
: Here we present a unique life path of Sergiy Gulyar, a world-known Ukrainian scientist who was overcoming extreme conditions studying them on himself. He has developed his determination and responsibility from his basic experiences as a surgeon and his desire to win from his involvement in sports. His research in underwater laboratories has shown a capacity to find untrodden pathways to understand and explain what others did not even suspect. His physiological studies on the role of sea depths, breathing mixtures, underwater exposures, and diving schemes marked the path to the aquanauts and undersea man. Studies of hyper depths equivalent to 2,500 m revealed the physiological limits of human being and demonstrated how to handle respiratory failure. Prof. S. Gulyar suggested a usage of natural physiological mechanisms to accelerate the re-adaptation process as a part of the high-mountain rehabilitation of hyperbaria-adapted people. Dissertations were defended, books and articles were written, a scientific school of followers was created. Unfortunately, during the Soviet period, Prof. S. Gulyar faced a set of organizational problems and obstacles from the Soviet regime. In particular, his works were classified or hushed up, his intellectual property was often used without mentioning the author and his scientific team was intimidated by criminal investigations. Main recognition during this period had come from professional societies in Europe and the United States. After the collapse of the Union of Soviet Socialist Republics (USSR), Prof. S. Gulyar introduced a number of innovations and inventions in electromagnetic medicine and physiology. In particular, he has managed to account for main common features of physiological effects of light stimulation produced by Lasers, Light-Emitted Diodes and Bioptron light sources. By doing so, some mystical dogmas were filtered out and new paths to sensible light-induced treatments were developed. At the same time, Prof. S. Gulyar has preserved the legacy of many generations of medical professionals who used light in their treatments. He has shown in his physiological experiments that stimulation of biologically active zones including acupuncture points light stimulation has a wide spectrum of biological effects including alleviation of pain symptoms. Now mono- and polychromatic visual and transcutaneous light therapy of pain has been recognized scientifically and clinically, and its place in medicine has been firmly established. Prof. S. Gulyar described a new functional system of the organism that regulates the electromagnetic equilibrium. A step into the future was the first experimentally grounded technology for the use of fullerene-modified light. Positive changes have been proven with its percutaneous and ocular use. These first results have yet to be fully analyzed, so a full investigation will be needed. Prof. S. Gulyar has published 20 monographs, 470 papers and abstracts, and received 11 patents. Many of his inventions have been implemented, the others are still awaiting implementation. This essay is based on the data obtained by the authors during many years of their personal cooperation starting from the Laboratory Ichthyander to the present day at the Bogomoletz Institute of Physiology of the National Academy of Sciences of Ukraine, as well as from the memoirs of Prof. S. Gulyar and the materials he provided.
: NDM (neonatal diabetes mellitus) is a rare genetic disease, which happens in the 6 months of life. Some patients with NDM heal on their own, but the other patients will show the symptoms of NDM for their whole life. Based on the presence of the remission phase, NDM is divided into two categories: TNDM (transient neonatal diabetes mellitus) and PNDM (permanent neonatal diabetes mellitus). As a genetic disease, the mutations in different genes lead to different symptoms and the presence of the remission phase. The mutation in INS gene will cause PNDM, and the mutations on KCNJ11 and ABCC8 genes and the abnormalities of chromosome 6q24 will lead to TNDM. Previous articles have always given information about diseases rather scatteredly: they only studied only mutated gene in an article. Unlike them, this article will discuss and analyze these three types of mutation and both PNDM and TNDM in one article. It will give the pathologic mechanism of the diseases, and, for different mutated genes, three treatments are given in this article at the same time. The compilation shows more clearly about the different symptoms corresponding to the different mutated genes and the mechanism of the disease.
