Patients are seldom engaged to elicit their primary needs or concerns during acute care episodes, often resulting in sub-optimal transitions home from the hospital due to persistently unmet patient needs. Our goal was to engage patients during an acute care visit to identify individual-defined priority outcomes and goals upon discharge from the emergency department (ED) and to identify individually tailored interventions that may be used to achieve their outcomes.
