
Background Contraceptive counselling advice for adolescents with special health concerns is a highly relevant but often neglected aspect of healthcare. Risk constellations can influence not only physical health due to chronic medical conditions, organ alterations, congenital conditions or potential drug interactions but also psychological, cognitive and emotional well-being. While contraindications and interaction profiles for hormonal contraceptives in adolescence mostly do not differ from those in adults, it is of paramount importance to address the unique developmental characteristics of this age group. Material and methods This article presents counselling aspects during gynecological consultations of adolescents requiring hormonal contraception based on four exemplary risk constellations and discusses these in the context of the literature. The risk constellations include autism spectrum disorders, congenital heart malformations, urogenital malformations and psychosocial conflict situations. Conclusion Adolescents require individualized contraceptive counselling that includes both medical factors and psychosocial needs. Not only general information about the benefits, side effects and risks of hormonal contraception should be addressed but also chronic conditions and personal life circumstances. This process involves providing guidance during the period of transition, including the shift to adult healthcare and often addresses broader topics such as independence, autonomy and personal responsibility.
Hormonal contraception is widely used. Contraindications as well as additional therapeutic benefits have to be considered during contraceptive counseling. Finding the most suitable contraceptive method can be challenging in women with comorbidities, such as depression or premenstrual dysphoric syndrome (PMDS). This is especially the case if first line options, such as combined oral contraceptives (COC) are not an option due to contraindications or the response to antidepressant medication is insufficient. Although there is a slightly elevated risk of depressive moods when using progestin-only contraceptive (POC) methods, pre-existing depression does not usually worsen with POC use and newly occurring mood changes appear to resolve quickly after discontinuation of hormone use. Therefore, all POC-based methods can be used for contraception after appropriate patient clarification. A close follow-up regarding mood changes is recommended after restarting in patients with a higher risk of depression. Currently, there is no evidence for the impact of progestins in women with PMDS. The use of ovulation-inhibiting progestin-only methods can be considered in the individual risk-benefit assessment.
Several studies currently indicate an association between the use of certain progestins and the risk of undergoing meningioma surgery. The risk seems to be associated with the dose and duration of use. The absolute incidence for such an event is low; however, with the broad use of magnetic resonance imaging (MRI) asymptomatic meningioma is detected in about 1% of the images. It is a challenge for gynecologists to decide for these patients, if the use of a progestin could induce further growth of such a tumor. In such a situation collaboration with a neuro-oncologist is highly recommended. Enquiring about a meningioma diagnosis in the past should be part of the general medical history before prescribing hormone preparations containing progestins.

Hormonelle Kontrazeption ist weit verbreitet. Neben möglichen medizinischen Zusatznutzen müssen in der Beratung auch Kontraindikationen beachtet werden. Eine besondere Herausforderung in der Wahl des geeigneten Verhütungsmittels können Komorbiditäten wie Depressionen oder das prämenstruell dysphorisches Syndrom (PMDS) darstellen, insbesondere wenn kombinierte orale Kontrazeptiva als Erstlinientherapie aufgrund von Kontraindikationen keine Option sind oder Antidepressiva aufgrund eines fehlenden Ansprechens nicht verwendet werden können. Zwar sind depressive Verstimmungen mit einer leichten Risikoerhöhung unter Gestagen-only-Methoden möglich, jedoch wird eine vorbestehende Depression in der Regel nicht verschlechtert und neu aufgetretene Stimmungsveränderungen erscheinen nach Absetzen zeitnah reversibel. Nach entsprechender Aufklärung sind daher alle gestagenbasierten Methoden zur Kontrazeption anwendbar. Bei Patientinnen mit erhöhtem Risiko für eine Depression wird ein engmaschiges Stimmungs-Follow-up nach Neubeginn empfohlen. Für den Gestageneinsatz beim PMDS gibt es bisher keine Evidenz. Eine Anwendung ovulationshemmender Gestagen-only-Methoden kann in der individuellen Nutzen-Risiko-Abwägung in Betracht gezogen werden.
