
Cow’s milk is one of the main causes of food allergy in young and pre-school children. Symptoms of cow’s milk protein allergy can differ, depending on affected target organ. Thus, compiling anamnesis, it is necessary to conduct complex assessment of complaints and a child’s general condition. Active investigation is being conducted to improve diagnostics of cow’s milk protein allergy in children, particularly with involvement of modern laboratory capabilities. The aim of the work was to optimize diagnostics of cow’s milk protein allergy considering results of immunological investigations in children over three years of age. Materials and methods. Establishment of diagnosis included record of allergological anamnesis, elimination diet, conduction of elimination and provocation food test, determination of specific IgE, conduction of skin allergy testing and oral provocation test in adequate conditions. Optimal tactics of treatment is determined based on the results of conducted investigations. Results. A new criterion for diagnosis of cow’s milk protein allergy in children is consideration of the results of immunological investigations and skin allergy testing to establish a risk degree of allergic reaction development and determination of conditions for conduction of diagnostics of oral provocation test: specific IgE over 0.35 kU/L and less than 2 kU/L and/or papule 3-7 mm – low risk degree, allergy center; specific IgE over 2 kU/L and less than 15 kU/L and/or papule 8-11 mm – average risk degree, specialized inpatient department; specific IgE over 15 kU/L and/or papule over 12 mm – high risk degree, intensive care
The level of S100 and phospholipase A2 were analyzed in the blood serum of 34 newborns with pathology of CNS. Depending on the nature of CNS lesions, two groups of newborns were identified: the study group 14 premature newborns with bacterial meningitis (mean gestation age is 31,7±0,9 weeks), and the comparison group 10 premature newborns (mean gestation age is 32,3±0,4 weeks) and 10 fullterm newborns (gestational age is 38,7 ± 0,5 weeks) with severe hypoxic-ischemic injury of CNS. The study of children in the study group was conducted twice: in the acute period of the disease and in the period of convalescence on reaching their postconceptual age of 38-40 weeks. The control group consisted of 18 full-term newborns with a gestational age of 38,6±0,3 weeks without signs of CNS injury and inflammatory diseases. It was found that content of S100 protein in the blood serum of premature newborns has a diagnostic value in relation to assessing the risk of meningitis. With its value equal to 104,1 ng/l or more, meningitis is diagnosed with a sensitivity of 85,7% and a specificity of 75,0%. 2 Gennadiy Lezhenko et al. The determination of this protein in blood serum can also be used as an additional biochemical marker in premature newborns with meningitis to monitor the effectiveness of therapeutic measures. At the same time, increased serum phospholipase A2 activity is not a specific diagnostic marker of meningitis in preterm newborns, but reflects the processes of CNS damage regardless of the etiology of the disease.
The aim of the study was to detect the effect of RAAS genes polymorphism for recurrence of paroxysmal atrial fibrillation among patients with coronary heart disease combined hypertension.
Parkinson’s disease (PD) is a widespread, progressive neurodegenerative disorder with complex pathogenesis. The combination of its motor and non-motor symptoms significantly impairs a patient’s life quality. The question of targeted patogenetic therapy of Parkinson’s disease is still relevant, which is why biomarkers for early diagnosis and neuroprotective therapy are urgently required. Objectives: to investigate possible differences between αsynuclein (ASYN) and melatonin levels in plasma of PD patients in I and II Hoehn and Yahr stages and healthy controls and to research possible correlations between (ASYN) and melatonin levels and non-motor symptoms (cognitive impairment and sleep disorders). Materials and methods. We recruited 67 patients at I-II Hoehn and Yahr (H&Y) PD stages (25 patients at I H&Y PD stage and 42 patients at II H&Y PD stage, respectively) and 20 healthy controls. The Montreal Cognitive Assessment test (MoCA) was used to assess several cognitive domains in PD patients and healthy controls. Patients with