
Cet article propose un état des lieux des connaissances sur les inégalités sociales de santé liées à l’alimentation en France. Il aborde l’insécurité alimentaire, les déterminants sociaux et économiques des pratiques alimentaires des ménages modestes, ainsi que leurs conséquences nutritionnelles. Le constat important est qu’il est plus difficile d’adopter une alimentation à la fois saine et durable lorsque le budget alimentaire est contraint. Sans viser une analyse des évolutions nécessaires des politiques nutritionnelles, sociales ou environnementales (débat essentiel mais dépassant le cadre de cet article), notre objectif est de fournir aux professionnels de santé et de l’action sociale des repères pour mieux comprendre à la fois les contraintes structurelles auxquelles font face les personnes à faibles revenus et les stratégies qu’elles déploient au quotidien. En rendant visibles ces obstacles et ces marges de manœuvre, l’article vise à favoriser une adaptation des discours et des messages, afin de mieux accompagner ces publics. Nous identifions quatre stratégies complémentaires permettant de tendre vers des approvisionnements alimentaires plus sains et plus durables avec un petit budget : (i) maîtriser les quantités, (ii) éviter les aliments chers dans leur catégorie, (iii) acheter à faible prix un aliment donné et (iv) rééquilibrer les régimes en végétalisant de manière adéquate. Ces stratégies, déjà largement mises en œuvre dans les foyers modestes, sont analysées à l’aune de leurs bénéfices mais aussi des risques potentiels qu’elles comportent.
L’hypertriglycéridémie modérée<500mg/dL est une maladie qui touche un nombre croissant de personnes et est associée à une augmentation du risque d’événements cardiovasculaires. Les fibrates, traitement historique de l’hypertriglycéridémie, s’ils réduisent bien les triglycérides, n’ont pas montré de bénéfice clair sur la survenue d’événements cardiovasculaires. La supplémentation par acides gras oméga-3 par EPA à forte dose permet une réduction des maladies cardiovasculaires, mais celle-ci semble indépendante de la diminution des triglycérides. Enfin, de nouvelles molécules reposant sur des anticorps monoclonaux, des oligonucléotides antisens ou des petits ARN interférents font l’objet d’études pour traiter les hypertriglycéridémies modérées ou sévères. L’objectif de cette revue est de décrire les principaux traitements actuellement disponibles ou à l’étude dans l’hypertriglycéridémie.
The aim of this study was to reveal the effects of obesogenic diet on metabolic dysfunctions and histopathological alterations in visceral adipose tissue (VAT), in Gerbillus gerbillus, as well as to assess the possible curative effects of 20-hydroxyecdysone (20E) in the same animals. Fifteen male gerbils were divided into three groups: control (C), HC and HC-20E (animals receiving a high-calorie diet without or with a supplement of 20E, respectively). Twelve weeks later, body weight, plasma biochemical parameters were evaluated. The VAT was removed for pathobiological evaluation using histological and immunohistochemistry techniques. Compared to control animals, HC-fed gerbils showed an increased body weight, marked visceral fat deposition, adipocyte hypertrophy, VAT oxidative stress, inflammation, increased intercellular fibrosis combined with higher level of plasma glucose and dyslipidemia. The present study revealed that the supplementation of 20E ameliorated HC-increased adipose size, macrophage and neutrophil recruitment in VAT in obese gerbils associated with reducing oxidative damage. Our research suggests that 20E may represent a suitable natural compound that reduced VAT damage induced by the high-calorie diet. (c) 2026 Societe Franc & cedil;aise de Nutrition (SFN). Contexte La diversification alimentaire (DA) recouvre d’importants enjeux de santé et fait l’objet de nombreuses recommandations médicales qui sont inégalement suivies par les parents. Des disparités sociales subsistent dont les ressorts doivent être précisés. Méthodologie Au moyen de l’enquête ELFE représentative des naissances en France en 2011 (n=18 000), les effets des caractéristiques sociales des parents et de leur investissement dans la recherche d’information en matière de puériculture, sur la probabilité d’une DA dès 4 mois et de l’introduction avant 1 an de matières grasses végétales (MGV) et animales (MGA) dans le régimes des nourrissons, sont estimés par régressions probit en 2 étapes, afin de contrôler les biais de sélection généralement négligés. Résultats Le niveau de diplôme des mères est corrélé positivement, toutes choses égales par ailleurs, à l’introduction des MGV dans l’alimentation des enfants avant 1 an (p<0,001) et négativement à l’initiation de la DA dès 4 mois (p<0,001). Les mères qui ont consulté de multiples supports de puériculture ont plus fréquemment introduit des MGV et MGA dans l’alimentation de leurs enfants avant 1 an (p<0,001) mais aussi initié plus souvent leur DA dès 4 mois (p<0,001). L’investissement dans la recherche d’information de puériculture produit davantage d’effets sur la DA pour les catégories sociales intermédiaires (p<0,001) que pour les autres catégories. Conclusion Les parents sont inégalement exposés aux différentes recommandations nutritionnelles en matière de DA. Les niveaux d’éducation et les modèles de puériculture ont des effets propres irréductibles mais également conditionnés les uns aux autres. La conformation aux recommandations médicales en matière de DA ne peut être expliquée pleinement par les capacités socio-cognitives des parents à traiter l’information santé.
