•The number of melanoma cells in sentinel nodes (SN) determines melanoma spread to non-sentinel nodes.•Complete lymph node dissection for staging can be replaced by quantitative SN immunocytology.•A model combining quantitative SN immunocytology and tumour thickness improves outcome prediction.
