OBJECTIVES To prove the biocompatibility of biomaterials applied in biomedical devices, in vitro testing is crucial to render a material fit for medical application. The material of choice for dental implants is commercially pure titanium (cp-Ti), while other materials such as zirconia and polyetheretherketone (PEEK) are considered highly promising due to their functional and esthetic properties. The aim of this study was to determine whether PEEK with defined mean surface roughness and composition could achieve results equal to titanium or zirconia. MATERIALS AND METHODS Disks measuring 14 mm in diameter and 1 mm in thickness made from cp-Ti, yttria-stabilized zirconia (Y-TZP), and filled PEEK with a smooth surface finish were used for cell culture experiments. Human fetal osteoblasts (hFOB) were cultured in vitro on each material to observe changes after 1, 3, and 7 days regarding cell viability and lactate dehydrogenase (LDH) release. Additionally, mRNA expression of proliferative factors PCNA and Ki67 and cellular adhesion (vinculin mRNA expression and immunofluorescence staining) were analyzed after 3 days in the culture. RESULTS In hFOB cultures, adhesion and viability were decreased on PEEK platelets, while LDH release remained stable. No significant difference was observed in cp-Ti and Y-TZP when compared to the control. CONCLUSIONS The performance of cp-Ti and Y-TZP was equal to the control in all tests. It seems that highly polished PEEK in this particular composition cannot be recommended for osseointegrated implant applications due to decreased osteoblast attachment. Further investigations are recommended, especially in surface structures optimized for osseointegration.

Dental materials, especially in restorative dentistry, must not only be cost-effective, they must also meet many other requirements: biocompatibility, durability, excellent shade and light effects, abrasion resistance, hardness, mechanical strength under pressure, chemical resistance, surface density, ease of manufacture, and easy intraoral maintenance.
Acid etching and filled acrylic materials have been generating great interest in the past few years because of increased public demand for esthetic restorations and alternatives to amalgam. Composite resin and ceramic inlays, which use adhesive bonding, were introduced to make possible toothcolored restorations. The physical properties of composite resin restorations, and their poor wear resistance, have been known for a considerable time.1· 2 While heat-treated composite resin inlays exhibit significantly better homogeneity and marginal adaptation than conventional intraorally cured inlays, poor abrasion resistance and rapid degradation of the marginal integrity raised doubts as to their suitability for occlusal restorations. According to Marolf, composite resin inlays have a service life of 3 to 5 years. Whether composite inlays are suitable for occlusal restorations is still a controversial subject, especially since no longitudinal studies have been carried out to date. 3-? That exact primary marginal fit is possible was proved in numerous experimental and clinical studies.s-10 However, even improved systems cannot prevent disintegration at the inlaybonding composite interface after subjection to thermal and mechanical stresses or several months of intraoral service. 2. 5 . 11 The physical properties of porcelain and ceramic are comparable to those of dentin. The material is chemically inert; thermal expansion is similar to that of the tooth.12 Horn 13 and Calamia and Simonsen 14• 15 conclude that porcelain etching enhances retention and thus produces a clinically stable and durable blood with the dentin. Wenn der Patient seine Probleme im Mundbereich nicht mehr kommunizieren kann und auf Pflege angewiesen ist, sieht sich der behandelnde Zahnarzt mit größeren Problemen konfrontiert. Ziel dieses Beitrags ist, die häufig massiv erschwerte (Schmerz-)Diagnostik bei geriatrischen und häufig multimorbiden Patienten zu erläutern. Ferner werden die wichtigsten Risikofaktoren und deren Einbeziehung in die chirurgische Behandlungsplanung aufgezeigt.
Eine der häufigsten Strategien zur Heranführung an die Zahnlosigkeit ist die Immediatprothese. Grundvoraussetzung für die Akzeptanz dieser neuen Struktur ist ein hohes Maß an Adaptationsfähigkeit vonseiten des Patienten. Diese ist allerdings bei älteren Menschen aufgrund physischer und psychischer Defizite häufig nicht gegeben. Eine Alternative zu diesem Vorgehen stellt die Versorgung mit einer Adaptations- oder Aufbauprothese dar. Im vorliegenden Beitrag werden die Vorteile dieser Art Prothese aufgezeigt. Weiterhin wird das praktische Vorgehen von der Planung bis zur schrittweisen technischen Umsetzung erläutert.
Ältere Personen und speziell multimorbide Patienten weisen häufig eine eingeschränkte oder unzureichende Immunabwehr auf. Bei ihnen sind deshalb Mundhygiene und ihre sachgemäße Durchführung besonders wichtig. Dies bereitet aber in Senioren- und Pflegeheimen große Probleme. Anhand eines Fallbeispiels wird verdeutlicht, welche Möglichkeiten und Grenzen der Zahnarzt in Zusammenarbeit mit dem Pflegepersonal hat, Schmerzen zu lindern und eine suffiziente Mundhygiene zu etablieren.
