The purpose of the study was to find out the outcome of multifetal pregnancy reductions (MFPR) in relation to the starting and finishing number of fetuses. 373 consecutive transabdominal reductions (1998–2006) were reviewed. The mean maternal age was 34.7 ± 5.0 years. Initial numbers of fetuses: 72 twins, 203 triplets, 203 quadruplets, 17 quintuplets and 11 sixtuplets. 122 pregnancies were reduced to singleton, 258 to twins and three to triplets. Average gestational age at the procedure was 12.0 ± 2.1 weeks. Three groups were created: Group 1: good outcome—liveborn babies or/and stay of at least one of the babies for seven or fewer days in the NICU. Group 2: moderate outcome—at least one liveborn baby or stay of at least one of the babies for more then seven days in the NICU. Group 3: adverse outcome—complete pregnancy loss in utero or shortly (30 days) postpartum. Breakdown by the initial number of fetuses (Chi square test P = 0.01). (1) Successful outcome of pregnancy is dependent on initial number of fetuses. (2) Good outcome is inversely related to the final number of fetuses.
