
Retinitis pigmentosa (RP) ist mit einer Prävalenz von etwa 1: 4000 die häufigste Form der erblichen Netzhauterkrankungen weltweit. Erbliche Netzhauterkrankungen sind die führende Ursache für Sehbehinderung und Erblindung im Kindes-, Jugend- und erwerbsfähigen Alter in Österreich. Derzeit ist mit Voretigene neparvovec die einzige zugelassene Gentherapie für eine Unterform der RPE65-assoziierten Retinopathie verfügbar, während Gentherapie, Pharmakotherapie, Neuroprotektion und Stammzelltherapie intensiv klinisch erforscht werden. Die systematische Literaturrecherche erfolgte nach den PRISMA-P-Kriterien, die eine standardisierte und transparente Methodik gewährleisten. Klinische Studien zur Therapie der Retinitis pigmentosa aus dem Zeitraum von 2019 bis August 2025 wurden hinsichtlich Wirksamkeit und Sicherheit der jeweiligen Therapieansätze systematisch analysiert, wobei die ROBINS-I-Kriterien Anwendung fanden. Von 4004 identifizierten Publikationen wurden 22 eingeschlossen: 5 zur Gentherapie, 6 zur Pharmakotherapie, 6 zur Neuroprotektion und 5 zur Stammzelltherapie. Die Studien zur Gentherapie zeigten funktionelle Verbesserungen bei insgesamt guter Sicherheit. Sowohl pharmakologische als auch neuroprotektive Ansätze bewahrten funktionelle und strukturelle Stabilität, während stammzellbasierte Verfahren teilweise eine verbesserte Sehschärfe und makuläre Sensitivität erreichten. Gen- und stammzellbasierte Therapien zeigen vielversprechende Resultate, deren Langzeitsicherheit und Wirksamkeit jedoch noch bestätigt werden müssen. Neuroprotektive und pharmakologische Strategien bleiben aufgrund ihrer breiten Verfügbarkeit eine genunabhängige Behandlungsoption. Es sind größere, kontrollierte Studien notwendig, um die langfristigen Effekte und die klinische Bedeutung dieser Therapieansätze zu ermitteln.
Die X‑chromosomale Retinitis pigmentosa (XLRP) ist eine seltene und progressive Form der Retinitis pigmentosa (RP), die aufgrund der X‑chromosomalen Vererbungsform üblicherweise Männer betrifft, früh eintritt und rasch fortschreitet. Ursache der Erkrankung ist in den meisten Fällen eine Mutation im RPGR-Gen. Frauen gelten aufgrund der Vererbungsform meist als asymptomatische Anlageträgerinnen. Dieser Fallbericht handelt von einer 31-jährigen Frau, die an der Universitätsklinik für Augenheilkunde der Medizinischen Universität Graz aufgrund einer progredienten beidseitigen Sehverschlechterung und Nyktalopie untersucht wurde. Es wurde eine umfassende ophthalmologische Untersuchung durchgeführt, die eine detaillierte Familien- und Sozialanamnese erfasste, zusätzlich wurde eine genetische Analyse vorgenommen. Die bestkorrigierte Sehschärfe lag bei 20/30 und 20/60. In der Goldmann-Perimetrie zeigten sich beidseits ausgeprägte Gesichtsfeldverengungen. Aufnahmen mittels optischer Kohärenztomographie (OCT) und eine Fundoskopie zeigten Retinitis-pigmentosa-typische chorioretinale Atrophien sowie Papillenblässe und typische Gefäßveränderungen. Die Familienanamnese ergab, dass sowohl der Bruder als auch der Onkel der Patientin ebenfalls von Retinitis pigmentosa betroffen sind. Die genetische Untersuchung ergab eine wahrscheinlich pathogene Mutation im RPGR-Gen (c.2635G>T) mit zufälliger X‑Inaktivierung, welche sehr wahrscheinlich ursächlich für eine klinisch manifeste X‑chromosomale Retinitis pigmentosa ist. Weibliche Mutationsträgerinnen mit einem zufälligen X‑Inaktivierungsmuster können einen Krankheitsverlauf zeigen, der dem Verlauf männlicher Patienten ähnelt. Bei weiblichen Patientinnen mit Netzhautdystrophie-typischen Symptomen sowie Retinitis-pigmentosa-typischen Befunden und positiver Familienanamnese mit betroffenen männlichen Verwandten sollte eine umfassende genetische Abklärung stattfinden, die auch die X‑chromosomal vererbten Formen einer Retinitis pigmentosa einschließt. Ein manifestes Krankheitsbild kann jedoch auch bei zufälliger X‑Aktivierung auftreten, was auf weitere, noch unbekannte Mechanismen in der Krankheitsentstehung hinweist.