Dentale CAD/CAM-Techniken sind computergestützte Fertigungsverfahren von Zahnrestaurationen. Mit diesen Verfahren sind Prozessketten von der digitalen Intraoralaufnahme bis hin zur NC-gefrästen Verblendung prospektiv planbar und der Workflow von der Abformung über das Modell bis zur Gerüstfertigung standardisiert und reproduzierbar. Im Vergleich zu konventionellen Techniken im ZT-Labor ist der Arbeitsaufwand geringer und kann effizienter gestaltet werden.
Die Versorgung mit Implantaten hat sich in den letzten Jahren als Standardlösung bei der Versorgung von Lückengebissen etabliert. Der Einsatz von CAD/CAM-Systemen bietet hierbei eine Vielzahl an Möglichkeiten der technischen Umsetzung. Der allgemeine Trend geht hin zu Vollkeramikabutment und Vollkeramikkrone. Im vorliegenden Beitrag werden einige Problemstellungen zum Procedere aufgezeigt, wie Materialauswahl, Implantatstellung und Achse, mögliche Spaltbildung zwischen Implantatabutment und Krone sowie Befestigungsart und -material, sowie Fragen der Kosteneffizienz.
Der Computereinsatz in der Zahnarztpraxis hat sich in der Praxisorganisation seit vielen Jahren bewährt. Ebenfalls bewährt hat sich seit zwei Jahrzehnten die lichtoptische Intraoralabformung zur Vorbereitung der Einzelzahnrestauration im Chairside-Verfahren (Cerec). Neue Scannersysteme und Computersoftware ermöglichen heute die Intraoralabformung der Ganzkiefersituation und berechnen virtuelle Arbeitsmodelle für die zahntechnische Verarbeitung von Verblendkronen und mehrgliedrigen Brücken. Dadurch entfällt vielfach der Elastomerabdruck, sofern die Präparationsgrenzen sekretfrei und gut einsehbar sind. Untersuchungen haben gezeigt, dass die Genauigkeit der digitalen Intraoralabformungen zumindest jener von Polyetherabdrücken entspricht. Auch die marginale Passung am Kronenrand kann durch die Digitaltechnik beeinflusst werden. Der Nutzen der digitalen Prozesskette ist die exakte Reproduzierbarkeit der Ergebnisse. Die Datensätze vereinfachen im Online-Austausch die Kommunikation zwischen Zahnarzt und Zahntechniker.
Intracoronal restorations have a variable risk of cusp fracture depending on the amount of tooth structure remaining. All-ceramic restorations with acusp-supporting design could be a long-lasting alternative. The aim of this study was to verify the clinical reliability of occlusal veneers. A total of 286 occlusal veneers made of Vita Mark II ceramic that were fabricated and inserted in 244 patients from 2003 to 2004 using the Cerec method with an optimized ceramic design were periodically re-assessed over the following years. Based on the fracture rate observed over 93 months of follow-up, the probability of survival was 99.3%, demonstrating the excellent practical suitability of the method. In light of this, it is worth reconsidering the question of treatment with intracoronal restorations versus occlusal veneers in the posterior region.

Zahlreiche Problemstellungen auf dem Entwicklungsweg galt es zu lösen, sind gelöst worden, und einige beschäftigen uns auch heute noch in der praktischen Umsetzung. Der Einsatz von CAD/CAM-Systemen ermöglicht nicht nur die Anwendung von hochfesten Keramiken, sondern auch eine Vielzahl an Möglichkeiten der technischen Umsetzung. Aus diesem Grunde gibt es auch von den unterschiedlichen Herstellern verschiedenartige Lösungsansätze.
Lava Ultimate Resin Nano Ceramic (RNC) blocks are innovative new CAD/CAM materials that make it possible to achieve superior esthetic results in easy steps. The blocks are made of nano ceramic particles embedded in a highly cured resin matrix. Therefore, composite materials can be used to characterize and adjust resin nano ceramic restorations after milling. The milled RNC restorations can be individualized intra-orally or extra-orally, either before or after insertion. Unlike conventional ceramic restorations, customization and glaze firing is neither necessary nor possible with RNC restorations. This opens up the opportunity for intraoral individualization and adaptation of the restorations.
Amelogenesis imperfecta refers to a hereditary dysplasia of the enamel. As a result of various defects, qualitatively and/ or quantitatively abnormal enamel forms, while the dental structure remains normal. The following article describes the condition and presents the case of an 8-year-old boy who was dentally reconstructed both functionally and aesthetically using the adhesive technique and machinable high performance polymer restorations.
The multipiece nature of different implant systems is problematic for a number of reasons. The greatest challenge is bacterial colonization of the implant and of the surrounding tissue, because of the implant/abutment and abutment/crown interfaces, as well as the micromovements of the individual components of the implant. A possibility of eliminating microgaps between the individual components of the implant on the one hand and avoiding micromovements on the other is described in this paper.
