•Our population consisted of acute coronary syndrome and stable CAD patients.•We compared the diagnostic accuracy of coronary MDCTA in these two groups.•Coronary MDCTA demonstrated similarly good diagnostic accuracy in both groups.•CAC score wasn't useful to exclude the presence of significant CAD in ACS patients.
