BACKGROUND:The use of mental health services by police staff is usually low. After the 2018 attack on the Strasbourg Christmas market, police officers exposed to psychotraumatic risks were found to have a higher PTSD risk. This study aims to describe the help-seeking and substance use by police staff after the attack. METHODS:A questionnaire was sent to 1697 police officers 3 months after the attack. The 475 respondents were classified as exposed (took action to stop the terrorists, pursued them in the city, or helped a victim) or unexposed and indirect-exposed. Counselling, treatment, alcohol and tobacco consumption, and their habitus changes were assessed. Univariable analysis was followed by multivariable analysis adjusted for gender, age, number of previous traumatic events, level of exposure, and probable PTSD or depression, using logistic regression. RESULTS:Of the 475 respondents, 10% consulted a psychologist/psychiatrist, 24% a physiotherapist and 62% their general practitioner within 3 months of the assault. Initiation of new anxiolytic/antidepressant treatment (OR = 4.59 [1.11-18.91]), consultation with a psychologist/psychiatrist (OR = 5.02 [2.44-10.32]), increased alcohol or tobacco consumption (OR = 10.42 [3.45-31.43]) were associated with probable PTSD. CONCLUSIONS:The use of mental health services is insufficient for the care of probable PTSD, which is associated with increased tobacco or alcohol use, encouraging further research to improve the use of mental health services among police staff.
Objectif Les internes sont exposés à des conditions de travail exigeantes et à des facteurs de stress pouvant altérer leur santé et leur bien-être. Le sommeil apparaît comme une vulnérabilité particulière pouvant être ciblé par des mesures de prévention. Dans le cadre de la 2e édition du Baromètre santé des internes de la subdivision de Lyon (BASIL-2), nous avons évalué les troubles du sommeil et leurs déterminants. Méthodes L’enquête BASIL-2 a été proposée à l’ensemble des internes en médecine/pharmacie/odontologie de la subdivision de Lyon (février–mars 2025). Un questionnaire en ligne recueillait les données sociodémographiques, caractéristiques du travail, habitudes de sommeil, état de santé, et consommations. Il incluait des instruments validés : qualité du sommeil (PSQI), insomnie (ISI), somnolence (ESS), bien-être mental (WEMWBS), détresse psychologique (K6), et activité physique (IPAQ). Un calage sur marges (sexe, spécialité) a corrigé la représentativité de l’échantillon. Les analyses descriptives pondérées ont été réalisées, ainsi que l’analyse uni- et multivariée des facteurs associés à la qualité de sommeil et à la somnolence. Résultats Sur 2092 internes sollicités, 592 ont répondu (61,3 % de femmes, âge moyen 27 ans). La qualité du sommeil était altérée (PSQI>5) chez 63,6 % des répondants, 25,5 % rapportaient une somnolence diurne excessive (ESS>10) et 8,3 % une insomnie modérée à sévère (ISI>14). Alors que les besoins de sommeil moyens étaient estimés à 8h13min, 71,8 % des internes déclaraient dormir habituellement moins de 7h. En analyse multivariée, une qualité du sommeil altérée était associée au fait de vivre seul (OR=1,69, p=0,02), à une santé perçue négativement (OR=2,41, p=0,0002), aux plaintes cognitives (OR=2,88, p=0,002), à la détresse psychologique (OR=4,64, p=0,03), et à la consommation fréquente d’anxiolytiques/somnifères (OR=9,88, p<0,0001). La somnolence était associée à la spécialité (p=0,03), au nombre d’heures hebdomadaires de stage (OR=1,03, p=0,01), à la dette de sommeil (OR=1,01, p=0,001), à une santé perçue négativement (OR=2,7, p=0,02), aux plaintes cognitives (OR=2,87, p=0,02), et à la consommation occasionnelle de somnifère/anxiolytique (OR=2,03, p=0,04). Conclusion Les troubles du sommeil sont fréquents chez les internes, et associés à des facteurs professionnels, comportementaux, ainsi qu’à leur santé. Ces résultats soulignent la nécessité d’actions de prévention ciblant le sommeil dans cette population.
