual health, not only of the women who endure it, but also their sex partners.Objective: This study sought to determine the relationship of sexual health and the impact of treatments between women subjected to treatment due to breast cancer and their sex partners. Methodology:This was a correlational-type study with cross-sectional design and quantitative approach.The sampling was non-probabilistic and the sample was comprised of 103 couples from five Colombian cities.The women received the Sexual Functioning Questionnaire-Women (SFQ-W); and the version for the sex partners, denominated Sexual Functioning Questionnaire-Men (SFQ-M) by Syrjala KL et al.For the variable Impact of treatments, the study used the subscale Impact of treatments version for women and men by Syrjala KL et al., 2000, with prior informed consent. Background:Virtual Reality in the field of medicine is making a considerable progress in psychiatry.Virtual reality exposure therapy (VRET) in particular can serve as a preferable solution for PTSD (Post-Traumatic Stress disorder) patients who avoids or finds retrieving image difficult.Relevant studies were reviewed to identify the pros and cons of virtual reality technology applied to psychiatry and to determine its medical effect. Methods:The application of virtual reality therapy to the field of psychiatry e.g., Panic Disorder (PD), Post-Traumatic Stress disorder(PTSD), Mild Cognitive Impairment(MCI), and Attention Deficit Hyperactivity Disorder(ADHD) and the strengths and weaknesses of VR therapy were investigated. Results:VRET can serve as a preferable alternative to PTSD patients and can be more economical than actual exposure therapy as it is accessible, convenient and reproducible.However, the VRET can cause vertigo, nausea, vomiting, and reduced awareness, and there is a concern of abuse by uncertified practitioners.For PTSD, according to the systematic review of VRET, the dropout rates of VRET is not higher than conventional exposure therapy, also 7 of 10 studies, the VRET is effective to PTSD patients, their effects were not different with conventional therapy as well.The important thing of VRET is that design contents scenario and establishing each hierarchy.For panic disorder, in respect with effectiveness, VR therapy compares favorably with conventional therapy.Conventional therapy versus VR therapy, it is revealed that similar therapy efficacy in exposure therapy, Interceptive exposure therapy, cognitive behavioral therapy.VR therapy is comparable to the conventional therapy in terms of efficacy.Treatments that had comparable clinical effect with the conventional VR therapy were exposure therapy, interoceptive exposure therapy, and cognitive behavior therapy.For MCI, studies using VR to cognitive training treats general cognitive function, such as frontal function, memory, spatial navigation and so on.For ADHD, researchers and clinicians may expect lower dropout rates in treatment and positive results in cognitive training. Conclusion:VR therapy was not inferior to conventional therapy in its application to psychiatry regarding PTSD, ADHD etc., and holds high potential as it enables convenient use of situations that cannot be easily reproduced in real life.However, technical support and effort are necessary to prevent its side effects including vertigo, reduced awareness and nausea.
INTRODUCTION:The Ebola Virus Disease Epidemic (EVD), confirmed in March 2014 in Guinea, spread to Liberia, Sierra Leone, resulting in 28,646 cases over two years, including 11,323 deaths across ten countries.Côte d'Ivoire, which shares borders with Guinea and Liberia, due to high level of population movements, was considered a country at high risk of contamination.Despite its proximity to epidemic outbreaks, Côte d'Ivoire recorded no cases of Ebola virus disease.Faced with a threat posed by this dreadful disease, the Ivorian authorities put in place a series of measures to prevent the spread of the epidemic throughout the territory.The present work aims at describing a control strategy in order to increase knowledge on methods to prevent the cross-border spread of the epidemics. PREVENTION STRATEGIES:The strategic axes were mainly coordination, surveillance, border health control, case management and community involvement. RESULTS:A total of 92 alert cases including 10 suspects were reported for which the Ebola tests were negative.There were 66,880 phone calls recorded across the two hotlines.Following the confirmation of the epidemic on 21 March, 2014, four forward coordination posts were established in the border areas of Guinea and Liberia.These posts made it possible, with the involvement of the administrative authorities, opinion leaders, traditional and religious communities, to raise awareness and inform all the populations of the border localities about the methods of transmission and measures to prevent the disease, in particular hand washing, preventing the handling of corpses, hunting and the consumption of game. CONCLUSION:Despite the measures taken to address the threat of Ebola, the risk of spreading the epidemic in Côte d'Ivoire existed.The strategies developed made it possible to prevent the cross-border spread of the epidemic in Côte d'Ivoire.The prevention measure relating to community involvement could be considered the fundamental factor that helped protect the Ivorian population against this dreadful disease.Certainly, there was a high risk of spreading the epidemic in Côte d'Ivoire; yet; not absolute.
