The negative impact of infertility and its treatments on sexual health is well-documented. Self-concept plays a crucial role in sexual behavior and overall sexual health. This study aims to investigate how couple-oriented counseling, using the Ex-Plissit model, can influence the sexual self-concept of infertile couples. This parallel-group randomized controlled clinical trial was conducted in Hamadan, Iran, at the infertility clinic of Fatemieh Hospital, from 2022 to 2023. One hundred twenty individuals (60 infertile couples) who referred to the clinic were randomly assigned to either the intervention group (n = 30 couples) or the control group (n = 30 couples). Based on entry criteria, the allocation sequence was determined using six blocks of 4 before the study began. Demographic characteristics and a self-reported sexual self-concept questionnaire were used to identify infertile couples with negative sexual self-concepts. The intervention group received four sessions of couple-centered sexual counseling based on the Ex_PLISSIT model, while the control group received routine treatments. Registration date in IRCT: 2022-09-13 .After the sessions, both groups completed the sexual self-concept questionnaire again. Data analysis was performed using SPSS version 26 software, including descriptive and inferential statistics (paired T-test, independent T-test, Wilcoxon, covariance) with a significance level of P < 0.05. The research findings indicated that the average score of women's sexual self-concept in the intervention group before the intervention was 21.50 (4.86), slightly higher than the control group's average of 19.87 (8.41). However, this difference was not found to be statistically significant (p = 0.361). Furthermore, there was no significant difference in the average sexual self-concept scores between the intervention and control groups of women after the intervention (p = 0.053). Interestingly, there was a statistically significant difference in the average sexual self-concept scores between the intervention and control groups of men after the intervention (p = 0.003). The paired t-test results revealed a statistically significant difference in the average score of men's self-concept before and after the intervention group intervention (p = 0.001). Furthermore, when comparing the mean (standard deviation) of men's self-concept scores in the post-intervention phase while controlling for pre-intervention scores, smoking, and male occupation, the average score in the intervention group was found to be 17.45 (3.23) lower than the control group's average score of 21 (3.23). This difference was also statistically significant (p = 0.001). The results of the current study indicate that couple-based counseling using the Ex-PLISSIT model has a significant impact on improving negative sexual self-perception in infertile couples. Therefore, infertile couples visiting fertility centers should be evaluated for their sexual function and self-perception. If needed, they should be offered educational, therapeutic, and counseling programs to provide them with the necessary information.Trial registration: Iranian Registry of Clinical Trial IRCT20120215009014N443.
INTRODUCTION:Premenstrual syndrome (PMS) significantly affects women's quality of life. This study compares the effects of propolis and fennelin capsules on PMS symptoms in students. METHODS:A double-blind, randomized controlled trial (2024) was conducted with 90 students at Hamadan University of Medical Sciences, who were randomized into propolis (500 mg), fennelin (30 mg extract), or placebo groups (30 participants each). Interventions were administered over two menstrual cycles. PMS symptom severity and frequency were assessed using validated questionnaires and analyzed with SPSS-21. RESULTS:Propolis and fennelin reduced PMS symptom severity by 78.9% and 79.3%, respectively (p = 0.016), whereas no significant change was observed in the placebo group (p = 0.206). No side effects were reported. CONCLUSION:Propolis and fennelin may serve as complementary options for short-term PMS symptom relief in young women. However, the small sample size and short duration limit the generalizability of the findings, highlighting the need for future studies with larger, more diverse populations and longer follow-up periods. Introduction/ Objective: Menopause and the decrease in estrogen levels have numerous negative effects on sleep quality and mental health in women, potentially leading to anxiety and irritability. Non-pharmacological treatments have gained attention as alternative methods for improving sleep quality. This study aimed to evaluate the effectiveness of Cognitive Behavioral Group Therapy (CBGT) with or without lavender aroma on sleep quality in post-menopausal women. METHODS:This randomized controlled trial (RCT), double-blind, enrolled 135 eligible menopausal women from health centers in Hamadan, Iran, between 2022 and 2023. Participants were randomly assigned to three groups: CBGT with placebo, CBGT with lavender aroma, and a control group. The intervention consisted of eight 60-90-minute CBGT sessions over one month for the first two groups, with placebo or lavender aroma administered. Sleep quality was assessed using the Pittsburgh Sleep Quality Index (PSQI) before and after the intervention. Data were analyzed using SPSS version 21, with statistical significance set at p < 0.05. RESULTS:Participants had a mean age of 60.1 ± 5.9 years and a menopause duration of 3.1 ± 1.2 years. In the CBT with lavender group, 79.1% achieved good sleep quality (PSQI ≤ 5) postintervention (p = 0.001), with significant improvements in subjective sleep quality (p = 0.001), sleep duration (p = 0.001), and sleep efficiency (p = 0.001), and reductions in sleep latency (p = 0.001) and sleep disturbances (p = 0.001). No significant improvements were observed in the CBT with placebo or control groups (p > 0.05). CONCLUSIONS:Combining CBGT with lavender aromatherapy is an effective nonpharmacological approach to improve sleep quality in menopausal women, potentially reducing reliance on pharmacotherapy and its associated side effects.