Abstract Background Hospital professionals face mental health challenges caused by the high demands of their work. Mindfulness meditation (MM) is a nonpharmacological practice with promising effects in promoting mental health and well-being across various hospital professionals. However, the benefits obtained from its implementation in the context of real work in hospital departments are understudied. Objective The aims of this pilot study are to implement and evaluate, at both the individual and collective levels, the exploratory outcomes of a workplace-based mindfulness intervention developed participatively for hospital teams in a French university hospital. The feasibility of the implementation of the intervention, the influence of the work environment, and the mechanisms through which it produces its effects will also be assessed. Methods For this prospective interventional study, a preliminary co-construction with the stakeholders (a task force with professionals and management) made it possible to develop and adapt the intervention to the needs and capacities of the professionals, using the intervention mapping model for health program planning. Inclusion and an MM information meeting were proposed to 8 hospital departments, which were then randomized into 2 parallel cluster groups (intervention or control group). In the intervention group, an MM test session was proposed before committing to the MM program (10 one-hour sessions) for a maximum of 15 professionals per department. A convergent mixed methods design with concomitant quantitative and qualitative data collection was used, before integration of findings. Quantitative evaluation was based on a comparison of “before-after” measurements, with a comparison between intervention and control groups. The primary exploratory outcome was the evolution of psychological flourishing at work, which was measured using the adapted and validated French-language scale Psychological Flourishing at Work (EEPMT-8), to which the Professional Life Satisfaction Scale (ÉSVP-5) was added, to form a new 13-item Psychological Flourishing at Work Scale (EEPMT-13). Realist evaluation by qualitative methods explored the influence of contexts on the implementation (barrier and facilitators), and the links between context, mechanisms, and effects of MM. The study was approved by the Sud-Est VI ethics committee (Clermont Ferrand, France). Results Funding was obtained in October 2021 and June 2024. This study was conducted from May 2024 to June 2025. As of May 2024, 108 participants were enrolled. Analyses are in progress. Results are expected to be published by the end of 2026. Conclusions Participative approaches are essential for a challenged, tailored implementation of a workplace-based mindfulness intervention in hospital teams. The evaluation of the intervention with mixed methods and the characterization of both individual and organizational levels are aligned with the recommendations to evaluate a complex intervention in a workplace; challenges include the use of an innovative primary endpoint and methodological pluralism to plan for scalability at a subsequent stage.
Over the past two decades, fatal workplace accidents have accounted for between 300,000 and 400,000 deaths a year worldwide. The aim of this study was to investigate the general characteristics and the toxicological profile of work-related deaths. A descriptive retrospective study was carried out on occupational-related deaths occurred in the Lyon metropolitan area from 2000 to 2020 from the autopsy reports of the University Institute of Legal Medicine. A total of 476 cases of work-related deaths were identified, of which 91% were men. The median age was 48 years, 44% of the deaths were due to cardiac origin, 34% to mechanical accidents and 18% to suicide. 16 homicides were also recorded. The cause of death differed significantly between socio-professional categories: suicide was the main cause of death among managers & intellectual occupations (50%), cardiovascular death among non-manual elementary workers (53%), manual elementary workers (49%) and craftsmen, shop keepers & business owners (33%), and physical accidents among farmers (50%) and manual elementary workers (48%). The main cause of death varied by sector: physical accidents in construction (61%), cardiovascular events in transportation and storage (67%) and manufacturing (51%). 27% (n = 130) tested positive for at least one psychoactive substance, including 75 for alcohol, 43 for sedative anxiolytics and 33 for cannabis. These results may help occupational health professionals to design policies and campaigns to prevent deaths among the workers concerned. Specific studies to assess the proportion of fatal accidents attributable to the use of alcohol, cannabis or benzodiazepines could contribute to reducing work-related deaths.
Objectif Les internes sont exposés à des conditions de travail exigeantes incluant les gardes de nuit, susceptible d’altérer leur sommeil. Dans la 2e édition du Baromètre santé des internes de la subdivision de Lyon (BASIL-2), nous avons évalué les troubles du sommeil et l’accidentologie associés aux gardes. Méthodes L’enquête a été proposée aux internes en médecine/pharmacie/odontologie entre février et mars 2025. Un questionnaire en ligne recueillait des données sur les gardes (fréquence, durée, repos de sécurité), la gestion du sommeil par rapport aux gardes, la somnolence (ESS) et l’accidentologie. Un calage sur marges (sexe, spécialité) a corrigé la représentativité de l’échantillon, permettant des analyses descriptives pondérées et l’analyse des facteurs associés à la somnolence et à l’accidentologie. Résultats Sur 2092 internes sollicités, 592 ont répondu (61,3 % de femmes, âge moyen 27 ans). Parmi eux, 72,1 % réalisaient des gardes (moyenne 16/semestre, en général 14h après une journée de travail). En garde, 66,8 % dormaient<3h, avec un repos de sécurité respecté chez 88,5 %. Après une garde, 82,1 % dormaient en journée (moyenne 3h55min), 35,4 % se couchaient plus tôt le soir suivant. La plupart des internes ne changeaient pas d’habitudes de sommeil avant une garde de nuit mais 35,4 % rapportaient avancer le coucher et 14,4 % faire une sieste anticipatrice. En garde, les principales stratégies pour stimuler la vigilance étaient le café (86,6 %) et l’alimentation (55,1 %). Concernant l’accidentologie, 14,6 % déclaraient avoir eu un accident de la route depuis le début de l’internat (21,5 % pendant/après une garde/astreinte, 20 % liés à la somnolence) et 27,9 % un accident d’exposition au sang (AES) (30,1 % pendant une garde de nuit). La fréquence des gardes était associée aux accidents routiers (p=0,003) et aux AES (p=0,004). Le risque d’AES dépendait de la durée du sommeil en garde (risque minimal si durée nulle ou>6h, p=0,004). La somnolence était moindre chez les internes adoptant des stratégies de sommeil anticipatrices (p=0,03) et ceux dormant plus longtemps après une garde (p=0,047). Conclusion Les gardes de nuit exposent les internes à une restriction de sommeil et à une accidentologie accrue. Nos résultats soulignent l’intérêt de stratégies de prévention ciblant l’anticipation et la récupération du sommeil.