Background:Extensive research has been conducted on wheat as food allergen probably due to high meal dependence of world population on wheat, but less research is conducted on wheat acted as aeroallergen. Methods:To characterize the protein fractions in wheat flour, responsible for aeroallergy in the asthmatic patients, twelve out of forty-five asthmatic patients were screened.These patients were sensitive to wheat and suspected of having wheat aero-allergy by their clinical allergic history, positive skin prick test and enzyme-linked immunosorbent assay (ELISA).Total level of protein in water salt soluble wheat extract was estimated through Lowry's method.Sodium dodecyl sulfate -polyacrylamide gel electrophoresis (SDS-PAGE) was used for the separation of protein fractions in wheat and immunoblotting assay was carried out to target the protein fractions responsible for aero-allergy in screened asthmatic patients. When a person has developed alcohol addiction the addictive condition does not vanish even when one has stopped drinking for a long time; instead due to the Alcohol Deprivation Effect, just having one drink later, may drive to relapse.Alcohol use disorder may turn into a persistent and relapsing disease.Many patients stop drinking easily, even on their own and without treatment when they are overwhelmed by "problems" derived from continued heavy drinking.Once achieved the stable abstinence, they may remain in remission for weeks or months but when they try a drink again, they may be affected by Alcohol Deprivation Effect which is associated to a powerful desire towards a high speed, continuous, unstoppable drinking, ending on the negative consequences of heavy drinking again.Opioid receptor antagonists (naltrexone and nalmefene) block Alcohol Deprivation Effect, helping alcohol dependent patients to avoid heavy drinking, if they are motivated to do it, furthermore if a patient maintains low-risk alcohol consumption it's possible to consider a clinical remission.Several controlled studies have demonstrated opioid antagonists effectiveness reducing alcohol consumption, especially among people with a low-severity alcohol use disorder.However, high-severity alcohol dependent patients must to start their recovery with a detoxification treatment since they have a better response to abstention than to a reduction oriented treatment program.
HIV infection has already been cured by hematopoietic cell transplantation.The cure was reported in the New England Journal of Medicine in 2009 by Dr. Gero Hütter and associates from Germany (1).The patient, Timothy Brown, had acute myeloid leukemia and HIV infection and is now referred to as "The Berlin Patient".It was well known at that time that HIV-1 enters host cells by binding to a CD4 receptor and then interacting with either CCR5 or the CXC chemokine receptor (CXCR4).Homozygosity for a 32-bp deletion (delta32/delta32; CCR5-/-) in the CCR5 allele results in an inactive CCR5 gene product and consequently confers high resistance against HIV-1 infection.Following transplantation using stem cells from an adult donor who had the CCR5-/-mutation, HIV could not be detected in Timothy Brown's peripheral blood, spinal fluid, lymph nodes, bone marrow, brain or GI mucosa as assessed with RNA and proviral DNA PCR assays.Further, there has been no recurrence of HIV in more than 10 years.Thus, the "Berlin Patient" has had a functional cure if not a sterilizing cure of HIV.
Mycobacteria are aerobic organism that are non-sporulating, non-motile (M. marinum is an exception) and have a waxy, relatively impermeable cell wall consisting of arabinogalactan and mycolic acids. There are over 188 mycobacterial species. Mycobacteria have a high GC content (around 65%) and the ability to grow over a wide temperature range (25-53 C). They are largely environmental organisms (M. tuberculosis and M. leprae are exceptions) and are slowly growing bacteria (M. leprae cannot be cultured on artificial media) with a long generation time of approximately twenty hours.

Preventive strategies and nutritional interventions seem to be promising approaches to delay neurocognitive decline and reduce the risk of neurodegenerative diseases.Among the various nutritional protocols, numerous studies confirm: the Mediterranean diet is the best food style to preserve the decay of our brain and live long.The elderly who constantly follow this type of diet would have a brain less "consumed" than those who follow a different food style.But what are the magic molecules present in fruits, vegetables, olive oil, legumes, able to protect us from aging and degenerative phenomena?The polyphenols: phytochemicals that fight cellular aging by counteracting the action of free radicals.These substances are able to stimulate cell repair systems, to amplify the endogenous antioxidant defenses, to specifically inhibit the action of inflammatory molecules.The trigger of this defensive response is linked to the peculiar ability of the polyphenols to modulate specific signaling mechanisms and transcription factors.Here we review the role of some "polyphenolic-food" of Mediterranean diet for the maintenance of cognitive performance, focusing specifically on human studies and the beneficial effects associated with overall diet composition, rather than single nutrient supplementations, for the prevention or the delay of neurodegenerative diseases.