Physical activity (PA) is a fundamental element in the primary, secondary and tertiary prevention of many chronic diseases. For seniors, physical activity helps to slow down age-related physiological changes, improving physical and mental health as well as social cohesion. In addition to preventing chronic disease, it also promotes autonomy by reducing the risk of age-related dependency. There are, however, age-related physiological changes, such as a drop in maximum heart rate, a reduction in muscular strength and power, or a reduction in vital respiratory capacity. This can lead to a loss of functional autonomy and limit physical activity. However, physical activity can be adapted, and recommendations are the same for the elderly as for young people. Physical activity has few contraindications, few risks and many benefits, which is why it is advisable to offer programs adapted to older subjects, such as CAPS or Vivafrail.
Obesity, a complex and multifactorial disease, has become the leading driver of type 2 diabetes (T2D) worldwide. Once viewed as purely metabolic disorders, obesity and T2D are now recognized as chronic inflammatory diseases. Adipocyte hypertrophy in obesity triggers infiltration of pro-inflammatory macrophages into adipose tissue, releasing cytokines such as TNF-alpha, IL-6, and IL-1 beta. These mediators disrupt insulin signaling in the liver, muscle, and adipose tissue, leading to insulin resistance, the hallmark of "metaflammation," a low-grade, chronic inflammation underlying diabesity. The emerging field of immunometabolism reveals that adipose tissue acts as an immune organ, where hypoxia, nutrient excess, and lipid accumulation perpetuate a self-sustaining inflammatory loop. This inflammation contributes to metabolic dysfunctions and complications affecting the cardiovascular system, liver, and brain. Traditional therapies focused solely on glucose control are often insufficient. Novel anti-inflammatory strategies, such as IL-1 beta or NLRP3 inhibitors, offer promising outcomes by enhancing insulin sensitivity, reducing weight gain, and mitigating cardiometabolic risk. A paradigm shift is thus underway: treating obesity and T2D through an integrative approach that restores metabolic and immune homeostasis. Beyond glycemic control, targeting inflammation emerges as the cornerstone of future therapies against diabesity. (c) 2025 The Authors. Brown adipose tissue is specialized in energy dissipation in the form of heat through the process of non-shivering thermogenesis. Numerous studies in rodents have demonstrated the major role of brown adipose tissue, as well as that of beige adipose tissue - which has similar metabolic and secretory properties but also differences that remain to be better defined - in thermoregulation and energy balance. In humans, brown/beige adipose tissues are very present in newborns, decline with age, and are positively associated with cardiovascular and metabolic health. However, the real impact of these tissues on human metabolism in adults remains highly controversial. In addition to their potential role in maintaining energy homeostasis at the whole organism level, these tissues may exert local effects by creating specific microenvironments that contribute to the proper functioning of tissues and organs present in their close proximity. In this review, we describe the biology of these tissues exhibiting remarkable oxidative capacities, the methods enabling their detection, and retrospective and prospective clinical studies investigating their role in human health. A summary is presented here. Cereal products have been consumed since antiquity and are an important component of the Mediterranean diet (Bach-Faig et al, 2011). The Mediterranean diet is considered a model of sustainable eating due to its health benefits, its social and cultural food identity, its potential for territorial development, its lower environmental impacts, and its contribution to biodiversity conservation (Lorca-Camara et al, 2024). Furthermore, recommendations from the EAT-Lancet report and various international organizations - the Food and Agriculture Organization (FAO), the World Health Organization (WHO), and the United Nations (UN) - emphasize the need for a transition toward more sustainable diets, based on reducing the consumption of animal proteins in favor of plant proteins. This summary on cereal products reflects the presentations from the webinar on (i) the health impact of glycemic index and glycemic response, (ii) their relevance as carriers of proteins with a focus on pasta, and (iii) the combined nutritional and environmental impact of pasta-based dishes. Superior mesenteric artery syndrome (SMAS) is responsible for an acute or chronic, complete or partial upper gastrointestinal obstruction, resulting from compression of the third duodenum between the superior mesenteric artery and the aorta. Diagnosis is based on a combination of clinical and radiological findings. Treatment consists mainly of gradual artificial re-feeding, with the main aim of weight gain and, at minimum, restoration of visceral fat mass sufficient to relieve the vascular compression in most of cases. Here, we report the case of a 24-year-old male patient with the characteristics of constitutional thinness who consulted after multiple unsuccessful attempts to gain weight in the context of digestive symptoms when increasing his food intake. Upper gastrointestinal series revealed gastro-esophageal reflux, gastric dilatation and partial obstruction of the contrast flow in the second duodenum. Abdomino-pelvic CT scan showed a significantly reduced aorto-mesenteric angle and distance. The diagnosis of superior mesenteric artery syndrome was finally made after a long period of diagnostic uncertainty. Enteral nutrition via a nasogastric tube over a 4 months led to weight gain which could be maintained with prokinetics despite the absence of significant opening of the aorto-mesenteric clamp.