STUDY OBJECTIVES:Narcolepsy type 1 (NT1) is increasingly recognized as a multidimensional disorder in which psychiatric and psychosocial difficulties contribute to disability. Yet little is known about how these difficulties are perceived by patients compared with close informants. This study examined discrepancies between self-reported and informant-reported psychobehavioral symptoms in adults with NT1. METHODS:Within the multicenter French NarcoScol-NarcoVitae cross-sectional case-control study, 174 adults with NT1 (18-58 years) and 126 age- and sex-matched non-relative controls completed the Adult Self-Report (ASR), a standardized measure of adult psychobehavioral problems. A close informant completed the parallel Adult Behavior Checklist (ABCL), yielding 97 NT1 and 96 control ASR-ABCL dyads. T-scores were computed for internalizing and externalizing scales, and the total problems score. Between-group differences (NT1 vs non-relative controls) and within-individual discrepancies between self- and informant-reported symptoms were analyzed. Narcolepsy severity (NSS), sleepiness (ESS), age at diagnosis, and a composite professional prognosis score were compared across concordant and discordant dyads. RESULTS:Patients with NT1 reported significantly higher psychiatric symptom severity than controls across all ASR scales, with 44.8% versus 19.1% in the clinical range for internalizing difficulties. Informants generally rated patients' symptom severity as lower than patients' self-reports, particularly for internalizing symptoms. Discordant dyads in which informants rated internalizing symptoms as more severe than patients were characterized by lower NT1 symptom burden (NSS). Concordant patient-informant evaluations were associated with a more favorable professional prognosis. CONCLUSION:Adults with NT1 report substantial psychobehavioral impairments that are not consistently identified by close informants, especially internalizing difficulties. This perception gap highlights the complementary value of patient and informant perspectives and calls for improved recognition of less visible symptoms within patients' close environment. Contexte Depuis les années 2010, le développement rapide des plateformes de livraison en France et à l’international a fait évoluer certaines formes d’organisation du travail urbain. Cette activité est majoritairement exercée par une main-d’œuvre étrangère. L’organisation des livraisons demeure peu documentée et repose sur des algorithmes prédéfinis, dont la prise en compte des contraintes humaines et environnementales apparaît limitée. Cette situation restreint l’analyse des conditions de travail et de leurs effets sur les professionnels de ce secteur en forte expansion. Méthode Le projet ERROLI a débuté en octobre 2025 à Paris et à Lyon. L’étude reposait sur le recrutement de livreurs directement sur 26 lieux d’exercice définis. Des analyses univariées, à partir des données d’enquête quantitative en face à face, ont permis de décrire l’organisation et les conditions de travail des livreurs. Résultats Les analyses ont été effectuées sur 1005 livreurs âgés en moyenne de 28,5 ans, presque tous des hommes. Pour l’ensemble des deux villes, 48 % des livreurs louaient le compte d’un tiers. Les déplacements étaient effectués principalement à vélo à assistance électrique (50 %), devant le vélo mécanique (23 %) et le deux-roues motorisé (20 %). Malgré des conditions de travail jugées contraignantes par les participants, 66 % se déclaraient satisfaits de leurs revenus. Ceux-ci se situaient majoritairement entre 1000 et 2000€/mois. L’activité quotidienne moyenne correspondait à 17 livraisons, 51,5km parcourus et 8,3heures de travail par jour, 6 jours par semaine.Des différences existaient entre Paris et Lyon. À Paris, la location de compte était plus fréquente (74 % contre 21 %) et l’ancienneté plus faible (16 % au-delà de 2 ans, contre 41 %). Les difficultés relationnelles avec les clients (66 %) et la plateforme (70 %), ainsi que les horaires contraignants (82 %), y étaient plus souvent rapportés qu’à Lyon (respectivement 37 %, 58 % et 76 %), pour des revenus principalement compris entre 1000 et 1500 € (61 %), plus faibles qu’à Lyon (58 % entre 1500 et 2000 €). Les distances quotidiennes parcourues étaient supérieures à Lyon (54km contre 49km), pour une charge de travail comparable. Conclusion Les livreurs urbains présentent des rythmes de travail intenses et des conditions organisationnelles hétérogènes, associant contraintes physiques, horaires longs et relations professionnelles souvent difficiles, soulignant la nécessité d’un encadrement adapté.