Background and Objective: Nipple fissures are a common issue during breastfeeding that can cause pain and discomfort for mothers and complicate the breastfeeding process. Therefore, the present study aimed to compare the effects of thyme infusion compress and chamomile infusion compress on the severity of nipple fissures in breastfeeding women. Materials and Methods: This randomized clinical trial was carried out at Fatemieh Hospital in Hamadan, Iran, with 102 breastfeeding women in 2022-2023. Participants were divided into three groups: two groups used tea bag compresses (thyme and chamomile), and the third group used expressed breast milk to reduce nipple fissures. Treatments were applied at least four times daily for 14 days. The severity of nipple fissures was assessed on days 4, 8, and 14 post-treatment using a checklist, and data were analyzed with SPSS (version 21) software. The significance level was set at P<0.05. Results: In the between-group comparison, the improvement in nipple fissure severity on days 4, 8, and 14 post-intervention showed a statistically significant difference among the three groups (P=0.001). The thyme tea bag group exhibited a greater reduction in nipple fissure severity compared to the other two groups, while the expressed breast milk group showed the least reduction. Conclusion: Thyme tea bags were more effective than chamomile tea bags and expressed breast milk in improving the severity of nipple fissures in breastfeeding women. Therefore, the use of these harmless herbal methods alongside other approaches is recommended.
Objective:To study the relationship,the mediating effect of organizational commitment on nurses' ethical climate and ethical behavior. Methods:Structural Equation Modeling(SEM)was employed to perform path analysis in this exploratory study.The participants consisted of 280 Iranian nurses who were studied through proportional random sampling.The research tool was consisted of(1)the Ethical Climate Questionnaire by Cullen and Victor,which examines the ethical standards of selfishness,philanthropy,and ethical principles,as well as the reference sources of ethical arguments,including individual,regional,and global ones;(2)Allen and Mayer's questionnaire,which measures organizational commitment in 3 areas:emotional commitment,continuous commitment,and normative commitment;and(3)Peterson's Ethical Behavior Questionnaire,which combines nine items to form interconnected ethical behaviors and is based on a 5-point Likert scale.The data were analyzed using AMOS and SPSS v.23 software. Results:The correlation coefficient between the two variables,ethical climate and ethical behavior,showed a significant relationship(r=0.10,p=0.085).The organizational commitment showed a significant relationship between some dimensions and the total ethical climate.The ethical climate affects the organizational commitment(beta=3.261)and behavior(beta=0.121).Organizational commitment does not show any significant relationship with ethical behavior.The ethical climate affects the organizational commitment and the ethical behavior. Conclusions:Nursing managers and nursing staff should consider the ethical climate that affects the organizational commitment and nursing ethical behavior.