Die Kontrazeptionsberatung bei Jugendlichen mit besonderen gesundheitlichen Bedürfnissen stellt eine wichtige, aber oft vernachlässigte Komponente der Gesundheitsversorgung dar. Risikokonstellationen können sowohl den medizinisch-somatischen Bereich durch unterschiedlichste chronische Erkrankungen, Organveränderungen, angeborene Einschränkungen oder Medikamenteninteraktionen als auch den psychischen, geistig-kognitiven oder emotionalen Bereich betreffen. Hormonelle Kontrazeptiva im Jugendalter unterliegen grundsätzlich denselben Kontraindikationen und Interaktionen wie bei Erwachsenen, die Herausforderung besteht in den altersbezogenen Besonderheiten der Adoleszenz. In der vorliegenden Arbeit wird anhand von vier exemplarischen Risikokonstellationen die in der jugendgynäkologischen Sprechstunde erfolgte Beratung bezüglich hormoneller Kontrazeption dargestellt und anhand der Literatur diskutiert. Die Risikokonstellationen umfassen Autismus-Spektrum-Störungen, kongenitale Herzfehler, urogenitale Fehlbildungen und psychosoziale Konfliktsituationen. Jede Jugendliche benötigt eine individualisierte Herangehensweise an die hormonelle Kontrazeption, die sowohl die medizinischen Besonderheiten als auch die psychosozialen Bedürfnisse berücksichtigt. Jugendgynäkologische Verhütungsberatung muss neben der allgemeinen Information über Vorteile, Nebenwirkungen und Risiken der hormonellen Kontrazeption auch die chronische Erkrankung und die konkrete Lebenswelt der Jugendlichen einbeziehen. Es geht um die Begleitung in einer Zeit von Veränderungen und der Transition in die Erwachsenenmedizin, oft auch um Themen der Ablösung, Eigenständigkeit und Eigenverantwortung.
Arterielle ischämische Ereignisse sind im reproduktiven Alter selten; das Risiko für einen ischämischen Insult (etwa 10,7 pro 100.000 Frauenjahre) ist höher als das für einen Myokardinfarkt (etwa 5,0 pro 100.000 Frauenjahre). Mit dem Alter, besonders ab 35 Jahren, steigt das Risiko. Wichtige Risikofaktoren sind arterielle Hypertonie, Diabetes mellitus, Dyslipidämie, Rauchen, Adipositas und familiäre Belastung. Migräne mit Aura ist ein unabhängiger Risikofaktor für einen ischämischen Insult. Kombinierte hormonelle Kontrazeptiva verdoppeln das Risiko für Schlaganfall und Infarkt, wohingegen das levonorgestrelhaltige Intrauterinsystem und das Kupfer-Intrauterinpessar als sichere Alternative gelten. Vor Verordnung einer hormonellen Kontrazeption wird daher eine individuelle Risikoprüfung empfohlen.
Die multiple Sklerose (MS) betrifft überwiegend Frauen im reproduktiven Alter. Fragen zu hormoneller Kontrazeption und Familienplanung sind daher klinisch hoch relevant, insbesondere im Kontext krankheitsmodifizierender Therapien („disease-modifying therapies“ [DMT]). Darstellung praxisrelevanter Prinzipien der kontrazeptiven Beratung bei Patientinnen mit MS, wobei ein besonderer Fokus auf der hormonellen Kontrazeption im Kontext von DMT liegt. Übersichtsarbeit auf Basis aktueller Literatur und internationaler Empfehlungen zu Kontrazeption und reproduktiver Planung bei MS. Hormonelle Kontrazeptiva erhöhen weder das MS-Risiko noch beeinflussen sie die Krankheitsaktivität negativ. Die Wahl der Methode sollte individualisiert erfolgen und krankheitsspezifische Faktoren wie Mobilität, Thromboserisiko, kognitive Einschränkungen und Adhärenz berücksichtigen. Lang wirksame reversible Kontrazeptiva sind aufgrund ihrer hohen Effektivität und geringen Anwendungsabhängigkeit besonders geeignet. DMT unterscheiden sich erheblich in Bezug auf Teratogenität, erforderliche Auswaschphasen und Effektivität. Während zu Interferonen und Glatirameracetat sehr günstige Sicherheitsdaten vorliegen, erfordern insbesondere Sphingosin-1-Phosphat-Rezeptor-Modulatoren, Teriflunomid und Cladribin eine zuverlässige Kontrazeption. Therapieentscheidungen müssen eng mit der individuellen Familienplanung abgestimmt werden. Eine strukturierte, interdisziplinäre Beratung von Neurolog:innen und Gynäkolog:innen ist essenziell, um ungeplante Schwangerschaften zu vermeiden und gleichzeitig möglichst sichere Schwangerschaften durch vorausschauende Therapieplanung zu ermöglichen.