MoCA scores of ˂26 were defined as PD-with mild cognitive impairment (PD-MCI). We used Night Sleep Assessment 52 Varvara Biriuk, Alina Demchenko and Svitlana Gorbachova Questionnaire (NSAQ) for sleep disorders evaluation and defined patients with NSAQ scores of 23-30 as PD-without sleep disorders, 18-22 as PD-with mild sleep disorders and 6-17 as PD-with severe sleep disorders (Vein A.M., 1998). Plasma for all participants was collected at a fixed time interval between 7:00 AM – 9:00 AM using a 10-ml K2-EDTA tubes (BD Vacutainer). Samples were centrifuged for 15 min at 1000×g at 2-8°C within 30 min of collection. Then 0.5 ml of supernatant plasma was removed from each tube and transferred into a 1.5 ml Eppendorf tubes. All the plasma samples were frozen at –80°C. Enzyme linked immunosorbent assay (ELISA) was performed afterwards. Results. The level of plasma ASYN in PD patients was significantly higher than in healthy controls (p = 0.001) and it did not depend on PD H&Y stage (Table 1). But the level of melatonin did not differ significantly between PD patients and healthy controls (p = 0.139). The area under the ROC curve (AUC) of plasma ASYN (Figure 3) level to distinguish PD patients from healthy controls was 0.736 (95% CI = 0.630 – 0.825; cut-off value = 70.388 pg/mL). Plasma ASYN level was significantly higher in PD-MCI patients than in PD patients with normal cognition (p = 0.002) (Table 2), and plasma melatonin level was significantly higher in PD patients with severe sleep disturbances than in PD patients with mild sleep disturbances or without sleep disturbances at all (p = 0.002). Conclusions. Our findings suggest that plasma ASYN level correlates with the disease stage progression and cognitive impairments in PD patients at I-II H&Y stages. Plasma melatonin level was significantly higher in PD patients with severe sleep disturbances only, but not in PD patients with mild sleep disturbances. Plasma ASYN level can differentiate between PD patients and healthy controls.
Determination of content of vitamin D and antimicrobic peptides (cathelicidin, β1defensins, lactoferrin and hBPI) in blood serum of 67 children with acute bacterial diseases of a respiratory system aged from 1 month up to 3 years taking into account the level of providing with vitamin D is carried out. The research showed that decrease in maintenance of a number of antimicrobial peptides in blood serum against the background of deficiency of vitamin D acts as risk factor of development of acute bacterial diseases of a respiratory system of children of early age.
The purpose of the study was to estimate the interleukin-22 (IL-22) level in the blood serum of hypertensive patients with abdominal obesity (AO), depending on common carotid arteries (CCA) remodeling presence. Ninety-five subjects: 83 hypertensive patients and 12 healthy controls were examined. The patients were divided into two groups, depending on the presence of AO followed by the division of patients into subgroups depending on the intima-media thickness (IMT). For all patients, the following anthropometric parameters were measured: weight, height, waist circumstances (WC), hip circumstances (HC); the body mass index (BMI) and the waist-to-hip ratio (WC/HC) were calculated. The serum IL-22 level was determined in the patients by means of the Bender Medsystems® Human IL-22 Platinum ELISA kit by immunoassay method. Additionally, the patients underwent an ultrasound examination of carotid arteries. The study revealed that the CCA IMT in hypertensive patients with and without AO compared to the control group were significant differences. Serum IL-22 levels in hypertensive patients with AO having IMT ≥ 0.9 mm was statistically higher as compared with patients having IMT < 0.9 mm. 64 Kateryna Kysylenko and Olga Kovalyova The IL-22 levels in hypertensive patients without AO having IMT < 0.9 mm and having IMT ≥ 0.9 mm were significantly exceeds cytokine level in the control group. The obtained results may indicate that the blood serum IL-22 level is associated with a greater degree of vascular wall damage in hyprtensive patient both with and without AO. Higher IL-22 levels in the groups of hypertensive patients both with and without AO, with normal IMT compared with the control group, probably indicate that elevated blood serum IL-22 content can promote an early affection of the vascular endothelium.