BACKGROUND:Postpartum hemorrhage (PPH) is one of the three leading causes of maternal mortality. Herbal medicine is one of the well-known methods of treatment in the world. OBJECTIVE:This study was conducted to determine and compare the effect of dates, grape seed and dill seed extracts on the PPH rate in women in the fourth stage of labor. METHODS:Two hundred eligible multiparous women visiting the maternity ward in Hamadan for normal delivery participated in this double-blind parallel-group clinical trial. They were randomly divided into four 50-member groups of experiment (3 groups) and control (1 group). The data were collected using a researcher-made questionnaire. In the experimental groups, in addition to routine care and oxytocin administration, the mothers received a capsule containing 15 mg of dill seeds extract or 20 mg of grape seed extract or 5 g of dates extract immediately after the delivery, and then their hemorrhage rates were measured one and two hours after delivery. The control group received routine care, oxytocin, and placebo. Data were analyzed in SPSS 21. RESULTS:The hemorrhage changes in the placebo group were much less than the intervention groups at 1 and 2 hours after delivery, and the four groups had a significant difference in terms of hemorrhage in both hours (p <0.001). The overall difference between the dates extract group and the placebo and grape seed groups was also statistically significant (p <0.05). CONCLUSION:All three extracts of dates, grape seed and dill seeds, especially dates, can be effective in PPH reduction.
Background: Uterine cervicitis in women requires careful selection of a suitable and straightforward treatment. This study aimed to examine the effect of Nika vaginal cream on the symptoms of uterine cervicitis in women. Materials and Methods: This double-blind, randomized, controlled clinical study was conducted on 66 women who where reffered to Hamadan health centers in 2021(33 people in each group). The participants in the intervention group were instructed to use Nika vaginal cream every night for 14 nights, along with a single dose of one gram of azithromycin, 400 mg of cefixime, and 500 mg of metronidazole twice a day for both the patient and her husband. The control group received a placebo cream and the same drugs as the intervention group. After the 14-day treatment period, clinical signs were assessed through examinations, and participants completed questionnaires again. Results: The study found that participants in the Nika user group experienced a 69.70% improvement in symptoms related to uterine cervicitis. This improvement was significantly higher than the control group, which only showed a 45.50% improvement. Although the Risk Ratio (RR) between the two groups was 53% higher in the intervention group, this difference was not statistically significant (p=0.06). However, the Risk Difference (RD) of 0.24 between the groups was found to be statistically significant (p=0.04). Additionally, participants in the intervention group reported a 76.00% reduction in dyspareunia (p=0.03) and an 84.00% reduction in urinary frequency (p=0.05). Conclusions: According to the result of RD, Nika vaginal cream had a significant effect on the improvement of cervicitis symptoms.
Background Husbands, being the primary and closest individuals to menopausal women, can provide crucial support during this transitional phase by gaining a proper understanding of menopause and its associated conditions. Aims This study aimed to examine the effects of couple counseling on the severity of vasomotor symptoms and the quality of life in postmenopausal women. Methods A randomized controlled trial study was conducted on 70 postmenopausal women and their husbands in health centers of Qorveh City in 2011. Couples were randomly assigned to two intervention and control groups of 35 people. Inclusion criteria included the spouse's being alive, hot flashes, first five years of menopause, no use of hormonal drugs and herbal remedies to reduce menopausal symptoms, physical and mental health, no adverse life events in the past six months, literacy, and consent to participate in the study. Data collection tools included a demographic profile form, a 24-hour checklist of hot flashes and sweating intensity, and a menopause-specific quality of life (MENQOL) questionnaire. The intervention group received four 90-minute couples counseling sessions for one month. Data were collected before and after the intervention and analyzed using SPSS-23. Results The results showed that before the intervention, the intensity, duration, and frequency of hot flashes in the study group were significantly higher than in the control group (p < 0.05). After the intervention, a significant decrease in the intensity and frequency of sweating was observed in the study group compared to the control group (p = 0.01). There was no difference between the quality-of-life scores in the two groups before and after the intervention. Conclusion Couples counseling is recommended as a strategy in care and support programs for menopausal women, as it has been shown to reduce the severity of vasomotor symptoms. To improve quality of life, it is essential to consider not only the physical condition but also the psychological, social, economic, and environmental well-being of couples.