(c) 2025 The Author(s). Constitutional thinness (CT) refers to a naturally low and lifelong stable body weight, without any identifiable pathological cause. It affects 1-2% of the population and is characterized by a low BMI in the absence of clinical or biological signs of malnutrition, together with strong heritability. The diagnosis relies on excluding nutritional, endocrine, digestive, psychiatric, or tumoral causes of low BMI. It is most often established during adolescence, when distinguishing CT from anorexia nervosa (AN) becomes crucial. Unlike AN, individuals with CT do not restrict their food intake, do not fear weight gain, and exhibit neither body-image distortion nor psychological features of eating disorders. Their weight trajectory remains consistently low, and menstrual cycles are generally preserved. Energy intake is comparable to that of the general population, although meals are fractionated due to early satiation. Despite a paradoxically positive energy balance, CT is marked by a pronounced resistance to weight gain, combining rapid lipid turnover with hypofunctional skeletal muscle. Bone mineral density is also reduced. From a therapeutic perspective, CT requires tailored educational and nutritional support, along with careful monitoring of bone fragility risk. Fermentation has held a central place in human nutrition for millennia, both for the preservation of perishable goods and for their transformation. Fermented foods indeed have unique sensory properties, in addition to health benefits that are increasingly well-documented. Furthermore, fermentation contributes to the sustainability of our food systems. As we move towards more natural, plant-based and sustainable diets, fermentation represents a key technology for transforming agri-food systems. The Ferments du Futur (FdF) initiative, led by INRAE and ANIA with the support of France 2030, aims to develop new ferments, to fermentation processes and to create high added-value fermented foods, drawing on biotechnologies, data science and public-private collaborations. This article outlines the major challenges associated with fermentation: food safety, sensory properties, nutrition and health and environmental impact, while highlighting the concrete solutions that FdF will provide, through its projects and technological platforms. Depuis ces deux dernières décennies, la restauration scolaire en France fait l’objet d’un accroissement des normes et des recommandations à plusieurs niveaux : accès, équilibre nutritionnel, produits de qualité et durables, diversification des protéines, gaspillage alimentaire, règles sanitaires. En septembre 2011, la publication d’un décret et d’un arrêté rend obligatoire certaines recommandations issues du Groupe d’étude des marchés de restauration collective et nutrition (GEM-RCN). En particulier, adapter le grammage des portions servies selon l’âge des convives afin de lutter contre le surpoids et éviter le gaspillage alimentaire. La taille des portions est ainsi estimée à 300g environ pour les jeunes mangeurs de 6 à 11 ans (en école élémentaire). Ces grammages ont été établis sur la base d’une seule étude qui n’a pas été publiée. À partir d’un travail d’enquête documentaire retraçant l’origine des recommandations extraites du GEM-RCN, je mets en lumière un point peu discuté des politiques nutritionnelles : le faible socle scientifique sur lequel repose une norme nationale largement appliquée. Se pose alors la question de savoir si les grammages préconisés demanderaient à être actualisés étant donné le nombre d’enfants qui estiment que la quantité de nourriture servie est insuffisante et un gaspillage alimentaire dans les restaurants scolaires encore bien présent.
Le tissu adipeux brun est spécialisé dans la dissipation de l’énergie sous forme de chaleur, par le processus de thermogénèse de non-frisson. De nombreuses études chez les rongeurs ont montré le rôle majeur du tissu adipeux brun ainsi que celui du tissu adipeux beige – présentant des similitudes dans les propriétés métaboliques et sécrétoires mais également des différences qui restent encore à être mieux définies – dans la thermorégulation et dans le maintien de la balance énergétique. Chez l’homme, les tissus adipeux brun/beige sont très présents chez le nouveau-né, déclinent avec l’âge, et sont positivement associés à la santé cardiovasculaire et métabolique. Cependant, l’impact de ces tissus sur le métabolisme chez l’adulte est très controversé. Parallèlement à un potentiel rôle dans le maintien de l’homéostasie énergétique à l’échelle de l’organisme entier, ces tissus pourraient exercer des effets locaux par la création de microenvironnements spécifiques contribuant au bon fonctionnement d’organes présents à leur proximité. Dans cette revue, nous faisons le point sur la biologie de ces tissus aux capacités oxydatives remarquables, sur les méthodes de détection et sur les résultats d’études cliniques rétrospectives et prospectives étudiant leur rôle dans la santé humaine.
