Установление связи между биологическим и социогуманитарным научными дискурсами все более преобразуется в задачу науки, и ее целью является раскрытие образа биологической идентичности как идентичности культурной. Этот феномен рассматривается как концептуальное определение жизненного мира через стремление расширить осмысление человека с использованием современных концепций в биологии, не отказываясь при этом от традиционных подходов, возникших в рамках философии и этики. Встраивание биологического и биомедицинского знания в социогуманитарные дисциплины становится запросом не только на познание, но и на самопознание человека или, по крайне мере, на нахождение альтернатив традиционным философским и этическим репрезентациям человека, с сохранением последних. Феномен биосоциальности выступает не только как способ решения локальных проблем отдельных сообществ, но и значительно шире — как дополнительная для культуры версия представления человека о самом себе, вытекающая из критики трансцендентального понимания субъективности и обращения к контингентности как основе, на которой можно было бы построить модель человека.
The article deals with the problem of respect for personal autonomy in sports and the practice of restricting athletes. The two justification factors for restrictions, medico-paternalistic and ethical, are in close correlation with each other and with the ethics of public competitions. The article analyzes the problem of respect for personal autonomy from the point of view of the contradiction between the formally rational set of rules that reflect the standards of fair sports and the unformalized impulse of an autonomous actor, determined by his desire for self-realization and public recognition in its entirety, including the features of biosocial identity. The article analyzes the case of female athletes with a naturally elevated level of testosterone in the blood.
Интеграция уважения персональной автономии в медицинскую практику остается одной из самых дискуссионных междисциплинарных задач биоэтики, несмотря на кажущуюся простоту процедуры получения согласия у пациента, в силу сложности репрезентации этического смысла медицинского действия, предубеждений и влияний окружающих на мнение пациента. Реализация принципа уважения персональной автономии потенциально сводится к защите от возникновения юридических тяжб, либо осуществление уважения персональной автономии происходит формально в силу ограниченности медицинских ресурсов, необходимых для разъяснения обстоятельств лечения или исследования. Вместе с тем персональная автономия остается признанным правом человека, взаимодействующего с медицинской системой, на соучастие в том, что предполагается делать с его телом или образцами его тела, которое проявляется через выражение его согласия или несогласия с действиями врача или исследователя. В статье анализируется концепция уважения персональной автономии как проблемы смыслополагания и ценностного обоснования в биоэтике с помощью философской аргументации. В работе анализируется концепция Ю. Хабермаса о философии как «интерпретаторе» и «местоблюстителе», оказывающая влияние на формирование моральных представлений, определяющих легитимность научного исследования в жизненном мире. Рассматриваются проблема комплексности модели персональной автономии и конкретные кейсы, в которых представлено различение телесного и ценностного аспектов, регулируемых с помощью уважения персональной автономии.
Social trust is the basis for the development of biomedical technologies that affect the value and moral spheres of modern society. Axiological factors, external to science, are decisive in the formation of the legitimation of scientific research. Bioethics allows for the “acclimatization” of rather controversial scientific developments, achieving through a deliberative social process the recognition of new scientific methods that directly affect human life. The need to obtain informed consent from donors of biobanks is associated with the generally accepted procedure for conducting clinical trials. In a broader sense, biomaterials in the context of research are considered as the inalienable property of patients, which implies their right to control what happens to samples of biomaterials. However, given the workload of biobanks, which can host dozens of studies, the process of informing all donors about each individual study is an extremely difficult task, requiring both technical and time resources. Today, the form of extended informed consent is considered as the most appropriate model for the interaction of donors and depositories of biomaterials. Thus, the legal approach to the problem of informed consent for biobanks would seem to be more functional, while the need to take into account the moral principles of biobank donors remains relevant as a condition for trust in science.