Background: The prevalence of functional ovarian cysts in women is high, and choosing an appropriate, and uncomplicated treatment is necessary. Objective: Compare the effectiveness of honey, olive, and propolis combined vaginal cream with OCP in the treatment of functional ovarian cysts in women of reproductive age in Iran. Methods: This randomized clinical trial study was performed on 40 women of reproductive age referred to Hamadan health centers in 2020. Patients were randomized into two groups, A and B. Group A received oral contraception pills for two consecutive cycles from the seventh day of the menstrual cycle tablets for 21 days after the end of menstruation until the beginning of the next menstruation. Group B received honey, olive, propolis combined vaginal cream. In both groups, the treatment continued for two consecutive cycles. Data was collected through a checklist that included clinical symptoms and ultrasound results. The results were analyzed by Stata-13 and the significance level cut-off was considered less than 0.05. Results: Intra-group comparisons showed that there was a statistically significant (P < 0.05) decrease in cyst size after the intervention in both groups. Intergroup differences were not statistically significant (P ˃ 0.05). Conclusion: Honey, olive, propolis combined vaginal cream (Nika) can be effective like oral contraceptive pills in reducing the size of ovarian cysts. Background and Objective: Organizational commitment of employees is one of the important variables in the effectiveness of organizations. One of the effective factors of nurses' organizational commitment is the organizational culture of health centers. The present study aimed to investigate the correlation between the organizational culture and organizational commitment of nurses at Hamadan University of Medical Sciences. Materials and Methods: This descriptive correlational study was conducted based on an analytical design. The statistical population included all nursing staff working in the educational and medical centers of Hamadan. The sample size was calculated at 257 nurses. Data were collected using the Denison Organizational Culture Standard Questionnaire and the Meyer and Allen Standard Organizational Commitment Questionnaire. In this study, SPSS software (version 16) was used for data analysis, and a p-value less than 0.05 was considered statistically significant. Results: Based on the results, the organizational culture showed a significant correlation with organizational commitment (P <0.001,r = 0.56) and its dimensions, including emotional commitment (P<0.001, r =0.5), continuous commitment (P <0.001, r =0.4), and normative commitment (P <0.001, r =0.38). Moreover, the organizational commitment had a significant relationship with the dimensions of organizational culture, including involvement in work (P <0.001, r =0.48), adaptability (P <0.001, r =0.52), compatibility (P <0.001, r =0.5), and mission (P = 0.001, r =0.5). Conclusion: As evidenced by the obtained results, there was a correlation between organizational culture and dimensions of organizational commitment of nurses. Therefore, it is suggested that managers make wise decisions to promote organizational culture and organizational commitment in medical centers.
Objective: This study aimed to compare the effect of jainus syrup and Ondansetron syrup on nausea and vomiting during pregnancy. Methodology: The present study is a single-blind randomized clinical trial that was conducted on 120 pregnant women referred to the health centers of Hamadan during 2017-2018. The participants were randomly divided into two groups, control and intervention, with 60 subjects in each group. Two groups of 60 received jalinus syrup and Ondansetron syrup for 7 days. Data collection tools, demographic questionnaire, and information associated with pregnancy, and PUQE-24 questionnaire were used to assess the severity of nausea and vomiting, and questions related to the assessment of side effects as well asparticipants’ level of satisfaction with the treatment were provided. All the data were analyzed using the SPSS software version 16. P-value < 0.05 was considered significant. Results: The severity of nausea and vomiting in both groups decreased after the intervention, but considering the independent t-test, there was no significant difference between the two groups in terms of severity of nausea (P >0.05). According to the Chi-square test, the severity of nausea of all participants was moderate in both groups before taking the medication, and after taking the medication, the severity of nausea reduced in most participants and they were placed in the mild group. Conclusions: Due to the similar effects of jalinus syrup and Ondansetron syrup in the treatment of nausea and vomiting in pregnancy and the absence of side effects arising from its use, herbal medicine can be used instead of chemical drugs.
