
Aim: This study investigated the effect of preparation design and thermal cycling on the marginal accuracy of hybrid ceramic laminate veneers.Materials and Methods: A total of twenty 3D printed dies of central incisors prepared with 2 preparation designs; butt joint and palatal wrap design were fabricated.Each type of preparation was assigned into 2 subgroups (n=5/ each subgroup) according to the type of material used for fabrication of the laminate veneer; hybrid ceramic and lithium disilicate (served as control).Marginal discrepancy was evaluated using a digital microscope before and after thermocycling.Results: Statistically analysis of marginal gaps revealed that before thermocycling, butt joint design demonstrated larger average marginal gaps compared to palatal wrap design in both Enamic (76.18±0.07)and E-max (76.11±0.09µm).While smaller gaps was recorded in palatal wrap design E-max (63.27±0.28µm) and Enamic (63.21±0.05).After thermocycling, palatal wrap design showed larger average marginal gaps in E-max (166.75 ±16.67 µm) and Enamic (157.52±15.44);while a significantly lower gap was recorded in butt joint design for Enamic (137.41±0.2) and E-max (137.41±0.09µm).There was no statistical significant difference between marginal gaps in Emax and Enamic within the same preparation design.Conclusion: Before thermocycling, the palatal wrap design laminate veneers showed smaller marginal gaps compared to butt joint.Thermalcycling caused a detrimental increase in marginal gaps .The effect of thermocycling was more profound on palatal wrap design veneers.The marginal accuracy of hybrid ceramic laminate veneers were statistically comparable to that of lithium disilicate.
OrthodonticsAim: Clear aligner treatment's main drawback is that the patient decides how long to wear them each day.For clear aligner therapy to be successful, compliance is essential.A poor treatment outcome would arise from noncompliant patients.Methods: The study was a randomized clinical trial of 2 parallel groups (7 patients per group) who underwent clear aligner orthodontic treatment having an age range of 17-24 years.A part-time group wore the aligners for 16 hours per day and a fulltime group wore the aligners for 22 hours per day.The study investigated the first eight aligners of the treatment.Compliance was recorded for each patient.Pain was evaluated by visual analogue scale.Periodontal health was examined by detection of bleeding upon probing.Results: Bleeding upon probing was detected in 21 percent of the patients (2 in full-time group and 1 part-time group).The intensity of pain peaked at 24 hours (mean VAS score 4.57±1.13 in part-time group and 4.86±0.9 in full time group).Part-time group had a mean wear time of 14.97±0.37hours while full-time group had a mean wear time of 19.54±0.75hours.Conclusion: There were no significant differences between part time and full time groups regarding periodontal parameters considered and pain intensity.Instructions of wearing the aligner for a longer duration resulted in increased overall adherence to wearing the aligner.
OrthodonticsAim: The aim of this clinical trial is to investigate maxillary first molar rotation during maxillary canine retraction at 6 weeks re-activation intervals with elastic power chain in first premolar extraction cases.Material and methods: a total of 20 quadrants of 10 orthodontic patients requiring maxillary canine retraction after extraction of upper 1 st premolars were recruited.Mini-screws (TADs) were used directly for maximum anchorage.Canines were retracted on 0.016×0.022-inchstainless steel arch wire using elastic power chain extending from a power arm on canine bracket to the miniscrew.Cone beam computed tomography (CBCT) was used to measure the upper first molar rotation.Results: The mean difference of the first molar rotation between pre-retraction and post-retraction measurements was 0.6250 o ± 0.6 o which was statistically insignificant.Conclusion: There is no significant difference between pre-retraction and post-retraction upper first molar rotation re-activated every 6 weeks using power chain.
Group I and 19.25 SS in Group II.Number of visits for patients in each group were recorded till reaching full alignment. Results:The number of visits in Group I ranged between 6 to 8 with an average of 7±0.58 visits.However, the number of visits in Group II was between 7 and 9 with an average of 8±0.58 visits.There was a statistically significant difference regarding alignment duration between the two treatment groups.Limitations: It was not possible to blind the clinician and only mild and moderate cases were included in the study with no severe or extraction cases.Conclusions: There was statistically significant difference in alignment duration between 0.018-inch and 0.022-inch slot bracket systems.The mean difference between both groups is one less visit in group I (0.018") which could be not significant clinically.