The transition from an irregular understanding of nature as a given to the regulatory concepts of human development is one of the central philosophical and socio-humanitarian issues in the development of not only biotechnologies, but also society as a whole. In the theory of philosophy of biomedicine, the discussion is structured as the positioning of various problematic approaches, modeled using the principles of bioethics and philosophical ethics, taking into account the actual experience of the application and social perception of biomedical technologies. The status of problematic approaches is determined not only by philosophical ethics, but also by the willingness of society to accept something new as its own future. At the same time, accepting the future is impossible without rooting the future in the past - the beliefs and expectations that legitimize the future. The correlation of such concepts as the authentic autonomy of J. Habermas and the expansion of utilitarianism into the problems of editing the human genome, the conflict associated with challenges requiring collective moral action, and the rigidity of traditional moral mechanisms lead to the search for such a sociobiological language that would be formed from competitively coexisting old, traditional, and new, bioengineering, concepts of human development. The idea of biocultural theory as a form of connection between culture and biological foundation is associated with the work of A. Buchanan and R. Powell, who propose a systemic definition of biocultural theory as a mutual biological and cultural transformation of a person. Biocultural theory is aimed at shaping such a philosophical horizon, where the body, not only carnal, such as organs, but also personal - the awareness of its own bioidentity, becomes open and understandable due to the expansion of the connection between biology and culture, but at the same time acquires problems that becomes the subject of philosophy and ethics, since now a person, comprehended as a body, receives a variability that is no longer associated exclusively with culture. The goal of the article is to show that editing a person is not so much a traditionally understood risk as a transformation of the understanding of the cultural and biological conditions for the formation of his bioidentity.
Развитие генетических технологий способствовало стремительной биосоциализации современного общества. Биосоциальность и биоидентичность - феномены, описывающие важнейшие глобальные и локальные процессы развития современной биомедицины, определяющие интересы все большего числа людей. Создание групп по биосоциальным признакам невозможно без принципа конвергенции усилий. Он работает на разных уровнях общественной жизни, оказывая влияние и на формирование научного знания. Телесность пациента может рассматриваться в качестве гибридной формы, где сплетается органика, знание, технологии и экзистенциальный опыт персональной истории болезни. Возможности легитимации в будущем генетических технологий улучшения человека закладывают основание для формирования новых локусов биосоциальности и биоидентичности. The development of genetic technologies has contributed to the rapid biosocialization of modern society. Biosociality and bioidentity are phenomena that describe the most important global and local processes of development of modern biomedicine, which determine the interests of an increasing number of people. Creating groups on biosocial grounds is impossible without the principle of convergence of efforts. It works at different levels of social life, influencing the formation of scientific knowledge. The physicality of the patient can be considered as a hybrid form where organics, knowledge, technology and existential experience of a personal medical history are intertwined. The possibilities of legitimizing the future genetic technologies for human enhancement lay the foundation for the formation of new loci of biosociality and bioidentity.
The COVID-19 pandemic caused by the SARS-Cov-2 coronavirus is a kind of crash test arranged by wild nature for humanity. This challenge requires maximum mobilization of the moral and material resources of states, communities, and citizens. Attempts to respond to it reveal the strengths and weaknesses of national health systems and international cooperation in this area. It is essential to analyze comprehensively the lessons of the pandemic and respond promptly and constructively to the emerged problems. What can we know about the rapidly developing adverse situation and how can this knowledge be conveyed to the society — to politicians and ordinary citizens? Where are the moral boundaries that transform the emergency measures necessary to contain the pandemic and mitigate its dramatic consequences from necessary means of rescue into the sources of moral evil, sometimes unacceptable? What can we hope for when we once again face a total natural or anthropogenic threat to the survival of humanity? What are the possibilities of medicine in a disaster and how are the tactics of providing medical care when there is an acute shortage of all the necessary resources? The participants of the round table seek to identify approaches to answering these questions, give a diagnosis of the situation from the point of view of humanities, and discuss the efficiency of the relevant intellectual and technological resources available to humanity. In particular, they show that in order to ensure stable development in the face of permanent external (natural) and internal (man-caused) challenges, there are proven tools that are not always in sufficient demand in the current emergency situation. They identify the rich intellectual, organizational and political traditions of human survival in conditions of anomie and chaos (wars, natural disasters, pandemics, etc.); discuss various options for matching the tasks of preserving public safety and respect for the dignity of the individual; archetypal structures of biopolitical devices of isolation (quarantine) and the phenomenon of emergency as an immanent characteristic of the human situation are investigated; heuristics of the humanitarian vision of the human situation in catastrophic circumstances are revealed.