BACKGROUND:This study performed a meta-analysis to compare the safety and efficacy of argon plasma coagulation (APC) concerning suture (SUT) and surgery (SUR). METHODS:The One-Way tests were applied, with p <0.05 significant. The value of R2 was analyzed (heterogeneity), and the risk of bias of articles was evaluated. RESULTS:A total of 34 studies were found, eight about APC, twenty about SUT, and six about SUR. For the percentage of Weight Loss Regained (%WLR), the means for APC, SUT, and SUR were 19.39 ± 10.94, 26.06 ± 16.71, and 17.05 ± 9.85, and for success rate (%SR), the means were 89.75 ± 16.75, 86.80 ± 11.44, and 76.00 ± 21.57, respectively with p >0.05, with R2 ≥ 50%. CONCLUSION:The APC proved to be efficient and safe like the other techniques.
Background: The utilization of effective educational methods is crucial for enhancing the clinical skills of health sciences students. Objectives: This study aimed to assess the impact of a blended teaching approach involving videos and demonstrations in clinical skills training on midwifery students’ performance and satisfaction. Methods: This quasi-experimental study was carried out with 41 midwifery students from Hamadan University of Medical Sciences, Hamadan, Iran, using a census sampling method within 2018 to 2019. The experimental group received training through a combination of video presentations and demonstrations; nevertheless, the control group received training solely through demonstrations. The data were collected using a demographic questionnaire, a satisfaction scale, and the objective structured clinical examination (OSCE). Data analysis was performed using the Mann-Whitney U test and chi-square test with SPSS software (version 16). Results: The experimental group exhibited significantly higher mean scores in vital signs assessment, intravenous therapy, injection techniques, urinary catheterization, application and removal of sterile gloves, and prepping and draping for normal delivery (P < 0.001). There were no significant differences in the mean scores for hand washing (P = 0.380) and oxygen therapy (P = 0.604) between the two groups. The combined group achieved notably better scores in the OSCE, with scores of 18.25 (1.58) compared to 16.60 (1.60) in the control group (P = 0.002). Additionally, the combined group expressed higher satisfaction with the training method, scoring 86.10 (9.50) compared to 75.52 (16.07) in the control group (P = 0.027). Moreover, the combined group reported significantly higher levels of satisfaction regarding the facilitation of educational content transfer and overall satisfaction with the training course (P < 0.05). Conclusions: Employing instructional videos and demonstrations as a combined teaching method for basic clinical skills training results in improved student performance and satisfaction.
Introduction: Student satisfaction assessment adapts the structure of the university based on student needs assessment and provides a platform for continuous assessment of the quality of university services, the most important of which is teaching and learning.The aim of this study was to determine the level of satisfaction of nursing students with the Flipped class teaching method in internships. Methods: This cross - sectional study was conducted in 2017 with the participation of 40 nursing students with available sampling and census in Hamadan Midwifery Nursing School. The instrument of study was the researcher-made questionnaire of satisfaction with the internship teaching method. Data were analyzed using SPSS software version 16. Results: The means student satisfaction with the Flipped class teaching method (35.1±5.61 Min. SD) was with a minimum score of 30 and a maximum score of 38. 100% of students reported experiencing less anxiety in the Flipped class than the usual internship teaching method. 100% of students preferred the Flipped class teaching method to the usual teaching method. Conclusion: Students were satisfied with teaching the Flipped class method in the internship. Therefore, it is suggested that the Flipped class be considered more than ever in clinical education.
Introduction: Sleep disorder is one of the common disorders during pregnancy and it is important to identify effective interventions to improve the health of pregnant women. The present study was conducted aimed to determine the effect of acupressure on the sleep quality of pregnant women in the third trimester of pregnancy.Methods: This clinical trial study was conducted in 2018 on 60 pregnant women in the third trimester of pregnancy referring to the health centers in Hamedan city. The study tools were Pittsburgh Sleep Quality Index Questionnaire and Visual Linear Pain Scale Questionnaire. The subjects were placed in two groups of test and control. In the test group, acupressure was applied to real points for 3 weeks, and in the control group, acupressure was applied to false points for 3 weeks. Both groups completed the visual linear pain scale questionnaire before and after the intervention. Data were analyzed by SPSS statistical software (version 18) and Mann-Whitney, Fisher's exact and repeated measurement analysis. P<0.05 was considered as a significant level.Results: There was no significant difference between the two groups before the intervention in terms of sleep quality and other investigated variables (p=0.691). The effect of the intervention over time in improving the quality of night and daytime sleep was significant between the two groups (p<0.001), but the mutual effect of the intervention between the two groups did not show a difference in the duration of daytime sleep (p=0.534).Conclusion: Acupressure can improve the quality of night and day sleep in pregnant women in the third trimester of pregnancy, so this method can be used as an auxiliary method in improving the sleep disorder of pregnant women.
