1Beyond the Dashboard: Teaching Value-Based Care Through the Eyes of the Patient
    Mark D. Olszyk

Value-based care challenges physician leaders to move beyond dashboards and help clinical teams understand what performance measures mean for patients’ lives. Common hospital metrics — mortality, length of stay, readmissions, complications, and patient experience — remain essential, but they are only useful when connected to outcomes patients and families value. Using Porter’s Outcomes Hierarchy, this article offers physician leaders a practical framework for teaching value in terms of survival, recovery, treatment burden, sustainability of health, and long-term quality of life. The framework reframes familiar measures as signals rather than endpoints: Length of stay becomes part of time to recovery, readmission reflects the sustainability of the care plan, and complications represent the burden imposed during treatment. For chief medical officers and other physician executives, this perspective strengthens quality improvement, aligns clinical and financial goals, and reconnects measurement with professional purpose. By teaching teams to view metrics through the eyes of the patient, physician leaders can make value-based care more clinically meaningful, ethically grounded, and actionable.

2From Fragmented Intake to Reliable Access: Standardizing Pediatric Cardiology Second Opinions
    Steven Jumps, Mark McDonald

Second opinions play a critical role in pediatric specialty care, particularly in complex conditions where diagnostic uncertainty, treatment variation, or parental concern may delay care. A pediatric cardiac center implemented a program designed to streamline intake, standardize triage, and reduce time to clinical evaluation for patients and families seeking additional expertise. Prior to implementation, second opinion requests were managed through fragmented and inconsistent workflows, limiting the ability to offer timely access or measure performance. A multidisciplinary team was established to centralize intake, expedite scheduling, and track outcomes. Performance was monitored using defined access and retention metrics. The program demonstrated sustained reductions in response and wait times, improved appointment availability within targeted access thresholds, and high rates of patient retention for ongoing cardiac care. These results suggest that formalizing second opinion workflows can improve access, enhance patient experience, and support broader organizational value without increasing clinical capacity or staffing.

3Let’s Get a Management Consultant! Six Common Situations to Get You Thinking
    AAPL Editorial

Healthcare practices sometimes face financial, operational, or strategic challenges that exceed the expertise of physicians and in-house managers. This article outlines six situations that may signal the need for a management consultant, including declining profitability, stalled performance, major projects, overloaded administrators, and sensitive physician-to-physician issues. It also offers guidance for making consulting engagements more effective through reference checks, clear preplanning, cooperation, and a focus on practical, actionable results. Ultimately, successful consulting depends on clear communication, careful implementation, and an honest assessment of whether outside expertise is more cost-effective than strengthening internal management capacity.

4The Feedback Protocol: Operationalizing Psychological Safety
    Bibhas Singla

This discussion article argues that psychological safety, while important, is not sufficient unless healthcare organizations can convert frontline concern into investigation, action, and feedback within two weeks. The central operational problem is not only fear-based silence, but the repeated failure of reporting pathways to produce visible consequences. The article examines how delayed review, anonymous reporting, and weak feedback loops erode trust and learning capacity. The author proposes a more practical leadership response centered on structured feedback processes, faster signal handling, and cross-level accountability. Introduced as the Feedback Protocol, it is important because patient safety and leadership accountability require systems that can move frontline observations into timely action.

5Self-Care for Physician Leaders: More Than a Nice Idea
    Brian A. Hicks, Robert Hicks

Physician leaders face intense professional demands that can lead to burnout, compassion fatigue, and diminished well-being, impacting leadership effectiveness and organizational outcomes. This article emphasizes self-care as a critical leadership discipline, not merely a personal wellness choice. Evidence-based strategies — mindfulness, self-compassion, constructive self-talk, physical wellness, healthy boundaries, and meaningful connections — are explored as proactive approaches to sustain resilience and professional effectiveness. By integrating self-care into daily routines, physician leaders enhance their ability to lead, support teams, and maintain high-quality care, ensuring both personal and organizational success.