Glucose-dependent insulinotropic polypeptide (GIP) was the first incretin identified and it plays a crucial role in maintaining glucose tolerance in healthy individuals. Until recently, GIP was overshadowed by the other incretin, glucagon-like peptide-1 (GLP-1), which has led to a class of highly effective new drugs for treating diabetes and obesity. In this review, we present an updated overview of the multifaceted biology of GIP. Its key actions include increasing glucose-stimulated insulin secretion, modulating lipid storage and lipolysis in adipose tissue, reducing body weight through centrally mediated appetite suppression, alleviating drug-induced nausea, decreasing peripheral and central inflammation, enhancing bone formation, and improving cognition. This pleiotropic peptide is now a promising candidate for the development of new pharmacotherapies adapted not only to disorders of energy, carbohydrate and lipid metabolism, but also to bone and neurodegenerative disorders. Objectives. - To evaluate the effectiveness of a hybrid therapeutic patient education program (face-to-face education plus WhatsApp-based follow-up) on gluten-free diet adherence, eating attitudes, and health-related quality of life in adults with celiac disease. Methods. - A two-arm randomized controlled trial was conducted. Adults (>= 19years) with confirmed celiac disease were randomized (1:1) to an intervention group or a control group using a computer-generated sequence. The intervention consisted of two face-to-face PowerPointbased education sessions (60 min each; total 120 min) followed by an 8-week WhatsApp followup with weekly online Q&A sessions. Outcomes were assessed at baseline (T0) and at 8 weeks (T1) using the Celiac Dietary Adherence Test (CDAT), the Celiac Disease Food Attitudes and Behaviors Scale (CD-FAB), and the Health-Related Quality of Life in Celiac Disease Scale (HRQoL-CD). Results. - The education program significantly improved dietary adherence, eating attitudes, and quality of life. CDAT scores decreased, while HRQoL scores increased across all domains. Conclusions. - Hybrid education programs incorporating digital follow-up can enhance adherence and psychosocial outcomes in celiac disease, offering a scalable complement to routine clinical care. Les complications métaboliques de l’obésité sont associées à une perte d’homéostasie intestinale et à une inflammation chronique de bas grade. Centrales énergétiques des entérocytes, les mitochondries jouent un rôle clé dans le maintien de cette homéostasie. Nous avons évalué si leur altération pouvait contribuer à l’hyperperméabilité intestinale observée dans l’obésité. Chez la souris, un excès de lipides alimentaires provoque une accumulation de gouttelettes lipidiques dans les entérocytes, entraînant une baisse du nombre et de l’activité mitochondriale, associée à une perte du phénotype mature et de la fonction de barrière. Dans les organoïdes jéjunaux murins, la réduction de l’activité mitochondriale limite la différenciation des progéniteurs en entérocytes matures, pouvant favoriser l’hyperperméabilité. Dans le côlon, le régime hyperlipidique induit aussi une dysfonction mitochondriale et une hyperperméabilité liée à une dysbiose marquée par les Desulfovibrionaceae et les sulfures, inhibiteurs du complexe IV mitochondrial. Ces travaux soulignent le rôle clé des mitochondries dans les déséquilibres intestinaux liés à l’obésité et ouvrent de nouvelles pistes thérapeutiques.

Synthèse du workshop SFN en partenariat avec Panzani donné en visioconférence le jeudi 5 juin 2025. Cette session s’est tenue en digital le 5 juin 2025. Les produits céréaliers sont consommés depuis l’antiquité et constituent un élément important du régime méditerranéen (Bach-Faig et al, 2011). L’alimentation méditerranéenne est considérée comme un modèle de consommation alimentaire durable pour ses bénéfices en matière de santé, son identité alimentaire sociale et culturelle, son potentiel pour le développement territorial, de plus faibles impacts environnementaux et une conservation de la biodiversité (Lorca-Camara et al, 2024). Par ailleurs, les recommandations du rapport EAT-Lancet et de divers organismes internationaux – l’Organisation pour l’alimentation et l’agriculture (FAO), l’Organisation mondiale de la Santé (OMS), l’Organisation des Nations unies (ONU) - insistent sur la nécessité d’une transition vers des régimes alimentaires plus durables, fondés sur une réduction de la consommation de protéines animales au profit de protéines végétales. La présente synthèse sur les produits céréaliers reprend les présentations du webinaire sur (i) l’impact sur la santé de l’index glycémique et de la réponse glycémique (ii) leur intérêt comme vecteurs de protéines avec un focus sur les pâtes alimentaires et (iii) l’impact combiné nutritionnel et environnemental de plats à base de pâtes alimentaires.
