To investigate injury-related specialized health care utilization and costs among primary health care patients participating in a multifactorial falls prevention programme compared with age-, sex-, and residence-matched population-based controls. Participants in the multifactorial falls prevention programme had higher injury-related specialized health care utilization and were more likely to incur injury-related costs than matched controls. Among participants with injury-related specialized health care utilization, costs per person-year were not significantly higher than among controls. Participants in the multifactorial falls prevention programme represented a clinically selected high-risk population with substantially higher injury-related specialized health care utilization than matched controls. Although they were more likely to incur injury-related specialized health care costs, costs among service users were not significantly higher than among controls. Our study aims to retrospectively investigate injury-related specialized health care utilization and costs among primary health care patients participating in a multifactorial falls prevention program (MFFP) compared with sex-, age-, and residence-matched population-based controls. This is a Finnish single-center observational, retrospective, register-based controlled cohort study including 527 community-dwelling primary health care MFFP patients and their 3:1 age-, sex-, and residence-matched population-based controls (n = 1581), who had not participated in the MFFP. Patients’ injury-related specialized health care utilization was followed from their first MFFP visit until death or the end of December 2019. MFFP patients had more injury-related outpatient visits [IRR 1.76 (95
To identify ways to assess functional trajectories in old survivors of intensive care. Frailty and functional independence have been found to exhibit a dynamic course after a critical illness. Although the concept of intrinsic capacity could provide a holistic framework to predict functional impairments, there still is only limited data on its usefulness in the context of a critical illness. Multi-dimensional measures related to the concepts of frailty, functional independence and intrinsic capacity should be used to capture the main aspects of a survivor’s recovery status. Patients with critical conditions admitted to an intensive care unit (ICU) undergo one of the most extensive trials of resilience. In addition to considering survival as a crucial outcome, there is a growing awareness of the importance of recovering functional abilities in old ICU patients. In this narrative review, a group of geriatricians and intensivists will discuss previous studies and current approaches to assess functional trajectories in old ICU survivors with a focus on frailty, functional independence and intrinsic capacity. In previous studies, frailty as evaluated with the Clinical Frailty Scale or variants of the Frailty Index has been found to exhibit a dynamic course after a critical illness. Measures of functional independence show a similar pattern after ICU discharge depending on pre-existing impairments and frailty. Although the concept of intrinsic capacity could provide a holistic framework to predict functional impairments, there still is only limited data on its usefulness in the context of a critical illness. Assessing the post-discharge trajectory in old survivors of a critical illness is crucial to detect early signs of complications and adjust individual support. However, current approaches in this field lack validation. Considering the challenges and importance of functional recovery in old ICU patients, more intensive research in the future is imperative in order to determine the best approach to describe, monitor and optimise the health status amongst these individuals.
This network meta‑analysis sought to compare the effects of different types of exercise interventions with varied duration, intensity, frequency and period parameters on sleep quality in older adults so as to provide evidence for developing scientific exercise prescriptions. Combined exercise yielded the best effect for improving sleep quality among older adults, followed byaerobic exercise and resistance exercise. The efficacy of different exercise types was not static and could change with intervention parameters and participant characteristics, and these preliminary results need further high-quality research for validation. Exercise benefits sleep quality in older adults, yet the effects of different exercise intervention types across various parameters (duration, intensity, frequency, and period) remain unclear. This network meta-analysis aimed to compare the effects of different intervention types across these parameters on sleep quality in older adults, providing an evidence-based basis for formulating scientific exercise prescriptions. Relevant randomized controlled trials were retrieved from five databases (including Web of Science) up to November 15, 2025. Two researchers independently extracted data. Study quality was assessed using RevMan, and network meta-analysis was conducted via Stata. Publication bias was examined with funnel plots and Egger's test, intervention effects were ranked by SUCRA, and leave-one-out sensitivity analysis was used to verify result robustness. A total of 32 randomized controlled trials involving 2,658 participants were included. Network meta-analysis showed that combined exercise (SUCRA=74.8
To describe nationwide hospital care use among centenarians in France between 2014 and 2023, including temporal and regional variation, emergency admission, surgical care, and inpatient rehabilitation. Acute care hospitalization burden decreased among centenarians, while the proportion of stays following emergency department admission increased. Musculoskeletal conditions represented a growing proportion of hospitalizations, inpatient rehabilitation remained frequently used, and marked regional variation in hospitalization burden was observed. These nationwide descriptive findings provide information on the changing patterns of hospital care use among centenarians and may help inform health-system planning as this population continues to grow. To describe nationwide hospital care use among centenarians in France, including temporal and regional variation, emergency admission, surgical care, and inpatient rehabilitation. We conducted a retrospective nationwide observational study using French hospital discharge data on centenarian hospital stays from 2014 to 2023. Over the study period, 110,585 acute care hospital stays were recorded among centenarians, of which 12.1
To compare different absolute, relative, and duration-based definitions of intraoperative hypotension in older adults undergoing hip fracture surgery and to evaluate their associations with postoperative acute kidney injury and 30-day mortality. Absolute and relative intraoperative hypotension thresholds were not significantly associated with postoperative outcomes. In contrast, longer duration of systolic blood pressure below 100 mmHg was independently associated with higher 30-day mortality, while higher ASA classification remained the main predictor of postoperative acute kidney injury. In older adults with hip fracture, the duration of intraoperative hypotension appears more clinically relevant than static blood pressure thresholds, supporting the need for geriatric-specific definitions and proactive prevention of prolonged hypotension. Intraoperative hypotension (IOH) is associated with adverse outcomes, but its definition varies widely and most definitions are derived from younger or mixed-age populations. Older adults undergoing hip fracture surgery are particularly susceptible due to reduced physiological reserve. This study compared absolute, relative, and duration-based definitions of IOH and evaluated their associations with postoperative adverse outcomes. This retrospective single-center cohort study included all patients aged ≥ 65 years who underwent hip fracture surgery at Maastricht University Medical Centre+ between January 2021 and December 2022. IOH was defined using absolute thresholds (MAP < 65 mmHg, SBP < 100 mmHg), relative reductions (≥ 20