Background Vogt-Koyanagi-Harada (VKH) disease is a rare, multisystemic autoimmune disease that affects melanin-containing tissues such as the eye, the inner ear, the meninges, and the skin. The typical ocular manifestation is bilateral granulomatous panuveitis with serous retinal detachments. The diagnosis is primarily made clinically using optical coherence tomography (OCT) and angiography. Systemic corticosteroids are used as the initial therapy, and immunosuppressive drugs are used for long-term treatment, if necessary. Materials and methods In this retrospective case series, we present the clinical courses and treatments of all patients with VKH disease (n = 9) who first presented at the Department of Ophthalmology at the Medical University of Graz between the years 2006 and 2022. Results The average age of patients was 37 years, with women affected more than three times as often as men (ratio 7:2). All patients reported visual deterioration at first presentation, 6 patients experienced headaches, and 2 patients had auditory symptoms. Initial treatment consisted of oral corticosteroids (50-100 mg/day) in 7 patients, with 2 patients initially receiving intravenous corticosteroids. Recurrences occurred in 5 cases; immunosuppressive therapy was added in 5 patients. Two patients experienced multiple recurrences and were ultimately treated with Tumor necrosis factor (TNF)-alpha blockers. Conclusion The results of this retrospective case series underscore the challenges in the treatment of VKH disease. The high rate of recurrences and the need for supplementary immunosuppressive drugs in many cases highlight the importance of long-term supervision and adaptation of treatment tailored to the individual requirements of patients. The use of TNF-alpha blockers in recurrent cases seems to be a promising option for long-term treatment.
To compare binocular distance optical performance in non-presbyopic optometry students under different contact lens conditions: no lenses, monofocal lenses, and multifocal lenses. A total of 20 optometry students (age: 23.00 ± 2.58 years) meeting eligibility criteria were recruited. The study employed a randomized crossover design with two sessions: (1) baseline without contact lenses (W-CL), and (2) experimental where each participant was fitted with monofocal contact lenses (M-CL; Clariti 1‑Day) and multifocal contact lenses (MF-CL; Clariti 1‑Day Multifocal) in random order, using the permutation method balanced 1:1 across participants within the same session. Optical performance was assessed by measuring binocular distance visual acuity parameters (corrected distance visual acuity [CDVA] and dynamic visual acuity [DVA]) and binocular depth perception parameters (fixation disparity and stereopsis), using the OptoTab POLAR 24″ SMT4V device under standardized conditions. Significant differences in visual acuity parameters (CDVA and DVA) were found across conditions (repeated measures ANOVA, p < 0.001), with post hoc analysis showing reduced acuity for MF-CL vs. W‑CL and M‑CL (Sidak, all p ≤ 0.002). For both parameters, effect sizes were negligible between W‑CL and M‑CL (all Cohen’s d = 0.07–0.09), and small and clinically meaningful when MF-CL was involved (all Cohen’s d = 0.08–0.17). No significant differences in depth perception parameters (fixation disparity or stereopsis) were found across conditions (all p ≥ 0.103) with negligible to small effect sizes (Cohen’s d, all p ≥ 0.38). In non-presbyopic participants, MF-CL significantly reduced visual acuity parameters compared to M‑CL and W‑CL, but not optical performance related to binocular vision.
