In children, fistula-in-ano is a common anorectal disorder. Infected anal glands trigger it in children, but the process is not the same as in adults. The cause in infants is repeated movement of soft stools or diarrheal episodes, which damage or traumatize the walls of the anorectal outlet, and also varies from adult anatomy. Adults do not have the same management and care protocols as children. Anything in the management and care plan for infants varies. Fistula in ano in infants and children has certain characteristics that differentiate it from adults, such as male predominance, manifestation at less than 12 months of age, multiple, but separate tracts in less than 20% of cases, rarity of complex fistulae, and low incidence of recurrence. In this case report, the efficacy of Udumber ksheer sutra (Ficus racimosa Linn) treatment and relief of fistula is well highlighted which helps in further research in this regard.

Background: Ballota nigra (Lamiaceae) has been widely used as traditional medicine in many countries for the treatment of neurosedative, antidepressant, antioxidant, antibacterial, insecticidal, anticholinesterase and antifeedant effects. Aim: The present study examined the effects of Ballota nigra extract in diethylhexyl phthalate (DEHP) exposed Chang liver cells in vitro. Methods: Chang liver cells (5 x 105 cells) were exposed to 400 µM of DEHP for 24h. At the end of incubation, cells were treated with different doses of (50, 100 and 200 µg/ml) B. nigra aqueous extract and kept for 48 h and used for the assessment of insulin receptor (IR), insulin receptor substrate-2 (IRS-2) mRNA and glucose uptake and oxidation. Results: IR and IRS-2 mRNA, glucose uptake and oxidation in DEHP exposed Chang liver cells were significantly reduced (p<0.001). Cells treated with B. nigra extract could effectively enhance IR and IRS-2 mRNA and oxidation and uptake of glucose in a dose-dependent manner (p<0.001). Conclusion: The present findings are the first to report in identifying the antidiabetic activity of B.nigra against DEHP exposed detrimental changes insulin signaling molecules and glucose oxidation. Hence, B. nigra may have influence in controlling diabetes and hence it could be used a therapeutic candidate for the treatment of diabetes mellitus. Further studies on the possible role of B. nigra on the protein expression of downstream insulin signaling molecules in liver cells are warranted in order to strengthen the potential of B.nigra.
Background: Azadirachta indica or Neem of Meliaceae family is a rich source of antioxidant. Azadirachta indica is regarded as a health-promoting medicine and used in various forms globally. It is lengthily used in Indian Subcontinent province for the treatment and prevention of an assortment of ailments. Objective: The objective of this study is to observe the anxiolytic action of methanol extract of Azadirachta indica. Materials and methods: Methanolic extract of Azadirachta indica (MEAI) was administered in the doses of 100, 300 and 600 mg/kg p.o. For determination of anxiolytic activity, hole board (HB), open field (OF), elevated plus maze (EPM) and light/dark exploration (LDE) tests were used. Results: The period and quantity of the poking of the head in HB test was amplified by the methanolic extract of Azadirachta indica in a substantial manner. The total exhausted duration was augmented by the extract in EPM. The extract created a remarkable boost in the total exhausted timing in LDE test. Also, it augmented the number of crossings and lessend the total period of the indolence in light box. Diabetic nephropathy (DN) is extremely severe and most general complication of diabetes which could be foremost reason of unceasing kidney failure. Aim: The aim of the present study was evaluated the reno-protective effect of lupeol in high fat diet and sucrose-fed diabetic nephropathy in Wistar rats. Methods: Lupeol was administered to the diabetic rats at a dose of (25 mg/kg body weight) was given once a day until the end of the study (60 days post induction of diabetes) to high fat diet-induced diabetic rats. Real Time-PCR analysis was performed in order to study reno-inflammatory signaling (IL-6, IL-1β and TNF-α). Results: Lupeol treatment significantly reduced hyperglycemia and significantly lowered oxidative stress markers in the renal tissue of treated diabetic groups. Moreover, lupeol treated animals exerted decrease in mRNA levels and low serum concentrations of IL-6, IL-1β and TNF-α compared to diabetic control rats. It showed a significant decrease in NF-kB activation in the diabetic kidney. Conclusion: Lupeol treatment mitigates oxidative stress and suppresses renal inflammation via inhibition of NF-kB in diabetic kidney in early progressive diabetic nephropathy. Therefore, the triterpenoid could be an effect treatment option in early progression of diabetic nephropathy in clinical trial.