Background: Sexual dysfunction is one of the most common causes of family breakdown. In recent years, different approaches have been used to solve this problem. Due to the negative effects of sexual dysfunction in women and its complications. Objective: The present study was conducted to investigate the effect of stress management counseling on the sexual function of infertile women referred to Fatemieh Hospital in Hamadan. Methods: The present study was conducted as a randomized two-group clinical trial with two pretest and post-test stages on 104 eligible women referring to Fatemieh Hospital in Hamadan in 2017-2018. Intervention group subjects received 4 two-hour group counseling sessions with a cognitive- behavioral approach on sexual issues. Data were obtained using two questionnaires of demographic characteristics and the Female Sexual Function Index. To analyze the data, SPSS version 21 software and paired t-test, independent t-test, and analysis of covariance were used. The level of significance was considered to be less than 0.05. Results: The results of the present study showed that the mean scores in all areas of total sexual function did not differ significantly before the intervention in two groups. However, after stress management counseling, a significant increase was observed in all areas and general sexual function in the intervention group compared to the control group (P <0.05). Conclusion: Stress management counseling in infertile women can play a positive and effective role in improving sexual function. Therefore, the effectiveness of this counseling in promoting sexual function can be confirmed with more confidence. BACKGROUND Cervix preparation is one of the main steps in the onset of labor and is very important for success in initiating or inducing labor. OBJECTIVE The present study aimed at investigating the effect of evening primrose vaginal capsule on the preparation of cervix and the consequences of labor in nulliparous women. METHODS This randomized clinical trial study was conducted in 2018 on 100 nulliparous women referred to Fatemieh Hospital in Hamadan. I n intervention group (50 people) one evening primrose capsule (500 mg) was used vaginally and then two hours later, another capsule was used, and Placebo was used in the control group (50 people). Then, the dilatation, effacement, Bishop scores and duration of the first stage of the labor were compared in two groups after 4 hours. Statistical analysis was performed with SPSS 21. The significance level was considered to be 0.05. RESULTS There were a statistically significant difference in the mean score and standard deviation of dilatation, effacement of cervix, bi-shop scores and duration of first stage of labor four hours after the intervention in the in intervention and Placebo groups, by controlling the effect of potential confounding factors (P <0.001). CONCLUSION The results showed that the use of evening primrose vaginal capsule can reduce the length of the first stage of labor, improve Bishop Score, and soften and ripen the cervix.
Abstract Background High-risk pregnancy causes different responses, including negative emotions, feelings of vulnerability and psychological stress in the mother. The aim of this study was to investigate the effect of individual counseling on the empowerment and the perceived stress of high-risk pregnant mothers. Methods This study was a two-group experimental study. The study was performed on 82 high-risk pregnant women hospitalized in Fatemieh Hospital in Hamadan, Iran. Inclusion criteria were high-risk pregnancy, being literate, gestational age 24 to 36 weeks. The samples were divided into experimental and control groups using randomized block design. Data were collected using Cohen’s perceived stress scale and Kameda empowerment questionnaires. For the experimental group, four sessions of individual counseling according to GATHER principles (Greet, Ask, Tell, Help, Explain, and Return) were performed for 45–60 minutes for two consecutive weeks. SPSS 25 software was used for data analysis. Results The mean score of the perceived stress after the intervention in the control and experimental groups were 27.07(5.80) and 25.30(4.95), respectively (P = 0.097). There was a substantial difference in the mean score of empowerment 84.76)9.14) and 88.75 (6.17) (P < 0.001) and different dimensions of empowerment (self-efficacy, Future image, self-esteem, Support and assurance from others) between the control and intervention groups after the intervention. Conclusions The findings of this study indicate individual counseling is effective in empowering the mothers with high-risk pregnancy but has no significant effect on their perceived stress.
