Donor-specific antibodies present at the time of kidney transplantation in immunologically unmodified patients increase the risk of acute rejection.

•Higher HLA class II DSA before transplant associated with increased risk of rejection.•Assessment of cumulative class II DSA levels before transplant may be helpful.•Further studies needed to establish if this helps in prolonging graft survival.