Bioethics is associated with the creation of flexible regulatory principles and their adaptation to an extremely diverse socio-humanitarian environment. Most biomedical interventions involve risks and from bioethical point of view, require attention and the patient’s autonomous approval, as well as strong arguments to justify the very fact of these interventions. The Covid-19 pandemic has brought bioethics to world-wide time pressure. In fact, it became a catalyst for radical security measures and restriction of freedoms, the creation of regulatory documents for the allocation of limited medical resources. The implementation of the principle of autonomy at the same time turned out to be extremely limited due to the introduction of rules for wearing masks and protective equipment, and at the same time, the solidarity of autonomous actions of many people allowed some countries to reduce the rate of development of the epidemic. During a pandemic, when the regulation of actions that have a medical justification becomes an absolute priority, bioethics based on the principle of autonomy cannot but change, since different models of interaction between autonomy and benefits during an epidemic form opposite algorithms of actions not only for patients, but also for doctors .The logic of the allocation of scarce medical resources and the substitution of patients' autonomous decisions with arguments of the greatest good is even more important. The results of the application of various strategies for the implementation of autonomy in a crisis generated by a pandemic can influence the formation of postcovid bioethics and research ethics. The article analyzes the bioethical aspects of the implementation of personal autonomy during the Covid-19 pandemic and draws a conclusion about the changes that are taking place in the understanding of personal autonomy.
Иосоциализация общества представляет собой комплекс явлений, среди которых в философском и этическом аспекте выделяются технонаучный подход к знанию, повышение роли автономии и проблема рефлексии. Для биобанков - это комплекс проблем, который реализуется как сложность разработки информированного согласия. Проблема подлинного информированного согласия является крайне важной для успеха биобанкинга. Если ученые рассматривают информированное согласие как формальность, то сами доноры биобанков, осознавая, что их биоматериалы, возможно, используются некорректно в одном из множества проектов, которые осуществляются на базе биобанка, могут не испытывать чувство справедливости и согласия с условиями, которые им предлагаются в рамках расширенного информированного согласия. Сохранение идеи информированности является важным условием формирования позитивного образа биобанкинга. The biosocialization of society is a complex of phenomenon, among which, in a philosophical and ethical aspect, the techno-scientific approach to knowledge, the increasing role of autonomy and the problem of reflection stand out. For biobanks, this complex of problems is realized as the complexity of developing informed consent. The issue of genuine informed consent is critical to the success of biobanking. If scientists consider informed consent as a formality, then biobank donors themselves, realizing that their biomaterials may not be used correctly in one of the many projects that are implemented on the basis of biobanks, may not feel a sense of fairness and agreement with the conditions that are offered to them within broad informed consent. Maintaining the idea of awareness is an important condition for the formation of a positive biobanking image.