Recent epidemiological research showed that inherited retinal dystrophies are the leading cause of severe visual impairment among children and working adults across Europe, notably in Austria. Currently, available treatments are primarily supportive with limitations. However, promising new therapeutic approaches such as retinal prostheses offer prospects for addressing these challenges. Approved retinal prostheses are currently restricted to epiretinal and subretinal implants. Here, we analyze and compared the medical risks and subretinal enhancements in visual function of both systems considering their respective rehabilitative differences. Furthermore, we explore the current state of knowledge and future developments of retinal prostheses. This systematic review is based on PRISMA‑P using diverse scientific databases. Only non-randomized clinical studies and case series were included. All included studies were individually assessed with the ROBINS‑I tool of the Cochrane Collaboration and the TREND checklist for bias assessment. Out of 564 screened records, 16 non-randomized controlled clinical studies were included, comprising eight on subretinal implants and eight on epiretinal implants. Data analysis revealed that epiretinal implants provided more consistent outcomes in motion detection and object localization, while subretinal implants showed greater variability with occasional superior gains in visual acuity but at the cost of higher complication rates. The overall bias risk was moderate. Both prosthesis systems can restore basic visual functions but differ in consistency, safety profile, and methodological rigor. Further standardized, long-term studies are needed to enable reliable comparisons and optimize patient outcomes. A glimpse into the future promises many new therapeutic approaches, such as artificial intelligence-supported retinal prostheses.
Das Vogt-Koyanagi-Harada-Syndrom (VKHS) ist eine seltene, multisystemische Autoimmunerkrankung, welche melaninhaltige Gewebe wie das Auge, das Innenohr, die Meningen und die Haut betrifft. Die typische okuläre Manifestation ist eine bilaterale, granulomatöse Panuveitis mit serösen Netzhautablösungen. Die Diagnose erfolgt primär klinisch mithilfe von optischer Kohärenztomographie (OCT) und Angiographie. Die Initialtherapie erfolgt durch systemische Kortikosteroide, Immunsuppressiva können zur Langzeittherapie bei Bedarf angewandt werden. In dieser retrospektiven Fallserie werden die klinischen Verläufe und Therapien sämtlicher PatientInnen mit Vogt-Koyanagi-Harada-Syndrom (n = 9) beschrieben, welche an der Universitäts-Augenklinik Graz zwischen den Jahren 2006 und 2022 erstvorstellig wurden. Das Durchschnittsalter der PatientInnen betrug 37 Jahre, Frauen waren mehr als dreimal so häufig betroffen wie Männer (Verhältnis 7:2). Alle PatientInnen berichteten von einer Sehverschlechterung bei der Erstvorstellung; 6 PatientInnen klagten über Kopfschmerzen, und 2 PatientInnen beschrieben auditive Beschwerden. Die Initialtherapie bestand aus oralen Glukokortikoiden (50–100 mg/Tag) bei 7 PatientInnen, wobei zwei PatientInnen initial mit intravenösen Kortikosteroiden behandelt wurden. Rezidive traten in 5 Fällen auf, bei 5 PatientInnen wurde im Verlauf eine zusätzliche immunsuppressive Therapie eingeleitet. Zwei PatientInnen erlitten mehrere Rezidive während des Beobachtungszeitraums und wurden schließlich mit einem Tumornekrosefaktor-alpha (TNF-α)-Blocker behandelt. Die Ergebnisse dieser retrospektiven Fallserie unterstreichen die Herausforderungen bei der Behandlung des VKHS. Die hohe Rate an Rezidiven und die Notwendigkeit ergänzender Immuntherapien in vielen Fällen zeigen, dass die Langzeitbetreuung und Anpassung der Therapie an die individuellen Bedürfnisse der PatientInnen entscheidend sind. Die Verwendung von TNF-α-Blockern bei rezidivierenden Fällen scheint eine vielversprechende Option darzustellen.