: To ensure the radiation safety for public, caregiver and family members from the patients who need high dose radioactive iodine treatment. The patients require isolation in a lead shielded room for certain period of time and released them if the radioactivity remaining in the body less than 1.2 GBq. The aim of this study was to estimate the isolation time and investigate the possibility of earlier release from hospitalization. This study was retrospective analysis of data from 136 patients who required hospitalization to treat thyroid cancer with I-131. The radiation dose rates were measured by using a radiation detector at 1.0 m from the anterior neck of patient immediately after I-131 administration and subsequent at 24, 48, 72 and 96 h respectively. The measured data were plotted using a Microsoft Excel; the effective half-life ( T eff) was derived using a curve fit function of the spreadsheet program assuming. The mean T eff to all patients obtained by excel were 17.33 and 34.65 h for the initial fast and the second slower clearance phase, respectively. The dose rate from patient decreased in a bi-exponential pattern, where there is a fast clearance of radioiodine in the first day post I-131 treatment and a slower clearance after this time. The isolation time depends on the T eff value instead of the administered I-131 activity and this study has demonstrated that after 24 h post I-131 radiation dose rate at 1 m was less than 70 µSv/h. Based on dose rate measurement to release the patients, this suggests that a majority of the patients could be discharged from the isolation room after 24 h after I-131 treatment.
: In experiments on laboratory animals ( n = 100) and in examinations of people ( n = 42), the anti-stress effect of low-intensity polychromatic or monochromatic halogen light of the Bioptron device (PILER/PL: 480-3,400 nm, 40 mW/cm 2 ) and LED (light-emitting diode) light of the Medolight-polychrome device (401+467.5+527.5+640.5+940 nm) has been demonstrated objectively. We found out that adult rodents (white mice) showed that 30 min of immobilization stress increased grooming duration by 330%, and after a PL (polarized light) session to acupuncture point (AP) E-36—only by 170%-230% compared to the norm. PL’s anti-stress effect was determined by its wavelength. Researchers found that red light (the long-wavelength part of the visible light spectrum) had a significantly greater anti-stress effect than green light (medium-wavelength). Red PL application reduced stress-induced grooming by 49.2% and other behavioural responses (sleeping, eating, and physical activity) were partially normalized, while green PL reduced grooming by 31% without affecting other behavioural responses. The short-term immobilization stress weakened the somatic pain response (formalin test) by 28.5% and the visceral one (acetate test) by 26.3%. Red PL has a less pronounced analgesic effect on animals under stress than on animals not under stress. In normal conditions, red PL suppressed somatic pain by 54.4%, visceral pain by 64%, and under stress by 31% and 46.1%, respectively. Under the action of low-intensity LED-light on AP, we have obtained experimental evidence of stress reduction in humans. The latent period for falling asleep in the subjects increased from 393.6 ± 47.1 to 749.3 ± 44.4 s under stress. Applying the Medolight-polychrome device to auricular AP weakened post-stress sleep disturbances: the duration of falling asleep was reduced to 512.5 ± 38.6 s. In persons with daytime stress, the frequency of dreams was 49% ± 5.7%, and after a PL session it was 14.79% ± 5.2%. The results of these studies can be used to develop recommendations for reducing stress in humans.

In this paper, I will present the main concepts that address the question of depend the theoretical framework of constructivism. The ways to work within present the ways how we learn from our efforts to help clients, including successes and failures.
Umbilical endometriosis accounts for 0.5%-1% of all extrapelvic endometriosis. It is usually secondary to surgery involving the umbilicus; primary nodules “Villar’s nodule” are very rare. We report a case report of endometriosis graft on umbilical scar secondary to open surgery. A 41-year-old healthy parous patient with past history: cesarean of the last child birth 6 years ago. § three years after, she was operated for linea alba hernia. One year after, she presented with a swelling painful umbilicus nodule, recently, which gradually has been evolving and associated with cyclical umbilical pain. Clinical examination at admission: umbilicus nodule 12mm, painful to the touch, irreducible, which could not be reduced by gentle digital pressure. Ultrasound and CT (computerized tomography) scan: parietal tissue mass of 12mm/8mm without intraperitoneal continuity. The surgical treatment consisted of wide local nodule excision. The histopathological examination confirmed the diagnosis. The patient was referred for further investigation to gynecology department where she underwent a hysterectomy for uterine adenomyosis.
