Biphenotypic sinonasal sarcoma is a rare tumor (149 cases described so far in the literature) that arises from the mucosa of the upper respiratory tract, and it is characterized by aggressive biological behavior and a marked tendency to invade the skull base and the orbit. According to current knowledge, surgery represents the gold standard for treatment. The role of adjuvant therapies remains unclear. The particularity of the presented case: the tumor had its insertion point in the right frontal sinus, and from this level, it invaded both ethmoidal sinuses through partial destruction of the frontal intersinusal wall and dislocation of the structures in the bilateral nasofrontal canal. Although we were dealing with a large and highly vascularized tumor, with a clear indication for external surgery (at least through the supraorbital approach), efficient preoperative embolization and the use of angled endoscopic instruments, under the control of 70° and 90° endoscopes, made it possible to achieve a grossly complete ablation through an exclusive endoscopic transnasal approach of the Draf III type (modified Lothrop procedure). Further studies are needed, including large surgical series and long-term follow-up, to define prognostic factors and establish clear therapeutic guidelines for this rare and peculiar pathological entity.
Tonsillectomy involves the surgical removal of the palatine tonsils, and it is one of the most common surgical procedures in the otorhinolaryngology department worldwide, performed in both children and adults. It is a procedure recommended to be carried out, according to European standards, under general anesthesia, minimizing patient discomfort and pain. Accordingly, nasotracheal intubation is more advantageous during tonsillectomy, as it provides a wider surgical field and helps prevent serious intraoperative complications, such as damage to the endotracheal tube. Currently, we are implementing this method in our practice, which enhances both the surgeon's access to the operative field and the patient's comfort and safety, without any reported complications.
Objective. To compare clinical performance, osseointegration, esthetics, maintenance, and patient-reported outcomes of multibody dental implants versus standard implants for partial and full-arch rehabilitation. Materials and method. A mixed cohort of approximately 50 patients was assessed in a prospective arm (multibody implants) and a retrospective arm (standard implants). Minimally invasive (flapless) surgery was planned with panoramic radiographs and CBCT, and primary stability was measured using resonance frequency analysis (Penguin RFA; typical ISQ range: 55-80). Immediate or delayed (4-6 months) loading was selected according to initial stability and systemic/occlusal context. Clinics and patient identifiers were anonymized; numerical values were rounded. Main results. Three-to four-year survival was comparable between groups (approximately 94-96%), with early failures ranging between 4% and 6%. Multibody implants simplified the prosthetic workflow (fewer connections), reduced the risk of intermediate component loosening, and enabled more predictable maintenance. In cases suited for immediate loading, ISQ supported the clinical decision; delayed loading remained prevalent. High-performance polymer (PEEK/BioHPP) superstructures showed favorable biomechanical behavior and superior esthetics, although long-term monitoring is advisable. Conclusions. For extensive edentulism or challenging anatomy, multibody implants represent a safe and effective alternative to standard implants, with prosthetic and maintenance advantages. Careful case selection, digital CAD/CAM protocols, and objective stability monitoring via RFA are recommended.
This study was conducted within the Department of Otorhinolaryngology of the "Nicolae Testemițanu" State University of Medicine and Pharmacy, Chişinău, Republic of Moldova, in partnership with the "Emilian Coțaga" Clinic in Chișinău, during the period 2019-2024. The aim of the research was to optimize diagnostic and treatment methods for chronic hypertrophic rhinitis (CHR) in children, by comparing the effectiveness of two surgical interventions: bipolar forceps cauterization and diode laser surgery. A controlled clinical study was applied to a sample of 128 patients, aged between 7 and 18 years old, randomly divided into two equal groups. The analysis included the evaluation of quality of life using the SNOT-22 questionnaire. The results highlighted the effectiveness of both methods, with a significant impact on symptom relief and quality of life. The study makes an important contribution by adapting the SNOT-22 questionnaire for children in the Republic of Moldova, representing an innovative step in the assessment of CHR.
Introduction. Chronic suppurative otitis media is a chronic, progressive, active inflammation of the mucoperiosteum, characterized by persistent otorrhea, perforation of the tympanic membrane and hearing loss. Complications include progressive damage to middle ear structures due to auditory polyps, cholesteatoma and other infections. Methodology. A retrospective study was conducted on a group of 87 patients, aged between 18 and 82 years old, who addressed the Chişinău ENT Clinic presenting otomastoid complications caused by chronic suppurative otitis media. The following parameters were evaluated: age, sex, etiology, treatment, associated pathology and outcome. Results. In the case of the 87 patients included in the study, the predominant environment of origin was rural, with a male/female ratio of 44/43. The most common complication was mastoiditis (52 patients), 35 patients presented labyrinthitis, while in 11 patients the lateral semicircular canal was affected, six patients had facial nerve paresis, and six patients had an open oval window. Regarding the laboratory examinations, biological samples were collected in order to establish the pathogenic germs. The survival rate of patients diagnosed with otomastoid complications, treated in our clinic, was 99%, with only two deaths. Conclusions. Otomastoid complications are increasingly common in the Chişinău ENT Clinic and represent life-threatening medical and surgical emergencies. Early diagnosis, antibiotic administration and targeted surgical interventions are essential in managing this condition, especially in disadvantaged rural populations.
Nowadays, the nursing profession is predominantly female, with women making up the majority in this field in many countries. They are responsible for the care of patients in hospitals, medical clinics, nursing homes, and other health-care institutions. In this multi-part material, we will present brief biographies of remarkable women who have effectively revolutionized nursing in Europe and the United States in the 19th century and the early decades of the 20th century.
Restoring masticatory, phonetic and esthetic functions, the full-arch implant-supported bridge represents the best solution in cases of complete edentulism. Although it is not the most affordable therapeutic alternative, it still represents the ideal solution for patients seeking effective chewing and appropriate esthetics.
