
PURPOSE:Despite the high incidence of 5th metacarpal neck (5MCN) fracture, the literature lacks concise information about the ideal treatment option. This review aimed to compare the functional outcome of conservative and operative management of 5MCN fractures. METHODS:The electronic databases PubMed and Scopus were used for the literature review. This review is intended to include all original studies comparing the outcomes of 5MCN fractures that were managed operatively and conservatively. RESULTS:The final inclusion of 5 eligible studies was based on full-text screening. A total of 311 patients were analyzed from the included five studies, with a majority of males (178/203, 83.7%). Significant reduction in palmar angulation (SMD -1.23, 95% CI -2.21 to -0.25) was noted in surgical group as compared to the conservative group. No significant difference was noted in terms of grip strength (SMD 0.96, 95% CI -2 to 3.98) or shortening (SMD 1.65, 95% CI: -2.39 to 5.69) in both the groups. CONCLUSION:Although no significant difference was noted in grip strength and shortening, there was statistically significant palmar angulation in patients treated conservatively. The vocation and expectations of each patient should be carefully considered before deciding on the course of treatment. Future studies with uniform functional outcome measurements must establish unambiguous clinical guidelines for treating 5MCN fractures.
BACKGROUND:Delayed-immediate breast reconstruction (DIBR) using a temporary tissue expander is commonly recommended when post-mastectomy radiotherapy (PMRT) is anticipated, with the aim of optimizing aesthetic outcomes. However, the actual benefit of tissue expanders compared with immediate breast reconstruction (IBR) remains debated. METHODS:We conducted a retrospective monocentric observational study including patients who underwent curative or prophylactic mastectomy followed by IBR or DIBR between January 2019 and December 2023. Reconstruction techniques included definitive implants, tissue expanders, and autologous flaps. The primary endpoint was aesthetic outcome assessed on standardized postoperative photographs using the Vrieling scale by six blinded plastic surgeons. Secondary endpoints included patient-reported outcomes using the BREAST-Q questionnaire, number of surgical procedures, capsular contracture (Baker classification), and postoperative complications (Clavien-Dindo). Multivariate analyses were performed to account for confounders, including radiotherapy. RESULTS:A total of 149 patients (192 reconstructions) were included (160 IBR, 32 DIBR). No significant difference in aesthetic outcomes was observed between IBR and DIBR overall, nor within implant-based or flap-based subgroups. BREAST-Q scores were comparable between groups across psychosocial, sexual well-being, and breast satisfaction domains, although chest physical well-being was significantly lower in the DIBR group (P=0.03). Patients undergoing flap reconstruction reported higher breast satisfaction than those with implant-based reconstruction (P<0.001). DIBR required significantly more surgical procedures than IBR (2.3 vs. 1.6; P<0.001). Radiotherapy negatively impacted patient-reported satisfaction but did not significantly influence aesthetic scores or complication rates between groups. CONCLUSION:Among patients who completed reconstruction, DIBR did not confer a measurable aesthetic advantage over IBR, while requiring more surgical procedures and being associated with lower chest physical well-being. Immediate reconstruction may be sufficient in selected patients, even when radiotherapy is required.
BACKGROUND:Perforator flaps have revolutionized upper limb reconstruction by providing reliable and aesthetically pleasing solutions while preserving major vascular axes. However, knowledge about their anatomy, surgical techniques, and indications remains scattered and poorly standardized, limiting dissemination among trainees and practicing surgeons. METHODS:We developed an interactive web platform (www.lambeauxperforants.fr) offering standardized educational sheets on eight major upper limb perforator flaps. Each sheet includes detailed anatomy, surgical technique, indications, intraoperative photographs, and instructional videos. The platform features an intuitive search engine designed for residents, fellows, and practicing surgeons. RESULTS:The platform centralizes critical knowledge on perforator flaps of the arm, forearm, and hand. Preliminary feedback from surgical trainees and specialists highlights its educational value, user-friendliness, and potential to improve preoperative planning. Early metrics show promising engagement, with positive evaluations regarding content quality, clarity, and practical applicability. CONCLUSIONS:This digital platform addresses a crucial gap in the teaching of upper limb perforator flaps. By improving access to structured, high-quality content, it fosters standardization of surgical practices and enhances resident education. Future developments will include 3D modeling integration, augmented reality tools, and expansion to other anatomical regions, setting a benchmark in the ongoing digital transformation of surgical education.