В статье рассматриваются различные типы свободы в профессиональной деятельности спортсмена. Показано, что возможны два типа самореализации персональной автономии в спортивной деятельности. Первый тип автономии связан с биомедицинскими ограничениями (выраженными в антидопинговом регулировании), гарантирующими защиту от рисков и идеализированную естественность спортсмена в проявлении его свободы при достижении высоких спортивных результатов. Второй тип персональной автономии в спорте, напротив, выражен в развитии конструктивистского и экстремального представления о теле спортсмена, безграничных возможностях его модификации и совершенствования доступными биомедицинскими средствами. Граница между первым и вторым типом спортивной автономии формируется не только под регулятивным воздействием института спорта, но и, что не менее важно, зависит от личностной саморегуляции спортсменов, принимающих на себя самые разнообразные риски (недостижения спортивного результата, угрозы здоровью, ограничения личностного потенциала). Важно отметить, что рекорды, полученные в рамках второго типа реализации автономии спортсмена, расширяют представления о естественных физических способностях и зачастую рассматриваются как проявление морфологической свободы человека, направленной на управление и инструментализацию собственного тела. Однако официально, при доказательстве использования допинга, такие рекорды не только не получают признание, но и негативно влияют на имидж спортсмена и его профессиональную карьеру. В статье на примере исследования возможностей разграничения терапии и улучшения в таких сферах жизни, как спорт и образование, поставлена проблема значения персональной автономии для определения антропологической нормы в профессиональной деятельности. Сделан вывод о том, что возрастающие требования к достижению спортивных результатов при неизменном запасе прочности человеческой природы стимулируют запрос на расширение персональной автономии. Современный спортсмен вынужден не только играть за себя, команду или страну естественными усилиями, но и участвовать в фармакологических и генетических играх. В то время как общество очерчивает объективные границы свободы в спорте, а спортивная этика разрабатывает жесткие правила поведения спортсмена, сами спортсмены также формируют и реализуют свое личное представление о физических возможностях собственного тела через признание персонального горизонта риска.
Расширение роли генетических данных в лечении, диагностике и определении биоидентичности человека представляет собой одну из форм биомедикализации общества. Чем больше мы узнаем о влиянии генома на здоровье человека, его жизненные траектории и поведение, тем привлекательнее выглядит использование биомедицинских результатов в социальной сфере. Но при этом наука, занимающаяся проблемами общества, организуется и развивается по принципам гибридной технонауки. Развитие биобанков нужно рассматривать как технонаучный проект, в котором пересекаются несколько ключевых проблем современной биомедицины: гибридизация культурного и научного дискурсов, генетизация и проблема защиты автономии в новой биомедицинской среде, построенной из научных сетей обработки генетическои информации. Биобанки представляют собой один из ключевых элементов генетизации общества — процесса, в ходе которого генетическая информация становится основным источником знаний болезни и поведении человека. В статье исследуется, как биобанки связаны с технонаучнои биосоциальностью и какое влияние они могут оказать на представления о благе и автономии. Отмечается явление, которое можно обозначить как генетизацию этики — это признание взаимосвязи генов и социальных траекторий и социальное признание объективности влияния экспрессии отдельных генов на автономию личности.
В статье рассматривается проблема генезиса онтологий социогуманитарных рисков, возникающих в связи с развитием биотехнологий. Гибридный характер сосуществования технонаучного и социального миров определяется как слияние социального ожидания и технологий. Вместе с тем уже сложившиеся социогуманитарные практики остаются важными теоретико-методологическими матрицами, в рамках которых определяются биотехнологические онтологии рисков. К этим методологиям можно отнести биоэтику, наноэтику, исследования науки и технологий (STS), проблематику улучшения человека, гуманитарную экспертизу. Результатом применения этих методологий к технонаучным биотехнологическим проектам является не только онтологизация рисков, но и их деонтологизация в процессе фактической реализации биотехнологических проектов. Этот процесс рассматривается на примере концепции генетизации общества.

The article reveals the heuristic potential of the concept of border zones of human existence for the development of human enhancement problems. These problems had been the focus of B.G. Обоснование границ допустимого является основной темой философского и этического сопровождения биотехнологических проектов. Причиной того, чтобы пересматривать существующие представления о целях применения биотехнологий, могут оказаться как развитие науки, так и появление концепций, реагирующих на философские и этические конфликты, порожденные восприятием биотехнологий. Современный тип технологической рациональности возникает как результат связывания технологий с биологией и физиологией человека и смешивания искусственного и естественно-биологического содержания науки. Постепенно, но неуклонно происходит переосмысление человека как самодостаточного природного явления, не нуждающегося в каком-либо улучшении. В статье рассматривается критика утилитаристского подхода к биотехнологиям (Дж. Харрис), ограниченного идеей природы человека как общего блага. Сложность с изменением человека заключается в том, что не всегда удается точно определить человека, которого собираются менять при помощи биотехнологий. Сложилась такая ситуация, когда как сторонники предостережений об опасностях, которые несет в себе улучшение человека (вплоть до потери не только традиционной формы гуманности, но и самоидентичности на биологическом уровне), так сторонники радикального улучшения человека, которые, напротив, не видят в человеке как в биологическом и духовном существе ничего, что следовало бы сохранить, находятся в периоде сомнений и размышления о будущем человека.

