    2Ultrasound Localization Microscopy
    Stefanie Dencks,Georg Schmitz

Ultrasound Localization Microscopy (ULM) is an emerging technique that provides impressive super-resolved images of microvasculature, i.e., images with much better resolution than the conventional diffraction-limited ultrasound techniques and is already taking its first steps from preclinical to clinical applications. In comparison to the established perfusion or flow measurement methods, namely contrast-enhanced ultrasound (CEUS) and Doppler techniques, ULM allows imaging and flow measurements even down to the capillary level. As ULM can be realized as a post-processing method, conventional ultrasound systems can be used for. ULM relies on the localization of single microbubbles (MB) of commercial, clinically approved contrast agents. In general, these very small and strong scatterers with typical radii of 1-3 µm are imaged much larger in ultrasound images than they actually are due to the point spread function of the imaging system. However, by applying appropriate methods, these MBs can be localized with sub-pixel precision. Then, by tracking MBs over successive frames of image sequences, not only the morphology of vascular trees but also functional information such as flow velocities or directions can be obtained and visualized. In addition, quantitative parameters can be derived to describe pathological and physiological changes in the microvasculature. In this review, the general concept of ULM and conditions for its applicability to microvessel imaging are explained. Based on this, various aspects of the different processing steps for a concrete implementation are discussed. The trade-off between complete reconstruction of the microvasculature and the necessary measurement time as well as the implementation in 3D are reviewed in more detail, as they are the focus of current research. Through an overview of potential or already realized preclinical and clinical applications - pathologic angiogenesis or degeneration of vessels, physiological angiogenesis, or the general understanding of organ or tissue function - the great potential of ULM is demonstrated.

3Use of Dose-Area Product to Assess Plan Quality in Robotic Radiosurgery
    Markus Eichner,Alexandra Hellerbach,Mauritius Hoevels,Klaus Luyken,Michael Judge,Daniel Ruess,Maximilian Ruge,Martin Kocher,Stefan Hunsche,Harald Treuer

Purpose: In robotic stereotactic radiosurgery (SRS), optimal selection of collimators from a set of fixed cones must be determined manually by trial and error. A unique and uniformly scaled metric to characterize plan quality could help identify Pareto-efficient treatment plans. Methods: The concept of dose-area product (DAP) was used to define a measure (DAP(ratio)) of the targeting efficiency of a set of beams by relating the integral DAP of the beams to the mean dose achieved in the target volume. In a retrospective study of five clinical cases of brain metastases with representative target volumes (range: 0.5-5.68 ml) and 121 treatment plans with all possible collimator choices, the DAP(ratio) was determined along with other plan metrics (conformity index CI, gradient index R50%, treatment time, total number of monitor units TotalMU, radiotoxicity index f12, and energy efficiency index eta 50%), and the respective Spearman's rank correlation coefficients were calculated. The ability of DAP(ratio) to determine Pareto efficiency for collimator selection at DAP(ratio) < 1 and DAP(ratio) < 0.9 was tested using scatter plots. Results: The DAP(ratio) for all plans was on average 0.95 +/- 0.13 (range: 0.61-1.31). Only the variance of the DAP(ratio) was strongly dependent on the number of collimators. For each target, there was a strong or very strong correlation of DAP(ratio) with all other metrics of plan quality. Only for R50% and eta 50% was there a moderate correlation with DAP(ratio) for the plans of all targets combined, as R50% and eta 50% strongly depended on target size. Optimal treatment plans with CI, R50%, f12, and eta 50% close to 1 were clearly associated with DAP(ratio) < 1, and plans with DAP(ratio) < 0.9 were even superior, but at the cost of longer treatment times and higher total monitor units. Conclusions: The newly defined DAP(ratio) has been demonstrated to be a metric that characterizes the target efficiency of a set of beams in robotic SRS in one single and uniformly scaled number. A DAP(ratio) < 1 indicates Pareto efficiency. The trade-off between plan quality on the one hand and short treatment time or low total monitor units on the other hand is also represented by DAP(ratio).

4Development and Clinical Implementation of a Digital System for Risk Assessments for Radiation Therapy.

Before introducing new treatment techniques, an investigation of hazards due to unintentional radiation exposures is a reasonable activity for proactively increasing patient safety. As dedicated software is scarce, we developed a tool for risk assessment to design a quality management program based on best practice methods, i.e., process mapping, failure modes and effects analysis and fault tree analysis. Implemented as a web database application, a single dataset was used to describe the treatment process and its failure modes. The design of the system and dataset allowed failure modes to be represented both visually as fault trees and in a tabular form. Following the commissioning of the software for our department, previously conducted risk assessments were migrated to the new system after being fully re-assessed which revealed a shift in risk priorities. Furthermore, a weighting factor was investigated to bring risk levels of the migrated assessments into perspective. The compensation did not affect high priorities but did re-prioritize in the midrange of the ranking. We conclude that the tool is suitable to conduct multiple risk assessments and concomitantly keep track of the overall quality management activities.

5A Body Mass Index-Based Method for “Mr-Only” Abdominal MR-guided Adaptive Radiotherapy

Purpose: Dose calculation for MR-guided radiotherapy (MRgRT) at the 0.35 T MR-Linac is currently based on deformation of planning CTs (defCT) acquired for each patient. We present a simple and robust bulk density overwrite synthetic CT (sCT) method for abdominal treatments in order to streamline clinical workflows. Method: Fifty-six abdominal patient treatment plans were retrospectively evaluated. All patients had been treated at the MR-Linac using MR datasets for treatment planning and plan adaption and defCT for dose calculation. Bulk density CTs (4M-sCT) were generated from MR images with four material compartments (bone, lung, air, soft tissue). The relative electron densities (RED) for bone and lung were extracted from contoured CT structure average REDs. For soft tissue, a correlation between BMI and RED was evaluated. Dose was recalculated on 4M-sCT and compared to dose distributions on defCTs assessing dose differences in the PTV and organs at risk (OAR). Results: Mean RED of bone was 1.17 f 0.02, mean RED of lung 0.17 f 0.05. The correlation between BMI and RED for soft tissue was statistically significant (p < 0.01). PTV dose differences between 4M-sCT and defCT were D-mean: -0.4 f 1.0%, D-1%: -0.3 f 1.1% and D-95%: -0.5 f 1.0%. OARs showed D-2%: -0.3 f 1.9% and D-mean: -0.1 f 1.4% differences. Local 3D gamma index pass rates (2%/2mm) between dose calculated using 4M-sCT and defCT were 96.8 f 2.6% (range 89.9-99.6%). Conclusion: The presented method for sCT generation enables precise dose calculation for MR-only abdominal MRgRT.