Die Verhütung bei Frauen mit Mamma- und Ovarialkarzinom ist komplex, da einerseits eine hohe kontrazeptive Sicherheit erforderlich ist, aber andererseits Hormone mit der onkologischen Sicherheit in Konflikt stehen können. Darstellung der aktuellen Evidenz zur Sicherheit und Eignung hormoneller und nichthormoneller Kontrazeptionsmethoden bei betroffenen Patientinnen. Narrative Auswertung aktueller Studien und Leitlinien zur Kontrazeption bei Mammakarzinom und Ovarialkarzinom. Nichthormonelle Methoden sind universell einsetzbar und onkologisch unbedenklich, wobei Kupfer-Intrauterinpessare und definitive Verfahren die höchste kontrazeptive Sicherheit bieten. Beim Mammakarzinom besteht eine Assoziation zwischen hormoneller Kontrazeption und Erkrankungsrisiko, ein erhöhtes Rezidivrisiko kann nicht ausgeschlossen werden. Leitlinien empfehlen daher den Verzicht auf hormonelle Methoden. Beim Ovarialkarzinom zeigen hormonelle Kontrazeptiva einen protektiven Effekt hinsichtlich der primären Erkrankungsinzidenz, während Daten zum Rezidivrisiko limitiert und uneinheitlich sind. Die Wahl der Kontrazeption erfordert eine individualisierte Nutzen-Risiko-Abwägung unter Berücksichtigung von Tumorbiologie, Begleiterkrankungen und Patientinnenpräferenzen.
Women with a genetic predisposition for breast and gynecologic cancer have concerns that hormonal interventions could increase their genetic cancer risk. As a result, they are often excluded from endocrine interventions for fertility treatment, contraception, hormone replacement after risk-reducing salpingo-oophorectomy (RRSO), or from treatment of peri- and postmenopausal symptoms. The following statements and recommendations were formulated by the Hormone Working Group of the German Consortium for Hereditary Breast and Ovarian Cancer and are meant to support physicians in counseling affected women.
Borderline tumors (BOT) are epithelial ovarian tumors with increased proliferative activity and a lack of stromal invasion. They are treated exclusively by surgery and do not need adjuvant therapy. The recurrence rate is around 5%. Tumors with invasive implants belong to the group of low-grade serous ovarian cancers (LGSOC) and no longer to BOTs. Accordingly, they are treated in the same way as carcinomas. Almost 70% of BOTs are diagnosed in premenopausal patients. Serous BOTs can possess hormone receptors and therefore the question arises about endocrine therapy of LGSOC and also hormone replacement therapy (HRT) in premenopausal patients following bilateral salpingo-oophorectomy because of BOT. So far there are no clear recommendations for endocrine therapy in LGSOC; however, studies are currently recruiting patients with hormone receptor positive LGSOC for an endocrine maintenance therapy. There are also no clear data for HRT after BOT, but the increased morbidity and mortality in premenopausal patients after bilateral salpingo-oophorectomy are well-known, so that HRT should be recommended until the natural age of menopause (ca. 52 years) to preserve the quality of life.
Granulosa cell tumors (GCT) of the ovary arise from the ovarian mesenchyme and sex cords and account for 70% of all sex cord stromal tumors but less than 3% of all ovarian neoplasms. Characteristic for these tumors is their ability to secrete estrogens (less frequently androgens and progestogens) as well as their tendency to recur many years after initial excision, sometimes 10-20 years later. The recurrence rate is around 30%. Due to the estradiol produced by the tumor, these tumors can be associated with abnormal vaginal bleeding at an early stage. The standard treatment in the early stages is surgical removal of the tumor. The majority of these patients (78-91%) are in stage I when the cancer is diagnosed and 10-year survival rates of 84-95% can be achieved by surgery alone. In contrast, the prognosis for 10-year survival is considerably worse in advanced stages or cases of recurrence. If the tumor is unresectable, platinum-based chemotherapy is usually employed, although with conflicting results. Because GCTs are radiation-sensitive, radiotherapy is also used for treatment. To improve disease-free survival, overall survival and quality of life, alternative treatment strategies are needed. As germ cell tumors typically express steroid hormone receptors, hormone therapy has been proposed as a potential treatment option, with some promising results.