В статье рассматриваются особенности становления персонализированной медицины как социотехнологического артефакта. С точки зрения теории исследований науки и технологий ценности общества оказывают фундаментальное влияние на развитие технологий. Научный проект как совокупность работающих технологий превращается в социотехнологический артефакт в том случае, если он признан обществом. Даже нефункционирующие технологии могут рассматриваться как работающие, если они получают общественное признание. Сторонники персонализированной медицины исходят из того, что люди с некоторыми особенностями, обнаруживаемыми с помощью генетических тестов, лучше реагируют на определенные лекарства. Так, фармакологическая компания Roche имела в своем активе лекарство Xeloda, которое, попав в организм, переходило в активное состояние под воздействием определенных ферментов, встречающихся только у некоторых пациентов. Персонализированная медицина опирается на самые современные достижения науки и техники, причем не только биомедицины, но и информационных технологий, связанных с хранением, анализом и построением алгоритмов обработки больших объемов информации. В связи с этим развитие персонализированной медицины сопряжено с технологическими трудностями. Несмотря на то что биологические данные позволяют не только делать новые обобщения и устанавливать биологические закономерности с помощью «омиков», они также несут в себе большое число ошибок на уровне хранения и обработки значительных объемов информации. Тем не менее это не мешает рассматривать персонализированную медицину как многообещающий проект, реализующий представления о медицине будущего. Становление персонализированной медицины предлагается рассмотреть как развитие идеи, согласно которой технодетерминизм выступает в качестве социогуманитарной ценности, а персонализированная медицина представляет собой проекцию социогуманитарного технодетерминизма на конвенциональную медицину.
The article deals with the philosophical aspects of social and humanitarian support (SHS) of innovative projects in the field of personalized medicine (PM). PM is based on the latest advances in molecular biology (genomics, proteomics, metabolomics) and bioinformatics. It aims to develop individualized (personalized) methods of accurate prediction and prevention of potential disease with active participation of patients (or clients). By their very nature, PM projects are not only biomedical, but social and humanitarian as well. Successful implementation of PM paradigm in healthcare system depends on SHS, i.e. on solving of a wide range of ethical, legal, social, economic, political, and other problems. Theoretical underpinnings of the SHS has been developed by the authors in their studies of the role of socio-humanitarian technologies in biology and medicine (e.g., bioethics and humanitarian expertise); of modern concepts of personality, including those of personalization, personification and individualization; of PM as a kind of foresight projects (at individual, family, group and national levels); of ethical and political procedures of social justice and individual autonomy of PM subjects; of the concept of innovation that combines traditional administrative implementations of innovation and the widespread use of modern social and humanitarian technologies, which ensure that PM stakeholders arise as subjects of health care, education, politics, business, science, industry, thus promoting their cooperation through education, use of social technologies, fundraising, outsourcing, transfer of knowledge and skills, etc. SHS is a necessary precondition for the society’s acceptance of PM as a new medical paradigm and acts as a guarantee that the most complex problems that society faces will be diagnosed and overcome. As every human achievement, PM will inevitably come at a certain price. Monitoring its possible positive and negative socio-humanitarian implications must thus become a crucial aspect of SHS.
The 8th Conference “Advanced Technologies and Bioethics” was taking place on 7th–8th September, 2011 at Moscow University for the Humanities. It was organized by the International Society for Clinical Bioethics, Russian Committee on Bioethics under the Commission of the Russian Federation for UNESCO, UNESCO Moscow Office and the Institute of Fundamental and Applied Studies at MosUH.
