Objective:To explore the clinical therapeutic effect of additive manufacturing Ti-6Al-4V titanium alloy implants in dental implant restoration.Methods:Retrospective analysis was conducted on 86 patients with posterior tooth loss treated in the Department of Stomatology, the Fifth Affiliated Hospital, Sun Yat-sen University, and the Department of Dental Implantation, the Affiliated Hospital of Guangxi Medical University from November 2020 to June 2022, with a total of 87 missing teeth. Among them, there were 43 cases in the test group, with 44 missing teeth, which were restored with Ti-6Al-4V titanium alloy implants made of self-developed additives. In contrast, another 43 cases in the control group, with 43 missing teeth, were restored with B & B implants (Italy) . The implant success rate, changes in bone tissue around the implant, gingival status, and chewing efficiency within six months after implantation were compared between the two groups with SPSS 23.0 at a significance level of 0.05.Results:In the test group, there was no implant loss, no redness or swelling of the gingiva, and the chewing function was normal after restoration. Two cases showed marginal bone loss, with a success rate of 95.5%. In the control group, two implants failed, and one case had a repeated loose of the abutment screw. The implant success rate was 93%, and there was no statistically significant difference in the success rate between the two groups (χ2 = 0.237, P>0.05) ; The test group had no occlusal interference and food impaction after implant denture restoration, with good chewing function and a total effective rate of 100%. In the control group, one case had poor chewing function due to loose screw, with a total effective rate of 97.7%. There was no statistically significant difference in chewing function (χ2 = 0.181, P>0.05) . The bone resorption around the implant was less than 0.6 mm at each follow-up time point in both the two groups, with no significant difference between them (t = 1.974, P>0.05) . Patient satisfaction with the restoration was 93.2% in the test group and 95.3% in the control group, which was not statistically different (χ2 = 0.236, P>0.05) .Conclusions:Additive manufacturing Ti-6Al-4V titanium alloy implant has achieved satisfactory short-term clinical outcomes in terms of success rate, chewing function and aesthetic effect in the restoration of missing posterior teeth, which can be applied clinically. However, its long-term clinical effect needs to be further observed.
数字化虚拟仿真牙医培训系统是一种新型的口腔医学教学系统,已在口腔临床医学教学多个领域运用,并获得了一定的成效.口腔修复学是应用符合生理的方法,采用人工装置修复口腔及颌面部缺损并恢复其相应生理功能,预防或治疗口腔颌面系统疾病的一门临床科学,其本科教学具有技术性强、示教和观察比较困难等特点.本文内容主要介绍数字化虚拟仿真牙医培训系统的优点及其在口腔修复学本科教学中的应用,为临床前口腔医学生的实践教学提供更好的指导.
目的:根据最新美国公共卫生协会评价标准(USPHS)研究两种修复缺损牙体方法的临床效果.方法:选取2015年10月-2017年10月来笔者医院就诊的牙体大面积缺损的123例患者(共145颗患牙),随机分为对照组62例和观察组61例.对照组采用传统方法桩核冠进行修复,观察组应用数字化椅旁CAD系统制作高嵌体进行修复,根据USPHS标准对患者修复治疗后的第6、12个月修复体情况进行评价,患者和口腔医师进行修复体满意度调查.结果:两组修复后大多数患者的效果均为"A"级,与对照组比较,观察组6个月和12个月的牙龈健康、12个月的边缘适合性及术后敏感性均明显改善(P<0.05),其余指标比较无统计学差异(P>0.05);患者自身评价:与对照组比较,观察组的舒适度明显增强(P<0.05);口腔医师评价:与对照组比较,观察组边缘密合性满意度较高(P<0.05).结论:数字化椅旁CAD系统制作高嵌体对修复牙体大面积受损效果良好,提升了临床疗效,数字化椅旁即刻全瓷修复技术可临床推广应用.
Objective:To analyze and the smile line of Zhuang-Chinese youths.Methods:1 00 Zhuang volunteers aged 1 8-25 years were recruited for this study.The faces were individually registered by digital videography.The length and thickness of upper lips and mandibular incisor crown height appearance at rest,smile and laugh were recorded.Statistical analysis was performed using Mann-Whitney-U Test and non-parametric analysis.Results:The upper lip in men was longer than that in women at rest(P<0.05 ).The upper lip length increased and thickness decreased at smile and laugh in all subjects,in men was longer and thicker than in women(P<0.05),but there was no significant difference of upper lip thickness at rest between sexes(P<0.05).The mandibular anterior teeth,gingival papilla of all subjects were appeared at smile and laugh,there was no significant difference between sexes(P<0.05). Gingiva appearance at smile was in less than 1 0%of the subjects.39%subjects had high smile line,46%had average smile line and 1 5% had low smile line.Conclusion:Most of the Zhuang youths have moderate smile line,less high smile line and least low smile line.