HIF-1α (hypoxia inducible factor 1 alpha) is a protein that controls cell growth and differentiation and has been linked to cancer in humans. In cancer cells, HIF-1 triggers the carcinogenesis cascade function. The HIF-1 receptor is thought to be the initiator of signaling and its `over expression has been linked to a variety of human cancers, including breast cancer, lung cancer, and colon cancer. As a result, HIF-1 becomes a possible therapeutic target in the development of HIF-1 inhibitors. The aim of this study is to look into natural potential inhibitors. To accomplish this, the chemical structures of all substances were downloaded from the PubChem database. PyRx was used to conduct the docking and Pymol was used to evaluate the resulting binding modes. Berberine, a natural compound demonstrated the strongest binding modes as compared to other inhibitors, with the lowest binding energies (-8.6 kcal/mol). This study indicates that Berberine interacts favorably with the necessary amino acid residues at HIF-1's catalytic site. Present findings of the molecular docking analysis may be useful as a compound model for further in vitro and in vivo research.
In recent years, there has been a greater focus on the possible toxicity of several herbal medicines. Many different forms of poly herbal formulations are utilized in ancient systems of medicine such as Ayurveda, Siddha, and Homoeopathy. Many herbal formulations have been shown in animal models and clinical trials to be an effective agent for treating coronary heart disease, peptic ulcers, and skin problems etc., however, nothing is known regarding PHF's potential chronic toxicity. The purpose of this study was to explore at the safety of poly herbal formulation (PHF) in laboratory rats. Wistar rats were separated into four groups of 50 rats each for chronic toxicity (20 males and 20 female). The experimental animals were administered PHF at doses of 250,500, and 1000mg/kg for 180 days, satellite group animals also treated with 1000 mg/kg. while the control animals were given distilled water. The animal's body weight, food and water intake were all tracked on a weekly basis. In addition, at certain observation time points, their biochemical and haematological parameters, histology, and body and organ weights were also measured. PHF did not induce noticeable anomalies in physiological parameters or pathological changes in the major organs of rats, according to the results of the chronic toxicity study. The results showed that PHF at the specified levels had no adverse effects on the animals. As a result, it is acceptable to conclude that PHF is safe at the dosage levels studied and does not produce chronic toxicity in animal models.

Oral health is important for the overall wellbeing of an individual. It is an important element of general health throughout life(1). It is a basic human right attributed to people to having essential oral health care, therefore ensuring health and quality of life(2).The most routinely caused diseases of the dental cavity are dental caries ,periodontal problems and malocclusions in children. Unfortunately, oral care remains more focused on treatment than on prevention. Oral health is an essential component of health throughout life, due to the association of oral cavity with the development of a healthy personality, perceptions, and the overall experiences of pleasure.(3) However, millions of individuals suffer from dental caries and periodontal disease, resulting in unnecessary pain, difficulties in chewing, swallowing and speaking and sometimes unaffordable high medical costs. Untreated oral diseases among children frequently lead to serious general health problems, significant pain, interference with eating and school absence.(4) Thus the aim of the study is to evaluate oral health status of 6 year old school going children in Chennai.

