Psöriyazise Eşlik Eden Membranöz Glomerülonefrit

OZET Psoriyazis; cilt ve eklemleri tutan, kronik inflamatuar bir hastaliktir. Eslik eden bobrek tutulumu; basta asemptomatik hematuri ve proteinuri olmak uzere daha once tanimlanmistir. Primer glomerulonefritlerle iliskisi oldukca nadirdir. Psoriyazise eslik eden glomeruler tutulumun etiyolojisine yonelik yapilan calismalarda; farkli immunolojik ve genetik faktorler suclanmistir. Ancak; psoriyazis ile iliskili bobrek tutulumunu gosteren herhangi bir spesifik histolojik lezyon tanimlanmamistir. Psoriyazise eslik eden bobrek tutulumu, genellikle eklem tutulumu olan hastalarda daha belirgindir. Bobrek tutulumundan siklikla hastaligin tedavisinde kullanilan sistemik etkili ilaclara bagli gelisen renal toksisite sorumludur ve glomeruler tutulum sik gozlenmez. Bu calismada, psoriyazis tanisi ile izlenen, nefrotoksik ilac kullanim oykusu ve eklem tutulumu olmayan ve membranoz glomerulonefrit saptanan bir olgu sunulmaktadir. Renal involvement comparising mainly of asymptomatic hematuria and proteinuria have been re - ported previously. Primary glomerulonephritis is not common during the course of psoriasis. Various studies have been undertaken in an effort to confirm the etiology of glomerular disease as - sociated with psoriasis. There are also arguments supporting the role of underlying genetic and im - munologic mechanisms for disease pathophysiology. The cause of the nephrotoxicity is the renal toxicity of the systemic medications that are renal disease is administrated for the treatment of pso - riasis. In this report we present a patient with membranous glomerulonephritis who previously di - agnosed as psoriasis, without use of previous systemic nephrotoxic medications or a history of joint involvement.