Background The demographic change in Germany is leading to an increase in age-related eye diseases such as cataracts. To assess the patient population at the time of cataract surgery, a database analysis was conducted. Material and methods We conducted a secondary data analysis of the "Management-und-Integration-der-Katarakt-OP-Dokumentation" (MIKADO) database of the AugenAllianz. In this process, on average the first 50 cataract surgeries performed by a surgeon per quarter year were retrospectively documented using standardized survey forms. In total, data from 18 surgeons across 12 surgical centers were included within the period 2012-2017. The distribution of age at the time of surgery, gender, eye laterality, American Society of Anesthesiologists (ASA) risk classification within the cohort, type of anesthesia, pre-existing ophthalmological conditions and general medical conditions were determined for the patient collective. Results A total of 21,868 eyes (58% female, 42% male) from patients aged between 18 and 103 years were included in the analysis. A significant increase in cataract surgeries was observed in the age group 75-79 years, with a median age of 75 years. Overall, women underwent surgery slightly later than men (mean age 74.3 years vs. 73.29 years). The right eye (58%) was more frequently surgically treated first than the left eye (42%). In this sample 23% of patients underwent topical anesthesia, 53% peribulbar anesthesia, 7% retrobulbar anesthesia and 14% general anesthesia or analgosedation. Conclusion A similar age distribution was observed in Germany compared to the European average. Comorbidities such as hypertension and surprisingly diabetes were associated with a higher age at the time of surgery. Additionally, the choice of anesthesia type was influenced by the presence of comorbidities.
To evaluate the short-term effectiveness of laser goniopuncture (LGP) after deep sclerectomy (DS) in reducing intraocular pressure (IOP) in patients with open-angle glaucoma (OAG). This prospective pilot interventional case series included 16 eyes of 16 patients with primary OAG who had undergone DS but failed to achieve the target IOP. Laser goniopuncture was performed using an Nd: YAG laser. Patients were followed up for 3 months with examinations at 1 day, 1 week, 1 month, and 3 months post-laser. Primary outcomes included the mean IOP, absolute and relative success rates, and failure rates, as well as the proportion of patients achieving a ≥ 30
Myopic choroidal neovascularization (mCNV) is a vision-threatening complication of pathological myopia, which, if left untreated, leads to rapid and severe visual deterioration. While short-term treatment outcomes are well-documented, significantly less long-term data is available. This study therefore aimed to evaluate long-term treatment outcomes of anti-VEGF therapy in patients with mCNV. This retrospective study included patients treated for mCNV between January 2018 and March 2024 at the Department of Ophthalmology, University Medical Center Mainz, with a minimum follow-up of 30 months. Primary endpoints included changes in visual acuity (VA) as functional measure as well as changes in central retinal thickness (CRT) and central 1‑mm macular volume (CRV) as morphological measures. To investigate the effect of re-treatment on final treatment outcomes, a subgroup analysis was performed between patients who received injections only within the first 12 months since initiation of therapy and patients requiring additional intravitreal injections (IVI) beyond the first year. A total of 22 eyes of 18 patients were included, with a mean follow-up of 5.0 ± 1.97 years. At the final follow-up, in comparison to baseline, VA remained stable (p = 0.566), while significant improvements in CRT (p < 0.001) and CRV (p = 0.002) were observed. Baseline VA was positively and significantly correlated with final visual function (r = 0.653, p = 0.002). Besides a higher number of IVIs in the re-treatment group, no differences in treatment outcomes were observed between patients requiring re-treatment beyond 12 months of therapy and patients treated only within the first year of the first IVI. Following long-term anti-VEGF therapy in patients with mCNV, a stable VA, as well as a reduced CRT were observed. Baseline VA was predictive for long-term visual function, and re-treatment after the first 12 months of therapy did not negatively impact final outcome measures.
The healthcare system is an important source of global greenhouse gas emissions. This particularly concerns ophthalmology which has a high volume of operations during which an enormous amount of waste is produced. The aim of this survey carried out by the Primary Physician Committee was to compare the hospital processes of various departments with respect to sustainability and to estimate the amount of waste generated during surgical activities. For this purpose an anonymous written questionnaire was distributed among the various ophthalmology departments. Participation was voluntary. Finally, the collected data were systematically analyzed. The questionnaire contained 37 questions focussing on 4 areas: 1) hospital processes, quality management and organization, 2) intravitreal injections (IVOM), 3) cataract surgery and 4) surgery of the posterior eye segment. The results of the survey showed a substantial savings potential in all areas, which led to the conclusion that increases in efficiency and optimization are possible.