Objectif Les gardes de nuit exposent les internes à des prises de décisions médicales lors de réveils nocturnes. L’inertie du sommeil est physiologiquement majorée en cas de privation de sommeil, de réveil en sommeil lent profond, au creux circadien, et chez les sujets jeunes, soit typiquement les conditions des éveils en garde. Cette étude visait à estimer la prévalence des épisodes confusionnels (EC) en garde, à décrire leurs conséquences et à identifier les facteurs associés. Méthodes Dans le cadre du Baromètre santé des internes de la subdivision de Lyon-2 (BASIL-2), un questionnaire en ligne a été adressé aux internes en médecine/pharmacie/odontologie entre février et mars 2025. Les questions concernaient les antécédents d’éveils incomplets avec comportement confus en garde/astreinte et leur description, les comportements de sommeil, et la présence de troubles associés (insomnie, somnolence, chronotype, inertie, parasomnies). Un calage sur marges (sexe, spécialité) a corrigé la représentativité de l’échantillon. Les analyses descriptives pondérées ont été réalisées, ainsi qu’une analyse uni- et multivariée des facteurs associés aux EC. Résultats Sur 2092 internes sollicités, 592 ont répondu (61,3 % de femme, âge moyen 27 ans) : 20 % rapportaient avoir présenté au moins un (dont 69,7 % plusieurs) EC, majoritairement en milieu de nuit (76,3 %). La durée dépassait 2min chez 34,1 %, et 33,2 % n’en prenaient conscience que le lendemain. Les manifestations incluaient désorientation, altération de la compréhension/mémorisation, réponses incohérentes, erreurs de jugement. Dans 8,5 % des cas, ces épisodes avaient impacté la prise en charge (retard, erreur diagnostique ou de prescription). Par ailleurs, 12,7 % présentaient des parasomnies du sommeil lent profond (77,1 % éveils confusionnels, 44,2 % terreurs nocturnes, 21,5 % somnambulisme). En analyse multivariée, les EC en garde étaient associés à la dette de sommeil (OR=1,3 par heure de dette de sommeil, p=0,03) et au chronotype du matin (OR=2,2, p=0,04). Le nombre de gardes était plus élevé durant la période de survenue de l’EC que durant la période actuelle (26,5[23,9–29,1] versus 16,8[14,6–19,0]/semestre). Conclusion Les EC lors des gardes nocturnes sont fréquents chez les internes. Leurs conséquences potentielles sur la prise en charge des patients et leur lien avec la dette de sommeil soulignent la nécessité d’actions de prévention adaptées.
Introduction Les données disponibles dans la littérature internationale suggèrent une altération de la qualité du sommeil chez près de la moitié des internes [1], [2], ce qui semble corroboré par les chiffres rapportés en France dans le cadre de thèses de médecine [3], [4], [5]. Méthode La deuxième édition du baromètre santé des internes de Lyon (BASIL-2) a été réfléchie dans la continuité de BASIL-1 qui avait pour objectif d’évaluer l’état de santé des internes. Cette évaluation périodique dans le domaine de la santé mentale, des conditions de formation et de la qualité de vie au travail des internes de Lyon s’inscrit dans le cadre du plan d’action institutionnel. Le développement de BASIL-2 a impliqué le Service de médecine et santé au travail (Hospices civils de Lyon), le Service de santé étudiante (Université Claude Bernard Lyon 1), des représentants des institutions, des médecins, des enseignants-chercheurs et des internes dans un processus de co-construction. Résultats 593 internes (69 % de femmes) ont répondu à l’enquête en ligne diffusée pendant 6 semaines à partir de février 2025. 52 % travaillent plus de 48heures par semaine, 72 % effectuent des gardes et 18 % des astreintes. 70 % estiment leur état de santé à bon à très bon, 21 % à moyen et 9 % à mauvais à très mauvais. 9 % consomment des anxiolytiques plusieurs fois par mois et 7 % des somnifères. Selon l’Index de Qualité du Sommeil de Pittsburgh, 63 % des internes présentent une qualité de sommeil altérée. Un quart des internes présente une somnolence diurne excessive selon l’Échelle de somnolence d’Epworth. Enfin, 8 % présentent une insomnie clinique selon l’Index de Sévérité de l’Insomnie. Des premières analyses bivariées montrent un lien entre ces indicateurs de sommeil et les conditions de travail, la détresse psychologique selon Kessler et la prise de médicaments. Discussion Au moins deux tiers des internes présentent une qualité de sommeil altérée. La prévalence d’un sommeil altéré augmente à la comparaison avec différentes enquêtes en France (37 % en 2015, 58 % en 2018 et 63 % dans cette étude). Des analyses complémentaires sont nécessaires afin d’identifier le rôle des conditions de travail, de la formation et des habitudes de vie dans ces troubles de sommeil. Le sommeil est un double enjeu de santé publique, tant sur la santé des internes que sur la sécurité des soins pour les patients. Des actions de promotion de la santé ciblant le sommeil dans cette population devront être mises en place.
