This case illustrates a ventricular fibrillation due to documented diffuse coronary vasospasm leading to necrotic and ischemic lesions, documented by cardiac magnetic resonance (CMR). This clinical report emphasizes the role of CMR to identify myocardial tissue abnormalities explaining that coronary vasospasm does not always have a benign prognosis, sometimes requiring implantable cardioverter-defibrillator. Moreover, the CMR allows the visualization of papillary involvement muscles representing a main substrate for ventricular fibrillation.