Introduction Arteriovenous malformations are high-flow vascular malformations linked to the presence of abnormal connections between feeding arteries and draining veins. They are frequently located in the head and neck, with major functional and morphological repercussions. Materials and methods Retrospective and descriptive study of patients operated on in our department for craniofacial arteriovenous malformations. Results Six patients: five male and one female, with lesions located in various areas: scalp, eyelid, infratemporal fossa and cheekbone. Treatment was standardised with preoperative embolisation and surgical excision of the arteriovenous malformation, except for two patients: one admitted as an emergency case for a ruptured AVM and the other for a superficial frontal AVM with surgically controllable feeding vessels, who were treated exclusively by surgery. The outcomes were straightforward with no signs of complications, and the loss of tissue was reconstructed using various repair techniques. Discussion AVMs are congenital high-flow malformations, but may become apparent later in life and sometimes after trauma. They have a highly polymorphic presentation: red or purplish skin and subcutaneous swelling, sometimes keratotic, warm, pulsatile with a murmur and sometimes a “thrill”; the course is unpredictable, sometimes stabilising and sometimes worsening, which can quickly become life-threatening for the patient. The diagnosis is radiological and clinical, and treatment is often based on a combination of embolisation and surgery. Conclusion The ultimate complication of AVMs is heart failure, which is a sign of slow progression. We recommend early treatment to prevent complications from occurring.
Objective This study aims to analyze global research trends, influential authors and institutions, citation dynamics, and the intellectual structure within the field of genioplasty using bibliometric methods. Methods Studies on genioplasty published between 1980 and 2025 were retrieved from the Web of Science Core Collection (WoSCC). Bibliometric analyses were conducted using R Studio and VOSviewer. Publication counts, citation trends, country productivity, author impact, journal distribution, co-citation networks, and keyword co-occurrence patterns were evaluated. Results The number of publications related to genioplasty has shown a steady increase over the years, reaching its peak in 2022 (n=77). The total number of citations peaked in 2024, indicating sustained interest and continued relevance in the field. The United States produced the highest number of publications and citations (302 publications, 5448 citations). China (n=118) and South Korea (n=87) also ranked among the most productive countries, reflecting the strong academic output driven by the high cultural demand for facial contouring procedures in Asia. Author analyses revealed that Hu J. and Posnick J.C. were the most influential contributors, while co-citation analysis identified Trauner R. and Bell W.H. as foundational figures in the intellectual structure of the genioplasty literature. Keyword analysis showed that “genioplasty”, “orthognathic surgery”, “osteotomy”, and “chin” were the most prominent terms, supporting the observation that genioplasty is frequently performed as a complementary procedure to orthognathic surgery. Conclusion Research on genioplasty has gained significant momentum over the past 45 years, resulting in a global research network predominantly led by the United States and Asian countries. The literature demonstrates that genioplasty remains a crucial component of aesthetic and functional facial surgery, with contemporary studies focusing on refining surgical techniques and improving long-term stability.
Background Volar dislocation of the metacarpophalangeal (MCP) joint of the thumb is an exceptionally rare injury due to the joint's inherent stability provided by the volar plate, collateral ligaments, and dorsal capsule. Chronic or neglected dislocations are even more uncommon and pose considerable surgical challenges because of fibrosis, soft tissue contracture, and adaptive bony changes. Case presentation We report the case of a 12-year-old boy who presented with a nine-year history of progressive deformity and restricted motion of the left thumb following blunt trauma with a wooden stick. Radiographs revealed a neglected volar dislocation of the thumb MCP joint with dorsal projection of the metacarpal head. Open reduction via a dorsal approach was performed under loupe magnification. Interposed fibrotic tissue and contracted structures were released, the extensor tendons were realigned, and ligamentous stability was restored. A uniplanar external fixator was applied to maintain joint congruity, followed by physiotherapy after fixator removal. Results At six months postoperatively, the patient achieved full correction of deformity, restoration of joint stability, and significant improvement in thumb motion, enabling normal daily activity. Conclusion Volar dislocation of the thumb MCP joint is extremely rare, particularly in pediatric patients. Early recognition and meticulous surgical management with soft tissue release, ligament repair, and stable fixation can yield excellent functional recovery even in long-standing cases. This report adds to the limited literature on chronic volar MCP dislocations and underscores the value of timely surgical intervention and structured rehabilitation.