Der demografische Wandel in Deutschland führt zu einem Anstieg altersbedingter Augenerkrankungen wie der Katarakt. Um eine Einschätzung über das Patientenkollektiv zum Zeitpunkt der Kataraktoperation zu geben, erfolgte eine Datenbankanalyse. Wir führten eine Sekundärdatenanalyse der Management-und-Integration-der-Katarakt-OP-Dokumentation(MIKADO)-Datenbank der AugenAllianz durch. Für die durchgeführten Auswertungen wurden retrospektiv die durchschnittlich ersten 50 Kataraktoperationen eines Operateurs pro Quartal mittels standardisierter Erhebungsbögen dokumentiert. Insgesamt wurden hierzu Daten von 18 Operateuren an 12 OP-Zentren in einem Zeitraum zwischen 2012 und 2017 eingeschlossen. Es wurde die Verteilung des Alters bei Operation, des Geschlechts, der Augenseite, der „American Society of Anesthesiologists“(ASA)-Risikoklassifizierung im Kollektiv, der Anästhesieverfahren, der ophthalmologischen Vorerkrankungen sowie der Allgemeinerkrankungen für das Patientenkollektiv bestimmt. Poromas are common tumors of the epidermal sweat duct unit. They are divided into hidroacanthoma simplex, poroid hidradenoma, dermal duct tumors, and eccrine poromas, which commonly arise in palms and soles. We report a case of a 59-year-old female patient with a large right upper lid mass, exhibiting rapid growth and necrosis with ulceration. Histopathology after an incisional biopsy confirmed diagnosis of eccrine poroma. Excision and reconstruction were subsequently performed with good cosmetic and functional outcome.
Das Gesundheitswesen ist ein bedeutender Verursacher von globalen Treibhausgasemissionen, dies betrifft insbesondere die Augenheilkunde mit ihrem hohen Operationsvolumen, wodurch eine enorme Menge an Abfall erzeugt wird. Ziel dieser von der Primarärztekommission durchgeführten Umfrage war es, die Krankenhausprozesse verschiedener Abteilungen hinsichtlich Nachhaltigkeit zu vergleichen und das Abfallaufkommen im Zuge der operativen Tätigkeit abzuschätzen. Dafür wurde ein anonymer schriftlicher Fragebogen an die unterschiedlichen Augenabteilungen in Österreich verteilt. Die Teilnahme war freiwillig. Im Anschluss wurden die gesammelten Daten systematisch analysiert. Der Fragebogen umfasste 37 Fragen, die sich auf 4 Bereiche konzentrierten: 1. Krankenhausprozesse, Qualitätsmanagement und Organisation, 2. Intravitrealinjektionen (IVOM), 3. Kataraktoperationen, 4. Operationen am hinteren Augenabschnitt. Die Umfrageergebnisse haben in allen Bereichen ein erhebliches Einsparpotenzial aufgezeigt, was zu dem Schluss führt, dass Effizienzsteigerungen und Optimierungen möglich sind.
Bakterielle Infektionen sind in Ländern mit hohem Einkommen bei Weitem die häufigste Ursache für die infektiöse Keratitis. Das klinische Erscheinungsbild kann je nach Art und Spezies der Bakterien sehr unterschiedlich sein und reicht von kleinen oberflächlichen Infiltraten bis hin zu nekrotisierenden Formen. Die zahlreichen Klassen verfügbarer Antibiotika machen den therapeutischen Spielraum vielfältig und komplex, insbesondere bevor der Erreger spezifiziert und die Empfindlichkeit gegenüber Antibiotika getestet wurde. Neue therapeutische Ansätze zur Verringerung der Virulenz sind in Forschung. Dieser DFP-Artikel befasst sich mit den klinischen, diagnostischen und therapeutischen Ansätzen zur Erkennung und Behandlung der bakteriellen Keratitis.