目的:探讨全瓷嵌体修复单颗倾斜低(牙合)后牙后提高咀嚼效率、平衡(牙合)力的临床效果.方法:选择5例后牙倾斜低(牙合)患者参与此项研究,用吸光度法测定患者嵌体修复前后的咀嚼效率并用T-ScanⅢ咬合力计测量两侧(牙合)力的平衡状况.结果:嵌体修复前后患侧咀嚼效能吸光度值分别为0.451±0.158和0.574±0.202,前后比较,差异有统计学意义(P<0.05);两侧(牙合)力差值修复前(-5.9400±1.623 0)%与修复后(-2.346 6±1.468 3)%,二者比较差异有统计学意义(P<0.05);(牙合)力中心点(COF)距中线的垂直距离修复前为(-3.426 7±1.692 9)mm,修复后为(-0.733 3±0.365 1)mm,差异有统计学意义(P<0.05).结论:全瓷嵌体修复后能提高单颗倾斜低(牙合)后牙的咀嚼效率,使两侧(牙合)力在左右向更趋于平衡,从而发挥正常的咀嚼功能.
Objective: To confirm the need of orthodontic treatment of Zhuang youths by conducting a satisfaction questionnaire survey of general dental appearance on them and measuring their Dental Aesthetic Index(DAI) in order to provide references for reasonable allocation of oral health resources. Methods: 77 Zhuang youths were selected randomly to take part in the satisfaction questionnaire survey of general dental appearance and their study plaster models were made to measure the DAI. Results: High rates of satisfaction of general dental appearance(male 80.8%, female70.6%) and great demand for orthodontic treatment(male76.9%, female 84.3%) of Zhuang youths in Guangxi were demonstrated in our survey. Subjective evaluation of malocclusion was no more than 50%. Dissatisfaction of discolored teeth was significantly higher in female(96.1%) than in male(73.1%). Tooth whitening was the treatment most demanded by Zhuang youths(male 50.0%, female 74.5%). Conclusions: The dental appearance could not be recognized objectively by most of Zhuang youths in Guangxi. They also lack of knowledge of the treatments that can improve the appearance of their own teeth, and their need for orthodontic treatments was large. Therefore, we should increase the allocation of oral health resources and enhance the prevention of malocclusion.
Objective:The aim of the survey was to obtain the basic information of shade selection on all-ceramic restorations of the anterior teeth so as to enhance doctor's understanding of color matching and improve their quality of shade selection.Method:474 pieces of anterior teeth working authorizations of all-ceramic restorations were divided into three groups according to the level of consignor.The questionnaire included tooth position,patient age,gender,including doctor partition colorimetric,whether digital photography auxiliary colorimetric or whether to record color special anatomical structure of the patient characteristics including stained,cracked,developmental ditch projects.Result:The statistical test showed there was significant difference in patient age,gender,doctor partition colorimetric and digital photography auxiliary colorimetric among these three groups.Conclusion:In this survey,the quality of shade selection on all-ceramic restorations of anterior teeth is generally ignored by dentists.Doctors for the partition colorimetric and digital photography auxiliary colorimetric technique needs to be future strengthened.
Objective To analyze the stresses in dentin of an endodontically treated maxillary central incisor restored with different dimensions of zirconia post-core crown using three-dimensional finite element method.Methods The model of maxillary central incisor restored with zirconia post-core crown was created by Micro-CT scanning,Mimics software and Pro-Engineering software.Five experimental models with different post length and diameter were simulated.100N inclined force loading beneath the incisal edge 2mm by 45° angle with tooth longitudinal axis on the palatal surface of the crown,and maximum principal stress,equivalent stress were calculated.Results There were no obvious differences of stress distribution of post in the 5 groups.The peak value of maximum principal stress was located at the post lingual middle surface.The peak value of Equivalent stress was located at the labial middle surface.The smaller diameter zirconia post was,the smaller stress was.The shorter length zirconia post was,the bigger stress was.Conclusion In certain extent,the smaller diameter zirconia post is,the smaller stress is.But meticulous post easily bend break.In certain extent,increasing the length of the post can reduce stress.Clinical doctors should properly prepared teeth according to its pulp cavity size.
