1Chasing the Wild Duck: Ethical Considerations and Repercussions of Psychiatric Diagnosis
Sabine Færge, Alexander Tobias Ysbæk-Nielsen, Anton Julius von Hofacker, Helene Speyer

Levinovitz and Aftab’s (2025) formulation of the Rumpelstiltskin effect captures a familiar clini- cal phenomenon in which receiving a diagnosis in itself can be therapeutic. This paper analyzes the structure and ethical implications of this effect in psychiatric diagnosis. We suggest that this effect may be amplified by what Gilbert Ryle termed a category mistake, in which descriptive diagnostic categories are mistaken for underlying monocausal neurobiological essences. This misinterpretation increases the perceived explanatory power of a diagnosis and may contribute to its thera- peutic effect. If so, the relief afforded by diagnosis may depend on what Henrik Ibsen described as a “life-lie,” a stabilizing but potentially misleading understanding of one’s condition. We term this the “wild duck dilemma”: an ethical situation in which correcting a potentially misleading explanatory belief could undermine relief or coherent self-understanding, while leaving it unchal- lenged may fix the person within a limiting framework and inflict a loss of trust in psychiatry if the life-lie is later revealed. To show how explanatory beliefs may limit patients, we draw on Heideggerian existential hermeneutics and argue that psychiatric diagnosis is not merely descriptive but world-disclosing: it orga- nizes how a person understands their past, present, and future possibilities. Heidegger’s account of solicitude is presented as a conceptual lens to articulate the dilemma by offering orientation and understanding without fore- closing the patient’s possibilities for self-interpretation. Finally, we show how associated debates in the ethics of disclosure and deontological versus utilitarian per- spectives may clarify the moral tensions and practical implications of the wild duck dilemma.

2Rethinking Muscle Dysmorphia: Autonomy, Normativity, and Epistemic Injustice
    Matteo Cresti

Muscle dysmorphia is currently defined in the Diagnostic and Statistical Manual of Mental Disorders, 5th edition, as a specifier of Body Dysmorphic Disorder and characterized by preoccupation with insufficient muscularity, repetitive appearance-related behaviors, and significant distress or impairment. However, many of the behaviors treated as symptoms are also central to bodybuilding as a normative practice. This raises a conceptual difficulty: distinguishing pathology from intentional, value-guided participation in a culturally specific discipline. Drawing on Fricker’s notion of epistemic injustice, this paper argues that the current diagnostic understandings of muscle dysmorphia risk misinterpreting the experiences and commitments of bodybuilders. Testimonial injustice arises when practitioners’ self-reports are discounted due to stereotypes concerning narcissism, insecurity, or poor insight. Hermeneutical injustice emerges when psychiatric frameworks lack the conceptual resources to recognize the normative structure of bodybuilding, thereby rendering its practices intelligible only as manifestations of a disorder. The paper further develops a conceptual analogy with the diagnostic history of gender dysphoria. Just as earlier clinical models misclassified transgender self-understanding by imposing external norms of gender, so the framing of muscle dysmorphia relies on culturally dominant assumptions about bodily appearance and a “balanced” lifestyle. The shift from “transition” to “affirmation” in gender discourse highlights how bodily modification can be understood as a legitimate form of self-definition rather than pathology. The paper concludes by outlining how a more epistemically just orientation to diagnosis could distinguish pathological distress from normative commitments in bodybuilding.

3Pathology or Protest? A Philosophical Analysis of Post-Traumatic Embitterment Disorder
    Leah Ritterfeld

Post-traumatic embitterment disorder (PTED) is a proposed diagnostic construct describing chronic and impairing embitterment following events experienced as unjust. This article offers a philosophical analysis of PTED, focusing on the normative assumptions built into its etiological and diagnostic framework. I argue that PTED’s etiological core—framed as a mismatch between ‘basic beliefs’ and a critical event—is better understood as the violation of normative expectations. This matters because the persistence that PTED theory treats as pathological rigidity may, in some cases, express fidelity to a warranted moral claim. I show that PTED’s exclusively symptom-focused framework risks flattening importantly different forms of embitterment and thereby pathologizing certain forms of justified, appropriate, or rational moral protest too quickly. I argue that PTED theory therefore stands to benefit from a supplementary normative framework around expectation warrant, appraisal accuracy, and the form and persistence of the emotional response.

4Nishida Kitaro’s Philosophy As a Non-Religious Foundation for Mindfulness Interventions: Self-Insight Meditation Therapy As Clinical Operationalization
Yoshinobu Fujita, Tomohiko Muratsubaki, Kenjiro Ohta

Contemporary mindfulness interventions derive core techniques from Buddhist contemplative traditions yet lack robust non-religious philosophical foundations. This paper proposes that Nishida Kitaro’s philosophy offers such a foundation. His concept of topos (basho) provides a rigorous ontology of self–world relations, where the self and the world co-arise within a unified, self-determining field. His concept of action–intuition (koiteki chokkan) articulates how contemplative insight manifests through engaged action rather than remaining as abstract understanding. We examined Self-Insight Meditation Therapy, developed in Japan, as a clinical operationalization of these concepts, emphasizing interpersonal situations as primary practice sites and the cultivation of active evaluative capacity. This framework converges with contemporary enac- tive approaches to psychiatry, a convergence rooted in shared philosophical sources rather than coincidental resemblance. As practice deepens, Self-Insight Meditation Therapy practitioners come to recognize that aware selves are plural focal points whose perspectives remain irreducible to one another, and that their prac- tice participates in world-formation within Nishida’s historical world (rekishiteki sekai). Ethical neutrality in contemplative practice is thereby shown to be ontologically impossible rather than merely morally undesirable.

5Using Wakefield's Harmful Dysfunction Analysis to Distinguish Different Types of Evolutionary Mismatch
Benjamin Griffin, Cos Savva, Riadh Abed

Abstract: Evolutionary mismatch is a versatile concept used across fields, denoting that our evolved biology may be poorly suited to aspects of modern environments. In evolutionary psychiatry, it has become a key explanatory principle for the prevalence of psychiatric conditions in modern populations. However, some authors regard the concept as vague and in need of philosophical clarification. We introduce a novel distinction derived from Wakefield's harmful dysfunction analysis: dysfunctional trait evolutionary mismatch, representing true disorders, and functional trait evolutionary mismatch, which do not. We further identify the possibilities of latent dysfunctional mismatch, where previously harmless dysfunctions become harmful in modern contexts, and attenuated-harmful dysfunction, where previous harm has been neutralized by modern environmental change. Recognizing these distinctions can improve disorder classification, and may provide the basis for improved prevention and treatment.