Adjuvant endocrine therapy (ET) for a minimum duration of 5 years remains the standard of care in hormone receptor-positive early breast cancer, with treatment extension recommended for patients at increased risk of recurrence. In postmenopausal women, aromatase inhibitors (AIs) or sequential strategies combining AIs and tamoxifen are preferred, while extended treatment durations of 7-10 years are advocated for high-risk patients. In premenopausal women, tamoxifen represents the standard in low-risk disease, whereas patients at higher risk derive additional benefit from ovarian function suppression combined with tamoxifen or an AI; in selected cases with persistently elevated recurrence risk, further extension of tamoxifen therapy by an additional 5 years may be considered. Given the high incidence of breast cancer, adjuvant ET represents a major public health issue: in Germany alone, approximately 70,000 women are newly diagnosed with breast cancer each year, of whom around 70-80% have hormone receptor-positive disease and are therefore candidates for longterm endocrine treatment. ET-associated adverse effects can lead to clinically relevant and persistent impairments in quality of life and should be adequately recognized and managed. Treatment adherence is critical for therapeutic efficacy, and switching to better-tolerated agents is generally preferable to premature discontinuation. Ongoing and emerging evidence is expected to further refine and individualize adjuvant ET strategies.
Vasomotor symptoms (VMS-primarily hot flashes and night sweats) are among the most common side effects of endocrine therapy in breast cancer and occur in approximately 30-95% of patients. They significantly impair quality of life as well as adherence to endocrine treatment. A qualitative literature search was conducted on the diagnosis and management of VMS. While the most effective diagnostic approach is the use of patient-reported symptom diaries, several quantitative assessment tools are available, particularly in clinical studies, such as the Hot Flash Score, the Hot Flash-Related Daily Interference Scale (HFRDIS), and the Menopause-Specific Quality of Life (MENQOL) questionnaire. Therapeutic options include complementary approaches such as acupuncture, cognitive behavioral therapy, and hypnotherapy, as well as various pharmacological treatments. In addition to agents long used off-label (such as venlafaxine, gabapentin, and clonidine), new neurokinin receptor antagonists have become available, including fezolinetant and elinzanetant, the latter recently approved in the European Union for use in patients with breast cancer. Raising awareness of VMS and their associated burden, along with knowledge of the full range of therapeutic strategies, is essential for providing optimal care to affected patients and is therefore the aim of this article.
Between 8 and 13% of all women of reproductive age are affected by polycystic ovary syndrome (PCOS), making it one of the most common endocrinopathies in women. In recent years, increasing attention has been paid to the long-term consequences of PCOS for metabolism and cardiovascular health, and to the need for early preventive measures. Since this can only succeed through an interdisciplinary approach, a new S2k guideline for the diagnosis and treatment of PCOS was developed under the leadership of the German Society for Endocrinology (DGE), with the support of the Professional Association of German Internists/Endocrinology/Diabetology Section (BDI). The treatment concept for PCOS is multimodal and should include basic measures such as behavior modification, a healthy diet, and physical activity. As pharmacotherapy, metformin is considered highly important and can be used off-label in women with PCOS without diagnostic testing of glucose-insulin metabolism. When choosing hormone therapy for endometrial protection, in addition to cardiovascular risks, the patient's preferences should also be taken into account in order to achieve the greatest possible adherence. Ovarian stimulation with letrozole is the most effective first-line therapy for PCOS patients wishing to conceive.

Endometriose ist eine Erkrankung hoch komplexer Pathologie. Charakteristisch sind Infertilität und chronische Schmerzzustände. Darüber hinaus besteht ein 2‑ bis 4‑fach erhöhtes Risiko zur Entwicklung eines Ovarialkarzinoms. Die Molekularpathologie kennt inzwischen somatische Gen-Alterationen, die potenziell zu Endometriose und auch zu einer malignen Transformation führen können. Genomische Alterationen liegen nahe, sind aber zum jetzigen Zeitpunkt noch eher spekulativ.
