•We examine the effect of the Danish fat tax on consumption patterns and IHD risk.•The total sales of twelve taxed foodstuff categories decreased by 0.9%.•The intake of saturated fat, unsaturated fats and dietary cholesterol decreased.•We estimated that the risk of IHD increased by 0.2% or decreased by 0.3%•We argue that taxation-subsidy schemes are more likely to benefit public health.