Bacterial infections are by far the most frequent cause of infectious keratitis in high-income countries. The clinical appearance can vary widely depending on the type and species of bacteria, ranging from small superficial infiltrates to necrotizing forms. The numerous classes of available antibiotics render the treatment scope diverse and complex, especially before the pathogen has been specified and the sensitivity to antibiotics has been tested. New therapeutic approaches to reduce bacterial virulence are in development. This CME article focuses on the clinical, diagnostic and therapeutic approaches to recognize and treat bacterial keratitis.
Inflammatory changes in the retinal vessels can be attributed to a wide range of etiologies. These include infections, intraocular and systemic autoimmune processes, general diseases and iatrogenic factors. As the endothelium of the retinal capillaries forms the inner blood-retinal barrier, a disruption of this structure is directly associated with consequences for the fluid electrolyte balance of the retina. Clinical sequelae can include leakage of the retinal vessels and macular edema, which are often functionally threatening and significantly reduce the quality of life of patients. As the eye can be affected as an "index organ", a work-up of the patient by the ophthalmologist is of great importance. In the age of "precision medicine", efforts are being made to gain new insights into the pathogenetic mechanisms of vasculitis through "omics" in order to develop innovative treatment concepts.
Entzündliche Veränderungen der Netzhautgefäße können auf ein breites Spektrum von Ursachen zurückgeführt werden. Dazu zählen Infektionen, intraokulare und systemische Autoimmunprozesse, Allgemeinerkrankungen sowie iatrogene Faktoren. Da das Endothel der Retinakapillaren die innere Blut-Retina-Schranke bildet, ist eine Störung dieser Struktur unmittelbar mit Konsequenzen für den Flüssigkeitselektrolythaushalt der Netzhaut verbunden. Als klinische Folgen können sich Leckagen der Netzhautgefäße und Makulaödem einstellen, die oft funktionell bedrohlich sind und die Lebensqualität der Patienten bedeutend einschränken. Da das Auge als „Indexorgan“ betroffen sein kann, kommt der Abklärung des Patienten durch den Ophthalmologen eine wichtige Bedeutung zu. Im Zeitalter der „Präzisionsmedizin“ bestehen Anstrengungen, neue Erkenntnisse über die pathogenetischen Mechanismen der Vaskulitis durch „‑Omics“ zu erlangen, um innovative Behandlungskonzepte zu entwickeln.
Purpose PURALID((R)) Lipogel (Santen Pharmaceutical Co., Ltd., Osaka, Japan) is a new product for treating acute eyelid diseases such as blepharitis. Despite its now frequent use in clinical practice, no controlled clinical study of efficacy of the treatment has been published to the author's knowledge. The aim of this study was to present whether there is a better symptom relieving effect with PURALID((R)) Lipogel with eyelid hygiene compared to eyelid hygiene alone. Methods This article presents a non-blinded, single center intervention study comparing eyelid hygiene with and without PURALID((R)) Lipogel for the treatment of meibomian gland dysfunction blepharitis. A total of 30 patients were recruited during daily office hours. Primary endpoints were NIK-BUT (non-invasive break-up-time) and the ocular surface disease (OSD) index. Secondary endpoints were meibomian gland dysfunction and LIPKO (Lid-Parallel Conjunctival Folds) folds. Of the patients 15 (12 women) underwent eyelid hygiene in combination with PURALID((R)) Lipogel, while 15 patients (13 women) underwent eyelid hygiene alone. All patients also applied Cationorm((R)) eye drops (Santen Pharmaceutical Co., Ltd.) three times daily. The duration of treatment was 2 months and patients were examined before the start of treatment and 2 months afterwards. Results Before the two treatment methods there were no differences in the named parameters, gender and age were also normally distributed. After treatment both subjective and objective criteria improved in both groups but the additional application of PURALID((R)) Lipogel did not provide any benefit. Safety parameters such as visual acuity and intraocular pressure (IOP) remained unremarkable in all patients. Conclusion The use of PURALID((R)) Lipogel in addition to tear substitutes and eyelid margin hygiene does not improve symptomatic blepharitis.