Age-related cataract is currently the leading cause of incurable blindness in the world and the most common eye disease in people over 60 years. Among all ophthalmic diseases, cataract is about 65%. The following medicines are used to treat cataracts: Oftan Catachrom (Santen AO, Finland), Taufon (Farmak, OAO, Kiev, Ukraine.), Quinax (Alcon -Couvreur, Belgium), Vita-Iodurol (Excelvision AG for Novartis Pharma AG, France/Switzerland), Potassium iodide (OOO “UNIMED PHARMA”, Slovak Republic). Based on the foregoing, it is clear that the range of drugs for the treatment of cataracts is limited, and mainly consists of imported drugs. All of the above opens up new prospects for the creation of new ophthalmic medicines for expanding the range of domestic drugs, namely eye drops with wound healing, reparative, anti-inflammatory effects. Therefore, the aim of our work is to create a new ophthalmic drug based on the original substance Angiolin, which will exhibit anti-inflammatory, wound healing, reparative properties. cataract was reproduced on both eyes of 10 animals (20 eyes) by chemical induction of free radical oxidation of biopolymers of eye tissues according to the D.K. Buyan method, for which 30 μl of a sterile solution of dibromide diquat at a dose of 600 nmol was injected once into the vitreous body. Treatment began from 7 days, when the initial opacity of the lens was formed, and was carried out by instillation of drugs into the conjunctival cavity of the eyes 3 times a day. Results: the course administration of 1% Angiolin eye drops had a significant therapeutic effect in the conditions of chemical cataracts. The use of Angiolin drops reduced the lens opacity of the eye of experimental animals by 43,5%. Findings: 1% Angiolin eye drops ((S) -2,6-diaminohexanoic acid 3-methyl-1,2,4-triazolyl-5-thioacetate) as an active ingredient, exhibit significant anti-inflammatory, wound healing, reparative properties, and also have a high therapeutic effect in the treatment of cataracts.
Introduction. When arterial hypertension (AH) is combined with rheumatoid arthritis (RA), there are significant imbalances between the production of lipid peroxidation (LP) derivatives and the ability to neutralize these reactive substances. Aim of the study: To investigate the lipid profile, the thickness of the intimamedia complex (TIMC) of common carotid arteries in patients with rheumatoid arthritis in combination with arterial hypertension, and to correlate these indicators with the manifestations of oxidative stress, and the antioxidant system. Materials and methods: The results of the study are based on a comprehensive survey of 96 patients with stage 2 hypertension in combination with RA, 45 patients with stage 2 hypertension and 31 healthy volunteers. Research results. The stage of hypertension significantly influenced the manifestations of atherosclerotic process and oxidative stress, average TIMC indexes of the right CCA 1.28 (1.08; 1.30) mm and left CCA 1.29 (1.06; 1.30) mm in patients with RA in combination with stage 2 AH were significantly higher by 24.2% and 24%, respectively, against rates of RA patients in combination with grade 1 AH (p <0.01). The detection rate of atherosclerotic plaques in the subgroup of RA patients 102 Nataly G. Ryndina in combination with grade 2 AH was significantly higher 10 times, compared to the subgroup of RA patients in combination with stage 1 AH (p <0.01). Significant relation between LP products indices and manifestations of atherosclerotic process in patients with hypertension in combination with RA, direct linear correlation of medium strength between indices of malonic aldehyde and LDL R = + 0,57, TG R = + 0,56, TIMC R = + 0.55, respectively (p <0.05), diene ketones and TG R = + 0.52, TG R = + 0.55, respectively (p <0.05). Conclusions. The highest predictive value for the detection of atheromatous plaques in the CCA system had malonic aldehyde (AUC 0.870, 95% CI AUC 0.784 to 0.931) at the optimum distribution point > 5.25 nmol/ml, sensitivity 88.73%, and specificity 90.43%, and diene ketones (AUC 0.869, 95% CI AUC 0.784 to 0.930) sensitivity 90.36% and specificity 88.87% at the optimum distribution point > 5 c.un./ml.
USE OF BIOMARKERS IN PLANNING PROSTHETIC REHABILITATION IN PATIENTS WITH PERIODONTIUM TISSUES DISORDERS
Heart failure (HF) is a major factor contributing to premature death in patients with established cardiovascular (CV) disease. Current cell therapies of HF based on at least two different approaches, in which stem cell transplantation and extracellular vesicles (EV) received from pre-treated cells, were used. Although in HF restoring cardiac function aimed improve survival is target of cell therapy, it is not yet yielded whether isolated EVs from circulating progenitor cells, induced pluripotent stem cells (iPSCs), human embryonic stem cells-derived CV progenitors (hESC-Pg) would be effective translators of reparative signals for cardiac repair. The short communication is depicted the role of different cell strategies in HF. It has emphasized lack of strong evidence of clinical advantages implanted EVs to human cells to iPSCs in HF requires comparison in large clinical trials.