Objectif Les conditions de travail des internes comportent des contraintes professionnelles pouvant générer du stress et être associées à des troubles du sommeil : rythme de travail avec longue amplitude horaire, pression temporelle, horaires atypiques, charge émotionnelle/mentale élevée, responsabilités humaines dans un environnement compétitif. L’objectif de l’étude est d’évaluer auprès des internes leur état de santé, en particulier la qualité et les troubles du sommeil au regard de ces conditions de travail. Méthodes Une enquête transversale observationnelle a été mise en œuvre du 14/02–30/03/25 auprès des internes de la subdivision de Saint-Étienne (enquête jumelle de l’observatoire lyonnais BASIL-2). Les données ont été collectées à partir d’un auto-questionnaire anonyme en ligne (logiciel Limesurvey). Des échelles validées ont été utilisées pour évaluer le sommeil (la qualité du sommeil (PSQI), la somnolence (ESS), l’insomnie (ISI)), l’état de santé mentale (détresse psychologique [Kessler Psychological Distress Scale]), bien être mental de Warwick-Edinburgh (WEMWBS)) et l’hygiène de vie (activité physique [IPAQ]), consommations dont alcool (AUDIT-C). Résultats Parmi les 241 internes ayant répondu à l’enquête sur 806 internes éligibles, 70,5 % sont des femmes, d’âge moyen 27 ans. La quotité moyenne du temps de travail est évaluée à 49h30min (ET=12,2). La qualité de sommeil est altérée pour 45 % des internes, avec une latence de sommeil élevée (2/3 des internes),une durée de sommeil inférieure à 7h pour 30 % des internes tandis que la durée moyenne du besoin de sommeil estimé est de 8h. Une insomnie modérée à sévère est présente auprès de 13,6 % des internes. Une somnolence diurne excessive concerne un tiers des internes et a déjà été en cause dans un accident de trajet chez 30 % d’entre eux. Près de la moitié des internes présentent une détresse psychologique modérée à sévère, avec une consommation de psychotropes (antidépresseur 10,8 %, anxiolytiques 22 %). Conclusion Ces résultats descriptifs soulignent l’intérêt de favoriser auprès des internes, la mise en œuvre de mesures de prévention dans les domaines de la gestion du stress, de l’hygiène du sommeil et de la prévention de l’accidentologie. Des analyses complémentaires permettront d’identifier les facteurs associés aux perturbations du sommeil, afin de cibler les mesures de promotion du sommeil.
Objectives: In this multi-center cross-sectional study, we compared substance use patterns (SUPs) between patients with narcolepsy type 1 (NT1) and controls, and investigated, among patients, factors associated with the consumption of the main psychoactive substances. Methods: Adult patients with NT1 and controls completed questionnaires about tobacco, alcohol, and cannabis use patterns. Unadjusted bivariable then multivariate analyses (adjusted for sex, age, education, family status, and depression) were performed to compare SUPs between controls and patients, and to explore sociodemographic, psycho-behavioral, and clinical determinants of consumptions. Results: We included 235 patients (63.8 % women, 36.4 +/- 14.7 years) and 166 controls (69.9 % women, 40.3 +/- 14.4 years). Substances co-consumptions were frequent in both groups. Patients with NT1 were more frequently current smokers (32.3 % vs. 20.1 %, p < 0.01) ore-cigarettes users (12.1 % vs 2.4 %, p < 0.001) than controls, while no difference was observed for cannabis use and alcohol misuse. Only the increased likelihood of vaping remained significant in adjusted analysis. Among NT1 patients, smoking was associated with disrupted nighttime sleep (OR[95%CI] = 2.28[1.02-5.12], p < 0.05) and less obesity (OR = 0.24[0.09-0.59], p < 0.05). Alcohol misuse was associated with sleep paralysis (OR = 2.11[1.13-3.91], p < 0.05) and treatments (modafinil: OR = 2.14[1.15-4.01], p < 0.05; sodium oxybate: OR = 0.41[0.17-0.97], p < 0.05). Tobacco and cannabis consumptions were associated with lower physical activity (OR = 0.46 [0.24-0.87], p < 0.05 and OR = 0.25 [0.10-0.66], p < 0.01). Alcohol misuse and cannabis use were associated with rule breaking behaviors (OR = 5.89[1.61-21.60], p < 0.05 and OR = 8.52[1.79-40.48], p = 0.01). Conclusion: Patients with NT1 do not seem less vulnerable to psychoactive substance use/misuse. Consumptions patterns are associated with multiple dimensions of the disease including sleep-related symptoms, comorbidities, treatments, and psycho-behavioral factors.
