Aspects of TB Epidemiology in Healthcare Workers in Romania 2010-2013

SESSION TITLE: Chest Infections Posters: Tuberculosis SESSION TYPE: Original Investigation Poster PRESENTED ON: Wednesday, October 28, 2015 at 01:30 PM - 02:30 PM PURPOSE: Since 2002, the incidence of active tuberculosis (TB) in Romania is continuously decreasing: 142.2% 000- 2002, 105.5% 000 - 2008, 72.9% 000 - 2013. The risk of infection and active tuberculosis is higher when there is increased exposure combined with inadequate infection control measures. The aim of our study was to establish the TB incidence in healthcare workers (HCW) in respiratory diseases between 2010-2013 in Romania and to follow the evolution of these cases compared by the TB cases in general population. METHODS: Using data from the National TB Control Program, we analyzed the evolution of the incidence and proportion of TB cases occurred among the HCW in respiratory diseases network between 2010 - 2013. The key epidemiometric indicators used to reflect the evolution of TB in this group were: incidence, calculated as the number of patients identified during one calendar year versus the general population which is the source of the cases during one year; bacteriological confirmation rate calculated as the rate of bacteriologically confirmed cases of all the studied cases; therapy success rate, calculated as the ratio of the number of patients with cured pulmonary TB or with completed treatment to the number of patients declared with recurrent pulmonary TB. RESULTS: The incidence of TB among HCW decreased (255.5 %ooo-2010, 314.46%ooo in 2011 and 176.08%ooo in 2013), but it remains double that in general population. Percent of HCW decreased among patients diagnosed with TB from 0.1% in 2010 to 0.05% in 2012 and 2013. Most affected are women, given the fact that in romanian health system works especially women. New cases represented more that 80% of cases in 2010 and 2011 (84,61% - 2010, 93,75% - 2011), but the percent of relapses is increased in 2013 - 45%. If in 2010 and 2011 all cases were with pulmonary TB, in 2012 and 2013 appeared the extrapulmonary localisation of TB. The percent of the source cases with smear positive decreased from 57,14% - 2011 to 16,66% - 2013. The percent of succes rate of treatment is more than 85% in all these years (92,3% - 2010, 93,75% - 2011), but in 2012 the mortality was 11,11%. CONCLUSIONS: TB remains a very important occupational risk for healthcare workers in respiratory diseases networks but the incidence of the illness is decreasing in the last years. Also, the succes rate of these cases treatment is higher that 85%. CLINICAL IMPLICATIONS: Administrative infection control measures appeared to have impact, but it is important to be continued in the next years.