Aim:To evaluate the efficacy of umbrella technique combined with sticky bone in horizontal ridge augmentation using Buccolingual width changes measured in CBCT.Materials and Methods: 15 male and female Patients were selected .Buccal and lingual Infiltration anaethesia to mandibular posterior area (area of interest), crestal incision flap.Decortication using round bur and straight hand piece then drilling using screw drills with 90 degrees to the buccal cotical plate.Umbrella Screw insertion using screw holder with 6 mm length and 1.6 mm diameter where 3 mm inserted in the bone and 3 mm are kept outside.The bone graft and the I PRF are placed into sterile dish, mixed together till sticky bone is formed.Application of the sticky bone buccally, on and in between the bone screws with proper condensation.Results: At baseline, the mean radiographic bone width value was (3.31), the standard deviation was (0.87), the median was (3.75), and the interquartile range was (1.01).After 6 months, the mean was (6.08), the standard deviation was (0.78), the median was (6.33) and the interquartile range was (0.99).For the radiographic difference after 6 months, the mean value was (2.62), the standard deviation was (0.15), the median was (2.66) and the interquartile range was (0.20).For the percentage change the mean value was (76.30), the standard deviation was (11.25), the median was (72.94) and the interquartile range was (10.60). Conclusion:Umbrella technique using bone screws has predictable results in the horizontal ridge augmentation.Tenting of the periosteum and soft tissue using tenting screws maintains space and minimizes resorption of the particulate graft volume.
Aim: This study aimed to evaluate the efficiency of passive ultrasonic activated irrigation in the removal of GuttaFlow Bioseal root canal filling material during retreatment.Materials and subjects: Root canals of thirty single-rooted human mandibular premolar teeth were prepared with ProTaper Universal rotary system and obturated with lateral condensation obturation technique using Gutta Percha and Roeko GuttaFlow Bioseal root canal sealer.All specimens were retreated with ProTaper Universal Retreatment System files then divided to two different groups according to the technique of activation of irrigation.Samples were sectioned, and the residual filling remnants were captured using digital camera attached to microscope.Data was collected by three different interpreters, to eliminate the subjectivity of the process, using the ImageJ Software.The mean value of the data was obtained and evaluated statistically.Results: The remaining filling materials in the canals irrigated with ultrasonic activation were less than these irrigated with passive side-vented syringe.Conclusion: The activation of irrigation techniques used were incapable of complete removal of filling material at root canal walls.
Aim: This study was designated to evaluate the effect of using intracanal cryotherapy versus ultrasonic activated irrigation on postoperative pain in single visit endodontic treatment of mandibular first molar with symptomatic, irreversible pulpitis using numerical rate scale (NRS) at 6, 12, 24, 48,72 hr and 1 week postoperatively.Materials and Methods: Seventy-eight patients diagnosed with symptomatic irreversible pulpitis in mandibular first molar with no periapical involvement.The cases were randomly divided into three groups according to final irrigation protocol after root canal preparation using Pro Taper Next; Groups A, B and C, twenty-six patients each group (n=26).Group A: final irrigation with room temperature saline using Navi-Tip; double side-port needle (31-gauge,27mm).Group B: final irrigation with room temperature saline with ultrasonic activation using (Ultra-X).Group C: final irrigation with 2.5ºc cold saline using Navi-Tip; double side-port needle (31-gauge,27mm).After a single visit root canal treatment and a specific final irrigation protocol, depending on each group, the patients were given a questionnaire on which the patient would mark the degree of pain in a scale from 0 to 10 at 6, 12, 24, 48, and 72 h and 7 days post-obturation.Data were statistically analyzed with a significance level of p≤0.05.Results: Ultrasonic group (Group B) showed statistically the least incidence and intensity of postoperative pain compared to the other groups after 6, 12 and 24 hours.Cryotherapy group (group B) showed lower Incidence and intensity of postoperative pain after 6,12, 24 and 48 hours.While control group (group A) showed statistically the highest incidence and intensity of post-operative pain compared to the other groups after 6,12, 24,48 and 72 hours.After 1 week there were no significant difference among the three groups where pain scores declined among the groups till it reaches score (0) at day 7. Incidence and number of analgesic intake was significantly higher in patients of the control group compared to ultrasonic and cryotherapy groups.Conclusion: The least post-operative pain incidence and intensity were associated with ultrasonic irrigation on the first day.The incidence and severity of post-endodontic pain are decreased when cold saline (cryotherapy) is used as the final irrigant during endodontic treatment of teeth with symptomatic irreversible pulpitis.