IntroductionAccording to the 2018–2019 People Mobility Survey, work-related journeys (commuting and on-duty journeys) account for approximately 25% of all journeys. The use of non-motorised (nm) and motorised (m) personal mobility devices (PMDs) has steadily increased since their introduction into the French market in the last decade.ObjectiveThis study aimed to describe the characteristics of work-related road accidents and their evolution since the introduction of new PMDs in France and the increase in the use of scooters.Materials and methodsThis was a retrospective, cross-sectional study using data from the Rhône Road Trauma Registry. Data were collected from 2015 to 2020. We included the data for the victims aged 18–70 years who were injured in work-related road accidents.ResultsWe identified 11 296 individuals aged 18–70 years who experienced work-related road accidents. An injury report was provided for a total of 11 277 patients. A total of 546 passengers and 78 drivers of other motorised vehicles (buses/trams, construction equipment and tractors) were excluded from the analysis. Seven patients died at the time of the accident and seven died after hospitalisation. Of the 10 653 (94.4%) victims, there were pedestrians (5.1%) or riders of bicycles (16.9%), scooters (3.8%), other PMDs (roller blades, skateboards, monowheels, gyropods and hoverboards; 0.4%) and motorised two wheelers (21.4%), or drivers of car (45.3%), and truck (1.5%). More than half of the scooter riders and 80% of other PMD riders were men. More than 60% of other PMD riders and 53% of scooter riders were under 34 years of age. Most scooter road accidents occurred during commuting (95.6%). 65% of the scooter accidents and 50% of other PMD accidents did not have opponents. Overall, one-quarter of the victims experienced accidents without opponents. Most scooter riders had injuries to their upper limbs (59.2%), lower limbs (46.8%), face (21.2%) or head (17.9%).DiscussionThis original study on work-related road accidents allowed us to characterise the increase in work-related road accidents associated with new modes of travel, particularly scooters. The results observed for users of scooters and other PMDs in this study were generally consistent with those found in the scientific literature. Despite limited data, the results suggest that accidents involving scooters or other PMDs are of low severity.ConclusionMany head injuries could be prevented with more widespread use of helmets, among scooter and other PMD users and bicycle users.
This multicenter comparative cross-sectional study aimed to describe educational and occupational pathways, quantify effort/reward imbalance at work, and identify factors associated with professional prognosis in patients with narcolepsy type 1. Adult patients with narcolepsy type 1 and controls answered online questionnaires (Epworth Sleepiness Scale, Beck Depression Inventory II, Siegrist questionnaire, adult self-report, and a questionnaire on academic and professional trajectories) and were compared using sex- and age-adjusted logistic regressions. Les policiers ont pour mission de lutter contre toutes les formes de violence. À ce titre, ils sont exposés à deux sources de risques professionnels :– les risques liés aux interventions : les agressions ou expositions à des scènes violentes qui peuvent être à l’origine de psychotraumatismes et ou de blessures physiques,– les risques liés à l’organisation et aux relations au travail, tels que les horaires de travail et les conflits vie privée/vie professionnelle, la qualité des relations au travail et les questions éthiques.Les difficultés touchant la santé mentale sont principalement : la dépression, l’anxiété et les troubles de stress posttraumatique. Les données présentées sont issues de la littérature internationale et de deux études menées en France : l’une après les attentats de Strasbourg de décembre 2018 et l’autre en période de coronavirus (été 2020). Pour la dépression, les prévalences dans la littérature sont très variables, allant de 0,95 % à 55,4 %. Cette variabilité s’explique probablement par le pays, la méthode, les outils utilisés et le type de service interrogé. Dans nos deux études, le taux de dépression était de 11 % pour la première après l’attentat et de 32 % en période de la pandémie du coronavirus 19. Concernant le trouble de stress post-traumatique, la littérature montre également des prévalences variables, mais moindres. Ainsi, après un attentat, la littérature montre des chiffres variants entre 1,3 % et 16,5 %. La méthode, le type de service et l’ampleur de l’attentat étudiés dans l’étude semblent être les principales explications à cette variabilité. À Strasbourg, la prévalence des TSPT était de 3 % pour les TSPT complets et de 9 % pour les TSPT partiels pour l’ensemble des policiers exposés ou non exposés à l’attentat, et de 6,6 % et 12,6 % pour les policiers directement exposés à l’attentat. Aux véritables dépressions et troubles de stress post-traumatiques s’ajoute l’accès à l’arme, qui expose les policiers à un passage à l’acte suicidaire. En France, nous n’avons pas, ou très peu, de données quantitatives. Une revue de la littérature internationale de 2018 concluait que les résultats des études étaient très variables. Certaines montraient des taux de suicides moindres, d’autres étaient égaux ou plus élevés que la population générale du pays. Quel que soit le chiffre, il est nécessaire de tout mettre en œuvre pour prévenir la survenue d’un suicide. La prise en charge des dépressions et des troubles de stress post-traumatiques fait partie de la prévention du suicide. En termes de prévention des psychopathologies, deux niveaux sont particulièrement importants : le développement du soutien du collectif et de l’administration après une intervention ou un événement difficile d’une part, et le repérage et l’orientation vers le milieu de soins spécialisés des policiers en difficultés d’autres part. Dans la police, encore plus qu’ailleurs, pour différentes raisons, le recours aux soins spécialisés en santé mentale est compliqué, et la lutte contre la stigmatisation toujours d’actualité.
