A case of acute tuberculous pneumonia diagnosed by transbronchial biopsy is presented. We postulate that the unusually rapid progression was due to an exudative hypersensitivity reaction to aspirated tuberculoprotein.
ASTHMA is a very common disorder about which there are many confusing issues and conflicting views. The general practitioner, the psediatrician, the allergist and the psychiatrist will often have very different concepts about asthma because each sees a different sample of patients. Parents with afllicted children often have equally different views. We can all be like Humpty Dumpty in our concept of asthma or can have our doubts like Alice. Some of our doubts can be expressed in our attempts to answer the following questions. How prevalent is asthma? What is the relationship of wheezy bronchitis to asthma? When does a child with recurrent attacks of wheezy bronchitis became asthmatic? What is the natural history? Can the outcome of attacks in early childhood be predicted? Are· there different types of asthma, such as infective asthma, allergic asthma, psychosomatic asthma? Can the attacks be prevented? In a recent article in the British Medical Journal, H. E. Williams and K. N. McNicoP have published the results of a continuing epidemiological study of wheezing in children in Melbourne, which help clarify some of these difficulties and answer some of these questions. They have avoided one of the major pitfalls of many previous studies by making random selection of subjects from the general community and by including subjects who not only have asthma but also have wheezy bronchitis. Three groups of seven-year-old school children with mild wheezy bronchitis, with moderate wheezy bronchitis and with asthma were randomly selected and compared with a control group, the groups being followed until the subjects were 10 years of age.
