•PNH-type cells and HLA-allele lacking leukocytes were detected in patients with late graft failure (LGF).•Two patients with PNH-type cells achieved transfusion independence after antithymocyte globulin therapy.•Immune mechanisms similar to aplastic anemia underlie half of the patients with donor-type LGF.