Background and objectives Narcolepsy type 1 (NT1) is a chronic, disabling neurological disease. Sleep-related symptoms and comorbidities such as psycho-cognitive disturbances, and a frequent childhood onset of the disease may negatively impact patients’ career. We conducted a multicentric comparative cross-sectional study in Reference/Competence Centers for Narcolepsy in France to investigate the educational and occupational paths of patients with NT1. Methods Between February 2020 and 2023, adult patients with NT1 regularly followed-up in the participating centers were invited to complete online questionnaires including the Epworth sleepiness Scale, Narcolepsy Severity Scale, Beck Depression Inventory II, Siegrist questionnaire, Adult Self-Report and Adult Behavior Checklist, and a customized questionnaire on academic and professional trajectories. Controls were selected from within the patients’ close circle. Comparisons were adjusted for sex and age, and the determinants of patients’ professional prognosis were quantified by a composite score including professional-related outcomes. Results Questionnaires were filled by 235 patients (63.8% women, 36.4±14.7 years, 86.5% treated, 66.4% with childhood onset) and 166 controls (69.9% women, 40.3±14.4 years). No difference was observed between patients and controls for grade repetition and graduation level distribution, but patients reported more interruptions in their scolarity which was considered difficult, with more absenteeism and lateness. No difference was observed for employment rate (69.5% vs 77.0%) and socio-professional category distribution, but income was lower in patients who reported more unwanted changes in position and part-time work, with increased effort-reward imbalance (OR=2.28 95% CI [1.20-4.33], p=0.01). Almost half of the patients benefited from an official disability recognition and 10.2% received invalidity benefits. Impaired professional prognosis was associated with depression (p<0.0001) and attention disorders (p=0.03), while being narcoleptic during schooling was a protective factor (p=0.02). Residual sleep-related symptoms were not significant predictors. Discussion Most patients with NT1 manage to achieve their careers goals, but at the cost of an effort/reward imbalance. Early diagnosis during childhood might allow a better adjustment to the disease. BackgroundThe endocrine-disrupting effects of phytopharmaceutical active substances (PAS) on human health are a public health concern. The CIPATOX-PE database, created in 2018, listed the PAS authorized in France between 1961 and 2014 presenting endocrine-disrupting effects for humans according to data from official international organizations. Since the creation of CIPATOX-PE, European regulations have changed, and new initiatives identifying substances with endocrine-disrupting effects have been implemented and new PAS have been licensed.ObjectivesThe study aimed to update the CIPATOX-PE database by considering new 2018 European endocrine-disrupting effect identification criteria as well as the new PAS authorized on the market in France since 2015.MethodsThe endocrine-disrupting effect assessment of PAS from five international governmental and non-governmental initiatives was reviewed, and levels of evidence were retained by these initiatives for eighteen endocrine target organs.ResultsThe synthesis of the identified endocrine-disrupting effects allowed to assign an endocrine-disrupting effect level of concern for 241 PAS among 980 authorized in France between 1961 and 2021. Thus, according to the updated CIPATOX-PE data, 44 PAS (18.3%) had an endocrine-disrupting effect classified as “high concern,” 133 PAS (55.2%) “concern,” and 64 PAS (26.6%) “unknown effect” in the current state of knowledge. In the study, 42 PAS with an endocrine-disrupting effect of “high concern” are similarly classified in CIPATOX-PE-2018 and 2021, and 2 new PAS were identified as having an endocrine-disrupting effect of “high concern” in the update, and both were previously classified with an endocrine-disrupting effect of “concern” in CIPATOX-PE-2018. Finally, a PAS was identified as having an endocrine-disrupting effect of “high concern” in CIPATOX-PE-2018 but is now classified as a PAS not investigated for endocrine-disrupting effects in CIPATOX-PE-2021. The endocrine target organs associated with the largest number of PAS with an endocrine-disrupting effect of “high concern” is the reproductive system with 31 PAS. This is followed by the thyroid with 25 PAS and the hypothalamic–pituitary axis (excluding the gonadotropic axis) with 5 PAS.DiscussionThe proposed endocrine-disrupting effect indicator, which is not a regulatory classification, can be used as an epidemiological tool for occupational risks and surveillance.