Objective: To evaluate the retention strengths of total-etching and self-etching adhesive systems on quartz fiber-reinforced posts, and to guide the rational choose of the luting systems during clinical work. Methods: 16 in-vitro sound maxillary central incisors were included in this study. After sound root canal therapy, all teeth were sectioned at the cemento-enamel junction level in adjacent sides, continued with standardized post space preparations. then roots were embedded into self-curing resin for support. 16 samples were divided two groups randomly, with 8 for each group, AESTHETI-PLUS quartz fiber-reinforced posts were placed into the root space using two adhesive systems (self-etching adhesive system and acid-etching adhesive systems) for each group. Pull-out bond strength tests were performed under unique protocol by 1 mm/min . Data were analyzed using t test. Results: The adhesive strengths for self-etching adhesive system and acid-etching adhesive system were 246.48N ±103.58N and 30.46N ±5.41N respectively, difference shows significance by t-test, P0.01. Conclusion: The bonding strength of self-etching adhesive resin cement is better than the acid-etching adhesive resin cement.
【Objective】To study the clinical effects of D.T. Light-post on restoration in the residual roots and crowns.【Methods】187 patients were included in this study,involving 324 teeth. After complete root canal therapy,teeth were restored by D.T. Light-post core and crowns,the follow-up was from 6 to 30 months.【Results】320 teeth were satisfactory,4 D.T. Light-posts were broken. The success rate reached 98.77%. 【Conclusion】The restoration by D.T. Light-post for residual roots and crowns can achieve satisfactory effects.
颅骨锁骨发育不全综合征(cleidocranial dysplasia,CCD),是一种先天性全身骨骼发育不全性疾病,常为遗传性疾病,其主要临床特征是全身多发性骨骼发育畸形及面、牙的发育异常[1].关于锁骨发育异常的报道最早可追溯到1765年,本文就近期就诊的1名CCD患者进行个例报道,为广西地区首次报道该类疾病.
Objective To compare the effect of Candida albicans on the surface of four soft denture-lining materials.Methods Four soft denture-lining materials(Promedica,COE SOFT,Nissin,Shanghai self-curing denture soft liner) were chosen and made into standard test pieces for fungal growth of Candida albicans(ATCC 10261) on the surfaces,and the pH values of the growth media were measured and recorded per 8 hours.Results The decreasing of pH values were observed in all culture media with test pieces coated with Candida albicans(ATCC10261).The curve dropped rapidly after 24 hours,and kept stable around 90-120 hours.Promedica silicone-based soft liner showed lower pH value,which was close to that of self-curing resin material and heat-curing resin material,pH value reached to a stable point after 72 hours.The pH value of media with Shanghai self-curing denture soft-liner showed slower dropping and reached to stable point around 120 hours culture.Conclusion There are three periods of growth for the Candida albican on the surfaces of denture soft lining materials,which are initial,rapid growth and stable periods.The duration varies among different material.It suggests that soft lining may have certain ability of inhibiting the growth of Candida albicans,and denture plaque control is needed when soft lining is used in prosthetics.
目的:比较2种取模方法对取模效果的影响。方法:在对门诊金属烤瓷单冠修复患者116例165颗牙中,随机分为2组,分别采用注射法和常规法制取印模,对模型清晰程度进行评价。结果:注射法与常规法相比较模型清晰程度有显著性差异,注射法印模质量高于常规法。结论:应用注射技术可以帮助取得清晰的印模。
Objective:To compare the effectiveness of three gingival distraction methods in fixed prosthetics procedure.Methods:144 teeth from 107 patients were divided into three groups randomly for porcelain-fused-metal crowns,different gingival distraction methods were applied in different groups,impressions were taken.The effectiveness of gingiva-preparation,bleeding,shoulder shape and sharpness of cast were compared among the three gingival preparation groups.Results:Gingiva retraction group has shown significantly better quality of teeth preparation than group without gingival retraction.Conclusion:The application of gingival retraction can prevent the gingival trauma and bleeding during teeth preparation for fixed prosthetics,and help to make accurate impression.
【Objective】Soft lining materials play an important role in clinical removable denture, however, there is an increased probability of yeast colonization on soft lining materials, which is a causative factor in denture stom-atitis. This study aimed to investigate the function of addition of Nystatin into soft lining material for anti-candida adherence. 【Methods】Four different types of soft lining materials were used in this study. Nystatin was added into materials respectively by 3,6,9 W/W drug/material ratio. Then making the mixed material into standard pieces for test the adherence of candida albican on the surface by culture, counting the cloning. 【Results】Among four kinds of materials, the pieces with Nystatin showed significantly stronger anti-fungal ability than the soft lining materials alone. The antifungal ability decreased within 6~8 days. No significantly difference observed between the 6%w/w and 9%w/w for the same material, however, both showed higher antifungal ability than samples with 3% Nystatin or with 0 % Nystatin. 【Conclusion】For any soft liner material, the inhibition ability to candida albican is associated with the concentration of Nystatin added. The reasonable effect can be achieved by 6% w/w, and decreased by time. It suggests that Nystatin may be added into soft lining material to prevent denture stomatitis in short time purpose.
