Background: The main objective of the current study was to assess the level of medication adherence, quality of life, risk factors for better treatment outcomes and to improve patient quality of life by giving TB education. Methods: A prospective observational study was conducted on 278 patients (139 tubercular patients and 139 health volunteers). A data collection form of various socio-demographic factors, lifestyle factors, and co-morbid conditions for tuberculosis were collected. The quality of life (QOL) was assessed by interviewing the subjects using WHO-QOL BREF questionnaire. Morisky – 8 item medication Adherence Questionnaire was used for assessing adherence. Results: In a total of 139 TB patients, high adherence (60.06%), medium adherence (20.86%) and low adherence (10.07%) are reported. WHOQOL-BREF mean domain scores were physical health (54.10±12.33), psychological health (51.73±16.24), social health (62.04±15.35) and environmental health (57.14±16.90) respectively. In present study male (67.63%) are higher than females (32.37%). Risk factors observed in study were statistically significant and discussed in present study. Conclusions: This study showed that the MMAS-8 had good reliability and validity for measuring adherence levels in rural TB patients. There was a high level of adherence to anti-TB treatment was seen in study area. WHO-QOL BREF questionnaire had good reliability and validity for measuring quality of life and Improved quality of life observed in patients with high adherence to anti tubercular drugs. Risk factors such as age, education, locality, food habits, income, smoking and alcoholism are independently associated with Tuberculosis.

In this report, we present the first case of post-stapedectomy reparative granuloma following the use of Biodesign (Cook Medical, Bloomington, IN) otologic graft material, an acellular matrix derived from porcine small intestinal submucosa, and review the literature of post-stapes surgery reparative granuloma.PATIENT:50-year-old woman who developed a reparative granuloma following stapedotomy with acellular porcine intestinal submucosa presenting with profound hearing loss and vertigo.INTERVENTION:Middle ear exploration with excision of granuloma and revision stapedotomy.MAIN OUTCOME MEASURES:Audiologic outcomes as measured by pure-tone air and bone conduction thresholds and word recognition scores. Improvement in vertigo.MAIN FINDINGS:Surgical excision of the reparative granuloma with revision stapedotomy resolved vertigo. Hearing has improved progressively postoperatively.CONCLUSIONS:We report the first case of post-stapedotomy reparative granuloma following the use of acellular porcine intestinal submucosa. Although exact etiology cannot be determined from a single case report, this illustrates the need for careful use of novel foreign graft material. This case additionally confirms that removal of granuloma and inciting materials can salvage serviceable hearing.
We report a spinal metastatic patient with an atypical presentation of inguinal pain. He was managed with L1 laminectomy and posterior instrumentation. After the surgery, his pain was significantly relieved. This study underscores the importance of spinal examination in those with a known tumor.

Magnetic resonance imaging (MRI) thirty years post injury revealed encephalomalacic changes in left frontal, temporal, and parietal lobes with mild left ventricular dilatation. Soon after injury, he developed a compulsion to steal, originally only food but eventually extending to other unneeded items, even though he did not lack access to financial resources and support. Ten years after the injury, the victim was arrested for shoplifting and kleptomania for food or other items (sometimes in duplicate) and had developed Pseudobulbar affect (PBA) in the form of mildly excessive/inappropriate laughter. After his psychiatrist died, he came under neurological care thirty-nine years post injury, and was treated with DM/Q (Nuedexta® 20mg/10mg), reaching a maintenance dosage of one capsule twice a day at week two. Reduced impulsivity and inappropriate laughter were seen by two weeks with complete remission after four weeks.