Abstract Introduction In the last decade, in several countries, the use of personal mobility devices (PMDs) has steadily increased. Objective This study aimed to describe the characteristics of work-related road accidents and their evolution since the introduction of new PMDs in France, in terms of incidence and severity. Methods A cross-sectional study was performed using data from the Rhône Road Trauma Registry, focusing on users aged 18–70 years who were injured in work-related road accidents between 2015 and 2020. Results 10,653 victims were identified. An increase in the number of work-related traffic injuries was observed, mainly due to an increase in the number of accidents while commuting (approximately 4%). The proportion of scooter drivers increased by more than 660%. In contrast, the share of car drivers decreased by 21% over the period, and motorized two-wheeler drivers by 24%. Most scooter users had upper limbs (59.2%), lower limbs (46.8%), face (21.2%) or head (17.9%) injuries. Most accidents were of low or moderate severity (97.9%; MAIS score < 3) compared to other users (93.4% for pedestrians, 95.5% for motorized two-wheelers, 97.5% for bicycle, p<0.0001). Less than one-fifth of injured PMD and scooter users wore helmets. Discussion The use of PMDs has steadily increased in France. However, accidents involving scooters or other PMDs seem to be of low severity. Conclusion Many head injuries could be prevented with more widespread use of helmets, among scooter- and other PMD users and bicycle users. Companies could take preventive actions to ensure that employees are better informed.
Background: In a working environment that is predominantly male, very tough physically, with a difficult working environment, occupational exposures and working, verbal and physical aggression can be more frequent than in other sectors. Fishing, merchant shipping and yachting are all sectors where fitness to sail is reassessed every year by doctors in the Seafarers' Health Service. Seafarers are increasingly reporting insulting, violent or sexist behaviour. The main types of abuse seen on board can be verbal and/or physical aggression, humiliation, whether in private or in front of others. Sexual harassment of women is a very worrying subject. Materials and methods: It was a retrospective observational study which is part of the professional monitoring of seafarers. The target population was adult seafarers coming for a fitness to sail visit. The group was recruited from seafarers aged over 18 who were being monitored by one of the seafarers' health services (or local centres). The inclusion period was 4 months between January and April 2023. All the information was collected using a self-questionnaire developed from the questionnaires of the Surveillance M & eacute;dicale des Expositions des Salari & eacute;s au Risques Professionnels (SUMER) for health status, job satisfaction and the European mini-module, verbal and physical aggression and psychological violence at work (based on the Leymann questionnaire), sexual violence and aggression based on the sexual harassment questionnaire and the PCLS-5 scale validated and translated into French to assess post-traumatic stress. The population studied was therefore 788 sailors. Results: The study population was predominantly male (82.3%). The average age was 41.4 years (standard deviation = 11.7). 46.7% of seafarers estimate being in very good health. During the past 12 months, overall, 24.5% of seafarers disclaimed having been victim in work-related context of a verbal aggression, with a significant difference according to the gender (21.1% for men and 41.0% for women). During the last 12 months, overall, 3.2% of seafarers have been victim in work-related context of a physical aggression (2.6% for men and 5.8% for women, NS), whereas 10.9% of seafarers reported hostile behaviour at present. Twenty per cent of seafarers reported sexual harassment in the last 12 months. During the entire working life of seafarers, 65.5% of women and 38.2% of men reported sexual harassment, and 38.8% of seafarers stated that they had experienced at least one traumatic event in the last 12 months. Discussion: A quarter of seafarers say they have been the victim of verbal or physical aggression at work in the last twelve months. These figures are high, and higher than those of the global survey on violence and harassment at work carried out by the International Labour Organization. One of the most alarming results of our study is the overexposure of women working in the maritime industry to the risk of physical, verbal or sexual assault of any kind. Indeed, in all the questions concerning the experience of verbal and physical aggression, the number of women victims is twice as high as that of their male colleagues, and this difference is statistically significant. As far as prevention is concerned, it seems that an effort is needed in terms of information, since only one seafarer in two knows the procedure to follow in the event of aggression within their shipyard. Communication between shipowners and seafarers needs to be stepped up to ensure that everyone is familiar with the procedures.