Objective: To assess the adherence ability of Candida albicans(C.albicans, C.a.)to four soft denture lining materials with varying surface roughness. Methods: In adhesion assays, four soft denture lining materials were made into standard test samples with different surface roughness. Suspension of C.albicans (ATCC 10261) strain was used to adhere on the test samples, and cultured to observe the adhesive effect of a mixed salivary pellicle with candida to these surfaces. Results: Significantly greater adhesion of C.albicans to rough surfaces rather than smooth surfaces of the same material was found (P 0.002), at the same time, C.albicans had different adhesive ability on different soft lining materials. Conclusion: Roughness of the surface affects the adhesion of C.albicans on the denture-base materials as well as the characteristics of the material itself. The rough surface promotes C.albicans adhesion compared to the smooth one.
目的回顾性分析并探讨影响金属烤瓷修复体固定修复的常见问题以及相应的预防措施。方法收集本科近1年因金属烤瓷冠、桥修复后出现问题就诊患者的资料以及临床结果,进行回顾性分析。结果出现崩瓷5颗,烤瓷冠松脱5颗,基牙折断3颗,龈缘黑线3颗,发生牙龈炎2颗,食物嵌塞4颗,共计22颗为术后不满意的病例。结论金属烤瓷牙在临床的应用可以取得良好的效果,常见的术后并发症总发生率较低,通过改进设计,精细操作可有效避免类似并发症的发生。
OBJECTIVE:To investigate the candidal carriage and the Candida species in HIV positive patients and to explore the relationship between oral candidal carriage and oral candidiasis.METHODS:Sixty-four HIV positive patients and 42 healthy controls were included in this study. Oral rinse technique was used to detect the candidal carriage. The isolates were identified using multiple measures, including Gram staining reaction, chlamydospore, pseudo-hyphal and hyphal production test, CHROMagar Candida test and API 20 C AUX yeast identification system.RESULTS:Thirty-nine of 64 HIV positive cases were diagnosed as oral candidiasis. Seventy-four Candida strains were isolated from 52 of 64 HIV positive cases, only 7 strains were isolated from 42 healthy controls (P < 0.001). Of the 74 Candida strains isolated from HIV positive cases, 39 were Candida albicans, 15 Candida tropicalis, and 20 other 6 species.CONCLUSIONS:A high prevalence of oral candidiasis and high candidal carriage were found in HIV positive patients compared with those in controls. Candida albicans and Candida tropicalis were the major species. The biotyping of the species isolated from HIV positive patients showed more diversified compared to healthy people, which may suggest the decreased immune ability of the HIV positive patients.
口腔内科实习在生产实习之间占有极为重要的位置,教师在其中又起着关键的指导作用.由于学生理论学习与生产实习相隔了较长的时间,故进入生产实习前理论知识和实际操作都比较生疏.
目的:探讨鼻咽癌(NPC)放疗期患者口腔反应及念珠菌负荷状况及生物分型.方法:同期观察NPC放疗期患者48例(实验组)和健康对照者42例(对照组)的口腔反应,并采取漱口法进行口腔念珠菌的定量检测,利用革兰染色法、厚壁孢子生成实验、CHROMagar显色培养和API 20 CAUX鉴定系统对分离株进行生物型鉴定.结果:实验组中有30例主诉口干、疼痛,其中10例伴有味觉障碍; 8例有放射性口炎;7例为口腔念珠菌病,与对照组比较差异有统计学意义(P<0.05或P<0.01);实验组口腔念珠菌的检出率为50.00%,高负荷检出均来自有口干主诉的患者,而对照组仅为16.67%,负荷均低于500 CFU/ml,两组比较差异有统计学意义(P=0.001,OR值=3.000,95% CI 1.44~6.24); 24个分离株中白色念珠菌15株,近平滑念珠菌2株,高里念珠菌2株,法氏念珠菌2株,其它3株.结论:NPC放疗期患者口腔健康状况受损,主要表现为口干和放射性口炎,口腔念珠菌病发生率增高.口腔念珠菌的检出率增加,除白色念珠菌外,其它菌型亦可能成为NPC放疗患者口腔念珠菌的病原菌.