Introduction L’otoplastie est une intervention fréquemment réalisée en chirurgie plastique. Une contention postopératoire est souvent préconisée par le chirurgien. Celle-ci n’est pas dénuée de risques graves. Nous présentons le cas d’une escarre après otoplastie en lien avec une contention inadaptée. Cas Une fille de 7 ans a consulté pour une cure d’oreilles décollées. L’intervention s’est déroulée sans complications. Le lendemain de l’intervention il n’a pas été possible de vérifier la contention que nous lui avions prescrite lors de la consultation préopératoire. Elle est rentrée à son domicile avec un pansement compressif sur-mesure. Il lui a été donné pour consigne de le relayer par le bandeau de contention recommandé dès le lendemain. À la consultation de contrôle à J10 postopératoire elle présentait une escarre de stade 4 du pavillon de l’oreille externe gauche en regard de l’anthélix. La contention mise en place n’était pas celle que nous avions préconisée. Nous avons réhospitalisé notre patiente. La contention a immédiatement été levée. Des soins locaux quotidiens lui ont été prodigués avec parage de la fibrine sous MEOPA, application de pansements gras et antibiothérapie prophylactique. Les suites ont été favorables lui permettant de regagner son domicile quelques jours plus tard. Un suivi rapproché en consultation a été mis en place. 3 mois plus tard elle ne présentait aucune séquelle esthétique. Discussion La nécrose cutanée après otoplastie liée à une contention inadaptée est une complication rare et peu décrite dans la littérature. Sa gravité ne doit pas être sous-estimée. Bien que plébiscité par de nombreux chirurgiens, la contention postopératoire après otoplastie ou toute autre intervention en chirurgie plastique n’a pas fait la preuve de son utilité dans la littérature. Sa prescription doit faire l’objet d’une grande prudence.
BACKGROUND:While typical Necrotizing Soft Tissue Infections (NSTI) are clinically overt, atypical and subacute presentations pose a complex diagnostic challenge. These forms-characterized by subacute evolution, deceptive local signs, or atypical pathogens-often defy standard recognition patterns. The Laboratory Risk Indicator for Necrotizing Fasciitis (LRINEC) score is frequently utilized for emergency risk stratification; however, its sensitivity remains contested in contemporary literature. METHODS:We conducted a retrospective observational study (2007-2025) of 37 confirmed NSTI patients at a regional tertiary Plastic Surgery referral center. We analyzed admission physiology, microbiological profiles, and calculated retrospective LRINEC and SIARI (Site, Immuno, Age, Renal, Inflam) scores. To validate findings against potential data handling bias, a sensitivity analysis comparing zero-imputation versus complete case analysis was performed. RESULTS:Despite high clinical severity (43.2% septic shock), the LRINEC score yielded a false negative rate of 18.9% (7/37 patients classified as "Low Risk"). These failures correlated with atypical profiles: head and neck localization, prior administration of non-steroidal anti-inflammatory drugs (NSAID), and subacute pathogens (Cutibacterium acnes, Actinomyces). Sensitivity analysis confirmed the statistical stability of these findings (P=0.66). Conversely, the SIARI score correctly reclassified all LRINEC false negative cases as high risk (Score ≥ 3). CONCLUSION:The LRINEC score exhibits insufficient negative predictive value to safely exclude atypical and subacute NSTIs, potentially causing hazardous delays in surgical intervention. We propose a stratified clinical algorithm designed to enhance surgical vigilance and reduce the risk of missed diagnosis.
Introduction Les techniques classiques de reconstruction du modiolus proposent une réparation secondaire par séparation de l’orbiculaire. Nous proposons ici une reconstruction primaire par remise en continuité de la sangle orbiculaire, au prix d’une microstomie temporaire. Méthodes La cas présenté est un patient de 70 ans victime d’une morsure animale entraînant une perte de substance de la joue droite et de la commissure labiale, avec atteinte du modiolus. La prise en charge a comporté deux temps opératoires. Le premier temps a consisté à rétablir la continuité de la sangle orbiculaire par suture directe des muscles sectionnés, combinée à une reconstruction par lambeau sous-mental. Le second temps, réalisé 4 mois plus tard, a consisté en une commissuroplastie par traction externe pour repositionner la commissure en position physiologique. Résultats Les résultats fonctionnels et esthétiques ont été évalués après chaque temps opératoire. Dès le premier temps, une continuité de la sangle orbiculaire a été rétablie. Après un deuxième temps de commissuroplastie, le patient a récupéré une continence labiale et une mobilité satisfaisante. Les temps ultérieurs de dégraissage ont amélioré l’aspect esthétique. Discussion Cette technique met en avant l’importance de la reconstruction primaire de la sangle orbiculaire pour restaurer la fonctionnalité du modiolus. Le lambeau sous-mental a permis une restauration de la pilosité en plus de la reconstruction cutanée et du lining interne. Cette approche, bien que décrite pour un cas traumatique, pourrait s’étendre à des indications carcinologiques.
