Objective To describe and compare the status of knowledge, attitude and screening practice (KAP) of cervical cancer among women between six urban and rural areas of China, and explore the influencing factors. Methods A total of 599 women were recruited by multistage stratified random sampling method in eastern, central and western regions of China. A self-designed questionnaire survey was conducted to estimate their KAP about cervical cancer. Results The awareness rate of cervical cancer among urban women was 16. 4%, which was higher than that in rural area(8. 0%) (P < 0. 05). The positive rate of attitude about urban women's cervical cancer prevention and treatment(72. 5%) was higher than that of rural women (70. 1%). However, the participation rate of cervical cancer screening among urban women (62. 8%) was lower than that among rural women (67. 4%) (P > 0. 05). The influencing factors for cervical cancer screening practice were education, provinces and attitude among urban women. However, the ethnicity, occupation, provinces and attitude could influence the screening practice among rural women. Conclusion Although the fact that women in urban and rural areas had very low level of knowledge about cervical cancer prevention and treatment, their attitude was more active. But their behavior of participating in screening practice still needs to be improved. The influencing factors of screening practice between urban and rural women were different. In the future implementation of the project, we should make different strategies based on urban-rural differences, so that both urban and rural women can obtain information in time, establish a positive attitude towards prevention and treatment, and take an active part in cervical cancer screening program.
Objective To compare the status of knowledge, attitudes and practice (KAP) related tobreast cancer among women in urban and rural areas, and explore the difference of influencing factors between urban and rural areas. Methods599 women were recruited through multistage stratified random sampling method. The differences among status, influencing factors of KAP related to breast cancer between urban and rural areas were analyzed. Results The rate of knowledge of breast cancer in urban (26. 2%) was higher thanthat in rural area (13. 3%) (P < 0. 05). The influencing factors of knowledge among urban women were nation (non-Han: OR = 0. 042),educational background (Junior College or higher: OR = 3. 826) and occupation (business and worker: OR = 7. 402),while educational background (high school: OR = 3.427,Junior College or higher: OR = 24. 975) was the only influencing factor in rural areas. The occupation (officer: OR= 20. 453) and province (Jiangxi: OR = 0. 373) were the influencing factors of breast cancer attitude in urban areas. However, the age (≥55: OR = 0. 140),education background (high school: OR = 2. 696,Junior College or higher: OR = 3. 116) and province (Jiangxi: OR = 4. 236,Zhejiang: OR = 4. 028) were the influences amongrural women. The education background (Junior College or higher: OR = 3. 140),province (Jiangxi: OR = 3. 322,Zhejiang: OR = 10.567) and attitude toward breast cancer (positive attitude: OR = 2. 577) could affect the practice of breast cancer screening among urban women, while the nation (non-Han: OR = 0. 316),province (Jiangxi: OR = 9. 107) and attitude (positive attitude: OR = 7. 973) could affect the practice among rural women. Conclusion The awareness rate of breast cancer among women in urban and rural areas was different. The influencing factors of KAP of breast cancer between urban and rural women were various. It issuggested that specific strategies should be developed according to the differences between urban and rural women forbreast cancer prevention.
Objective To understand the current situation of Chinese grassroots family planning team assuming health education tasks. To explore effective mechanisms and mode of resource integration. Methods Combing factors of area, independence of health education institution and integration situation,5 provinces including Beijing,Hebei,Anhui, Shaanxi and Gansu were selected as research field. Through qualitative interview method including group discussion and personal interview,134 objects were surveyed, consisting of leaders and skeleton staff in health and family planning commission, institutions of health education and family planning in Province-Municipality-County tri-level. Results There is a serious quantity shortage of health education professionals, especially at the grassroots level. The proportion of healthy education personnel in the county level was less than 0. 60 /100 000. Most interviewees (129 /134) believed that there is a good feasibility of grassroots family planning team assuming health education tasks. While, the opportunities and challenges coexist. Currently, some provinces have initially explored out some practices and experiences in grassroots family planning team assuming health education tasks. Against the transfer training of basic health education to the family planning personnel, the common primary health education skills could be roughly divided into three categories. Conclusion Chinese family planning network system could play a complementary role in the system of health promotion and education. Against grassroots family planning staff, targeted transfer training of health education should be strengthened. The division of responsibilities should be clear between grassroots health education professionals team and transition family planning staff.