Orthodontics Aim: To assess the accuracy of Dolphin Imaging software in predicting upper jaw profile changes in class II patients treated by extraction and non-extraction.Methods: The records of 60 Class II div 1 patients were selected; 30 patients treated by extraction of upper first premolar and 30 patients treated by non-extraction.The Pre-and post-treatment lateral cephalometric x-rays of each patient were traced using Dolphin software.The actual hard tissue changes due to treatment were calculated and installed into the prediction module to generate the predicted profile.The soft tissue measurements of the actual posttreatment and predicted profiles were compared using the Dolphin measurement function.The accuracy of the softwaregenerated prediction was analysed using Dahlberg's formula (RDE), Bland & Altman methodand Lin's Concordance Coefficient (CCC).Results: RDE of the measured variables were significantly lower for non-extraction group (P = 0.05).Regarding the extraction cases, prediction of the upper lip position relative to E line was not reliable (RDE = 19.1%).Mean difference (predicted-actual) of the upper lip thickness (+0.8 mm), upper lip length (+0.633 mm), and nasolabial angle (-4.09) showed a high Bland & Altman range.Regarding the extraction group, upper lip position and nasolabial angle showed poor agreement between the actual and predicted measurements (CCC= 0.505 & 0.38).Conclusions: Dolphin software prediction for soft tissue changes is more accurate in non-extraction treatments for class II div 1 cases.The software tends to underestimate the upper lip retractive response due to extraction.
Aim:The concept of oral health related quality of life (OHRQoL) is of great significance in the three areas of dental health, in particular the clinical practice of dentistry, research, and education.The reported prevalence of gingivitis in various developed countries is considered high.This rate escalates with the increase in age, especially around puberty.There is evidence that gingivitis is associated with the children's OHRQoL as there are common risk factors between gingivitis' of adolescence and their consequent OHRQoL.Aim: This study aims to evaluate the correlation between gingival redness as evaluated by digital photographs and oral health related quality of life as measured by the short form of child perception questionnaire 11-14.Subjects and methods: 91 participants 11-14 years-old Egyptian children from New-Giza University outpatients' clinic were recruited and OHRQoL was evaluated using the Arabic version of the regression short form (RSF) of Child perception questionnaire 11-14 (CPQ 11-14).Gingival redness was evaluated using analyzed digital photographs using Adobe Photoshop Creative Cloud (CC) 2021 software.Results: The study showed that there was insignificant (P >0.05), weak (r < 0.5), positive (+) correlation between the number of red pixels in digital photographs and oral symptoms domain and functional limitation domain.Conclusion: there was no statistically significant correlation between the gingival redness and the oral health related quality of life.
OrthodonticsAim: Cephalometric x-ray is considered as a crucial step in orthodontic diagnosis and treatment planning.However, patients nowadays, especially those pursuing cosmetic goals, are concerned about radiation dosages as a result of increased understanding of its hazards.Therefore, the aim of this study is to provide a simple reliable, affordable and reproducible way to evaluate the soft tissue measures for creating an orthodontic treatment plan.Methods: Comparing standardized profile photographs and lateral cephalograms were obtained from twenty-eight patients with maxillary protrusion having an age range between 14 -24 years.Evaluation and comparison were done for cephalometric radiographs by using webceph software as well as their corresponding standardized photographs using Adobe photoshop software, the compared outcomes were Nasolabial angle, Mentolabial angle, Upper lip protrusion/ E-line and Lower lip protrusion/ E-line.Results: Upon comparing soft tissue analysis using cephalometric radiographs and standardized photographs, it was proven that photographs are reliable for soft tissue analysis with insignificant difference between the two techniques.Conclusion: Although cephalograms provide precise measurements, unnecessary radiation exposure can be avoided by using photographic analysis.It is safer, quicker and affordable.It has been demonstrated that the photographic approach is reliable and reproducible.The photographic approach, with a uniform methodology, is a useful substitute for lateral cephalograms, particularly in situations when a non-invasive, affordable solution is required as soft tissue analysis not in skeletal analysis.
Aim:The aim of current study was evaluating effect of concentrated growth factor alone and mixed with hyaluronic acid on osseointegration of delayed implant.Materials and Methods: A total of 12 patients, ranging in age from 20 to 45 years old, were divided into two groups in random manner.Group A: Dental implants were placed associating with concentrated growth factor.Group B: Dental implants were placed associating with concentrated growth factor and hyaluronic acid.Treatment changes were evaluated for each group and compared between groups.Results: In the mean of implant stability a statistically significant difference existed within group A as implant stability increased from base line immediately after implant insertion to follow up period after six months, also statistically significant difference existed in the mean of Implant Stability within group B as implant stability decreased from base line immediately after implant insertion to follow up period after six months.Also, there was statistically significant difference in the mean of Implant Stability between the two groups.Conclusions: Concentrated growth factor alone have positive effect on bone osseointegration.