Introduction The supraclavicular artery island flap, initially described by Lamberty and Cormack in 1979 and subsequently refined by Pallua, is now a well-established option in cervicofacial reconstruction. Owing to its thin, pliable, and well-vascularized skin, it offers favorable color and texture match for facial and intraoral defects while limiting donor-site morbidity. Technical note This technical note describes the design, elevation, tunneling, and inset of the supraclavicular artery island flap for cervicofacial and intraoral reconstruction. Emphasis is placed on key anatomical landmarks, dissection planes, and technical precautions to ensure pedicle preservation and flap reliability. The technique is illustrated by a representative clinical case, highlighting practical aspects of flap planning, double-paddle design, and indications relative to other reconstructive options based on current literature. Conclusion The supraclavicular artery flap represents a valuable addition to the reconstructive armamentarium for maxillofacial surgeons. It offers a reliable, efficient, and aesthetically favorable option for complex cervicofacial defects, particularly in resource-limited settings.
Introduction La reconstruction des pertes de substance du canthus interne étendues aux deux paupières est un défi chirurgical rare et complexe. Les objectifs sont fonctionnels (mobilité, protection de la cornée) et esthétiques (éviter l’effet patch, recréer la concavité du canthus). Méthode Nous proposons, à travers un cas clinique, un procédé de reconstruction en quatre étapes : 1) canthopéxie temporaire : fixation des moignons de tarse au cadre orbitaire pour protéger la cornée en attendant les résultats histologiques ; 2) greffe de muqueuse buccale et lambeau frontal : une greffe de muqueuse buccale reconstitue la lamelle postérieure, tandis qu’un lambeau frontal paramédian sert de support vasculaire (« lambeau porte-greffe ») ; 3) sevrage du lambeau frontal et greffe cutanée : après 3–4 semaines, le lambeau est divisé, la couche cutanéo-graisseuse est remplacée par une greffe de peau rétro-auriculaire ; 4) division de la reconstruction et second greffon muqueux : une greffe de muqueuse buccale fine couvre les berges de l’incision pour restaurer la symétrie des fentes palpébrales. Résultats À 6 mois, le patient est satisfait. Fonctionnellement, il n’y a ni lagophtalmie ni irritation cornéenne. Esthétiquement, la colorimétrie est adaptée et la concavité du canthus est restaurée. Discussion Le lambeau frontal et la muqueuse buccale offrent une couverture adéquate, évitant certaines complications comme l’entropion. Conclusion Cette approche en plusieurs temps, combinant lambeau frontal et greffes muqueuses, nous semble être une solution efficace pour les pertes de substance du canthus interne étendues aux paupières.
Objectif Évaluer la précision et la stabilité à long terme de la correction chirurgicale guidée des asymétries maxillomandibulaire à l’aide d’une planification numérique 3D associée à des guides sur-mesure. Matériels et méthodes Étude rétrospective monocentrique portant sur dix patientes présentant une asymétrie maxillomandibulaire, traitées entre 2019 et 2023 par un protocole combinant préparation orthodontique, planification chirurgicale 3D et chirurgie guidée. La précision a été évaluée en comparant les résultats postopératoires à un an au plan virtuel, à partir de sept points de repère standardisés. Le suivi clinique et radiologique a été poursuivi sur cinq ans. Résultats L’écart moyen entre la planification et les résultats postopératoires était de 0,79mm pour le maxillaire et 1,33mm pour la mandibule. Aucun cas de récidive n’a été observé après cinq ans. La technique a permis une réduction des ajustements peropératoires et une optimisation de la stabilité occlusale. Conclusion La planification numérique 3D associée à des guides personnalisés constitue une approche fiable et précise pour la correction des asymétries faciales complexes, offrant des résultats stables à long terme et une morbidité réduite. Des études multicentriques avec un suivi prolongé sont nécessaires pour confirmer ces données.