Neuroblastomas are the most common extracranial cancers in children.They have a complex morphology and heterogeneous clinical course.Amplification of the MYCN oncogene is seen in 50% of high-risk neuroblastomas and is associated with a poor clinical outcome with a higher rate of progression and relapse when compared with tumors without MYCN amplification.MYCN amplification drives genomic instability, dynamic remodeling of chromosomes and changes in nuclear organization.However, understanding of how the chromatin structure responds to MYC overexpression in neuroblastoma is nebulous.In the present study, we examined neuroblastoma samples with and without MYCN amplification as well as in the neuroblastoma cell line SH-EP transfected with MYCN pUHD.Our neuroblastoma cases segregated into one of three prognostic clusters of increasingly poor prognosis.Clusters I and II encompassed tumors with no MYCN amplification, while cluster III included tumors with MYCN amplification.Measurements of both the DNA structure (p<0.001) and structure of the DNA-free space (p < 0.001) showed significant DNA structural alterations associated with MYCN amplification indicating that there is a progressive disruption of nuclear DNA organization with MYCN amplification suggesting that MYCN may play an functional role in determining the nuclear structure in neuroblastoma.
This study has been undertaken to investigate the influence of copolymers poly-L-co-D, L-lactic acid (PLDLA) 70:30 and Poly (ethylene glycol)-block-poly (propylene glycol)-block-poly (ethylene glycol) (P407) in the transmural transport of 3'-azido-3'-deoxythymidine (azidothymidine (AZT), zidovudine).Based on anterior studies we use the proximal duodenum to evaluate, in vitro, the apparent intestinal permeability.The binary solid dispersion (SD) was prepared with, AZT, PLDLA, and P407.The supercritical antisolvent (SAS) technique was used to obtain the SD, whose was studied by scanning electron microscopy (SEM).The differential scanning calorimetry (DSC) showed that pure AZT had an endothermic peak corresponding to AZT melting point of 125,10 °C, which indicated its crystalline nature.In the SD the same melting point peak was observed.DSC thermogram indicated that thermal properties of P407 were altered in the SD, but the SD did not alter the thermal properties of AZT and PLDLA.Powder X-ray diffraction (X-RD) was used to detect, qualitatively, a material with long-range order.Sharper diffraction peaks indicate more crystalline AZT and P407.The diffraction spectrum of pure AZT and P407 confirmed their crystalline nature.Sharper diffraction peaks indicate more crystalline AZT and P407.The diffraction spectrum of pure AZT and P407 confirmed their crystalline nature.The X-RD spectrum of SD showed modifications in the crystalline lattice, the physical mixture
Background:When someone develops an alcohol use disorder (AUD), their addiction conditioning does not go away simply by refraining from drinking for some weeks or months.On the contrary, due to the deprivation effect, if one day they try to have just one alcoholic drink they will feel a strong and imperative biological necessity to keep drinking very fast, not being able to stop, which will lead to immediate negative consequences.The good news is that opiod receptor antagonists, such as naltrexone and nalmefene, have shown efficacy for reducing the deprivation effect, and for reducing alcohol consumption, in people who suffer from low-severity alcohol use disorders.Alcohol use disorders may turn into a persistent and relapsing disease.Many patients stop drinking easily, even without treatment, when they are overwhelmed by their "problems" associated with heavy drinking.Afterwards they may remain in remission for weeks or months.However, relapses may be devastating either for patients or for their families.The hypothesis of this study is that the self-monitoring treatment program with nalmefene may obtain a fast, significant, and continued reduction in alcohol consumption, without negative consequences in lowseverity AUD patients.
Evidence-based interventions are available which, if scaled-up, can prevent roughly half of neonatal deaths; these include tetanus toxoid immunization to mothers, clean and skilled care at delivery, newborn resuscitation, exclusive breastfeeding, clean umbilical cord care, and/or management of infections. Apart from these conventional approaches, few innovative approaches have been developed to reduce neonatal mortality.