At the end of incubation, cells were treated with different doses of (50, 100 and 200 µg/ml) cuminaldehyde for 48 h. After 48h, the cells were used for the assessment of insulin signalling molecules (IR, IRS-2, and Akt) and glucose oxidation in Chang liver cells. Results: IR, IRS-2 and Akt mRNA and glucose oxidation in DEHP-exposed Chang liver cells were significantly reduced (p<0.05). Treatment with cuminaldehyde enhanced IR, IRS-2 and Akt mRNA and oxidation of glucose significantly (p<0.05). Conclusion: The present findings are the first to report to identify the antidiabetic role of cuminaldehyde against DEHP exposed detrimental changes insulin signalling molecules and glucose oxidation in liver cells. Hence, cuminaldehyde may have significant role in controlling diabetes and hence it could be used a therapeutic natural drug for the treatment of diabetes mellitus.
Calcaneum is the largest tarsal bone which forms the prominence of heel. A plantar calcaneal spur is an outgrowth of the calcaneus. Chronic inflammation at the insertion of tendons or plantar fascia is a common cause of spurs. Few studies showed a correlation between plantar fasciitisandcalcaneal spurs. Present study also explains that correlation is profound in subjects with risk factor such as advanced age, weight and developed Osteoarthritis. About 40 patients of known age and sex with history of heel pain were studied. Inform consent had taken prior to study. All 50 patients had completed X-ray lateral view of affected foot. Radiological study shows that about 30 patients were having calcaneal spur. Thus this study helps in not only understand the correlation of plantar fasciitis with calcaneal spur but it also helps the surgeon or orthopaedician to draft a line of treatment depending on the severity of symptoms that is conservative or surgical intervention. CONCLUSION: About 80% of patient of Painful heel having plantar fasciitis with PCS and is associated with high proportion in elderly, overweight, female patients. It is known that majority are found deep to plantar fascia, surrounded by fibrocartilage. Histologically PCS has a trabecullar pattern and is in association with the factor listed above that determines the heel pressure. Potential causes of tendencies at heel, includes tear in plantar fascia, fracture of calcaneum or fat pad atrophy or any other neural cause appear prominently in of PCS.
Informed consent for clinical treatment has become a vital part of contemporary medical practice; it means different things in different contexts, is variably practiced and rarely achieves the theoretical ideal. Informed consent has become the primary paradigm for protecting the legal rights of patients and guiding the ethical practice of medicine. It may be used for different purposes in different contexts: legal, ethical or administrative. Study title: “Knowledge and opinion regarding written consent in hospital practice among patients and family members.” Materials and Methods: The study adopted Non-Experimental Descriptive design carried out among 200 subjects (100 patients and 100 family members). Non-probability purposive sampling technique was used and was statically analysed after collecting the data through A structured questionnaire for assessing the knowledge and opinion regarding written consent. Ethical clearance was taken from the Institutional ethics committee. Data analysis was done using descriptive statistics. Results: The study results showed that majority 61% of patients have average knowledge, 23% samples have good knowledge and 16% samples have poor knowledge with the mean of 9.36 and SD of 3.39. The majority of samples 66% in group of family members have average knowledge, 25% samples have good knowledge and 11% samples have poor knowledge with the mean of 9.95 and SD of 3.20. Conclusion: The findings of the study showed demographic variables like age and marital status of the patients are associated with the knowledge regarding written consent while only marital status of the family members is associated with the knowledge regarding written consent. Opinion wise distribution shows that family members and patients are having similar opinion regarding written consent in hospital practice. Recommendation: It is suggested that a similar study can be indicated with a larger population.

The functions of vascular endothelial cells are vasomotion, blood coagulation, and inflammation. Numerous mechanoreceptors are present on the surface of the endothelium. Mechanotransduction occurs across the endothelial cells as a result of the shear stress stimulation which will alter the structural, functional, and genetical modification of the endothelial cells. Blockage of blood flow is the root cause for most of cardiovascular diseases. Poor physical activity and unhealthy lifestyle habits favoring the deposition of fats plague formation in the vessel walls. Endothelial dysfunction is a condition where endothelial cells are not able to release the substance which maintains vascular tone. This is an overall review that summarizes the endothelial cells' pathophysiological aspects along with the methods to detect endothelial dysfunction.