Introduction La chirurgie du sein, quel que soit le type d’intervention, exige l’obtention d’un résultat symétrique. Or, à ce jour, ce résultat est dépendant de l’expérience de l’opérateur et soumis à sa subjectivité. Notre étude a pour but d’étudier la corrélation entre le poids de résection de glande mammaire et la variation de volume mesurée par une caméra 3D portative, ajustée sur la densité mammaire mesurée électroniquement. Matériels et méthodes Trente patientes prises en charge pour une réduction mammaire bilatérale ont été inclues dans l’étude. Chaque patiente bénéficiait d’une mesure du volume mammaire par caméra 3D (GOPRO de Créaform) lors de la consultation préopératoire et à 4 mois postopératoire. Pour chacune d’entre elle, une mesure de densité mammaire par MyotonPro était réalisée. Résultats L’analyse de la corrélation entre le poids de résection et la variation de volume pré- et postopératoire met en évidence un coefficient de détermination r2 à 0,822 (95 % IC : 0,713–0,892). L’ajustement sur les paramètres mesurés numériquement par le MyotonPro ne semble pas influencer la corrélation. Notre modèle met en évidence pour un poids≥500g, une variation de volume de 666mL avec une sensibilité de 85,3 % et une spécificité de 86,4 %. Conclusion Il existe une forte corrélation entre le poids de résection et la variation de volume mesurée par caméra 3D, sans influence de la densité mammaire. L’usage de la caméra 3D en pratique courante permettrait au chirurgien plasticien de mieux prévoir chaque chirurgie et d’optimiser nos résultats.
Introduction Les personnes transgenres reportent de nos jours une prise en charge médicale peu optimale, voire discriminante dans certains cas, tandis que les professionnels de santé se déclarent peu préparés à prendre en charge correctement ces patients. L’objectif de cette étude était d’évaluer l’apport de connaissances sur la transidentité aux étudiants en médecine en 4e année lors d’un enseignement optionnel. Matériel et méthodes Un enseignement optionnel de 20h était proposé aux étudiants de 4e année de médecine. Les aspects médicaux et juridiques de la transidentité était abordés par différents professionnels. Un questionnaire d’évaluation des connaissances étaient remplis par les étudiants (n=90 au total) avant le début de l’enseignement et répété à la fin de l’enseignement. Résultats Les connaissances des étudiants à propos de la prise en charge des patients transgenres ont significativement augmenté entre avant et après l’enseignement. Les étudiants ont également exprimé leur satisfaction d’avoir participé à cet enseignement optionnel. Discussions Quatre demi-journées d’enseignement optionnel ont permis aux étudiants en médecine d’acquérir des bases sur la prise en charge des patients transgenres. Les inégalités auxquelles fait face cette population en termes de santé mentale, physique et d’accès aux soins nous obligent à réfléchir sur la formation des professionnels de santé. L’intégration d’un enseignement lors des études médicales pourrait permettre une régression de ces inégalités de prise en charge.
BACKGROUND:Phytophotodermatitis is a rare and poorly recognized cause of chemical burns. It consists of a non-immunologic cutaneous reaction caused by exposure to plants containing photosensitizing substances, followed by ultraviolet (UV) radiation exposure. The clinical manifestations resemble those of second-degree burns. Numerous differential diagnoses are possible. Diagnosis is based on a detailed history, highlighting the characteristic temporal sequence of contact with a phototoxic substance followed by UV exposure. Treatment is mainly symptomatic. OBJECTIVES:This study aims to report five cases of phytophotodermatitis and to provide an overview of its diagnosis and management. METHODS:We report five cases managed in the Plastic Surgery Department at Montpellier University Hospital between April 2015 and May 2024. Data collected included demographic characteristics, time to diagnosis, aetiology, duration of management and healing, potential need for surgical intervention, and long-term complications. RESULTS:The implicated plant species belonged to the families Apiaceae (2 cases), Moraceae (1 case), and Rutaceae (2 cases). The mean time to symptom onset was 28.8h. Cutaneous manifestations included erythema (5 cases), vesicles (2 cases), and bullae (3 cases). All patients received immediate irrigation of the affected areas with water, followed by topical corticosteroid therapy. None of the patients required surgical management to achieve re-epithelialization. Post-inflammatory hyperpigmentation persisted as a sequela in one case. CONCLUSIONS:A thorough history is essential to identify the characteristic temporal sequence and to initiate treatment promptly. Management primarily relies on analgesics and topical corticosteroids dressings, with rapid healing in most cases. Long-term sequelae may include persistent cutaneous hyperpigmentation for several years.
Contexte L’amibiase cutanée primaire est une manifestation rare de l’infection à Entamoeba histolytica, de diagnostic souvent méconnu en raison de sa rareté. Objectif Rapporter un nouveau cas et analyser les données épidémiologiques, cliniques et diagnostiques à travers une revue de la littérature. Cas clinique Une patiente de 23 ans a présenté une lésion cutanée de la cuisse gauche. Le diagnostic d’amibiase cutanée primaire a été confirmé par l’examen histologique. Conclusion Devant toute lésion cutanée atypique, notamment sur une personne vivant en zone endémique, l’amibiase doit être évoquée. L’histologie demeure l’examen clé pour poser le diagnostic de cette forme rare, permettant ainsi une prise en charge adaptée.
